AVID is Advancement Via Individual Determination, is a nonprofit college-readiness program designed to help students develop the skills they need to be successful in college.

Change Makers 45 students To provide Howard Bishop Middle school students with opportunities to become Change Makers in their community with lessons and activities based on public health community needs assessment.

Exploravision All of 7th grade This project is to encourage our students to use their imagination in order to think of future solutions to problems that exist in the world.

Tutor Teach Campus wide Is a volunteer tutoring program were students can go from 3:47-5:00pm to get help with their homework. Students must stay for 30 minutes to participate in the program. Various subjects

Vision Board Project All 8th grade students (185) To increase 8th grade students who are applying for magnet high school programs. Howard Bishop Middle school partner up with MOTIV8U to assist the 8th graders with leadership, attitude, relationship, goal setting & attainment, and excellence. The purpose of these practices is to help with their application process.

TITLE SERVING DESCRIPTION

Body Mass Index Screening – Florida Department of Health

BMI is a formula that uses a student’s height and weight to determine if his/her weight is in a normal range for his/her age. A higher BMI can contribute to the development of health problems. Students who fall outside of the normal range for his/her age group receive a referral for resources.

Endeavour students in grades Kindergarten, 1st, 3rd, 5th and 6th grades

Case Management Assistance with referrals for medical, dental and vision services. In conjunction with Brevard Health Alliance, we assist with appointment scheduling, transportation/accompaniment to medical / dental appointments.

Endeavour CPS students and families

Crisis Support Emergency Provide immediate support to children with severe emotional disturbance, emotional disturbance or those at risk of developing emotional disturbance to help them remain successful in the least restrictive setting possible via (crisis support, outreach) as evidenced by (phone calls, referring, crisis intervention).

Endeavour CPS students 11 – 14 years old

Dental Sealant Program – Florida Department of Health

A dental hygienist performs a screening exam, applies sealants and fluoride to students’ teeth to prevent cavities.

Endeavour CPS students in the 2nd and 6th grades

Mental Health Support (LLC Brevard) School Based Mental Health Counseling Mental Health Referrals for Endeavour, Mental Health Counseling (individual, family, and group), family support, crisis intervention, social emotional skills (individual and group), parenting support and resources

Scoliosis Screening – Florida Department of Health

Screening exam for curvature of the spine. Students who do not pass the screening receive a referral to an outside provider for follow-up.

Endeavour students in grade 6

Vision and Hearing Screening – Florida Department of Health

Students receive screenings for visual and hearing impairments. Students not passing initial screens receive a referral to an outside provider for follow up.

K-2nd Grade students In-school and out-of-school reading/writing tutoring program to enhance students’ literature attitudes and competency skills.

G3 Life Applications Tutoring and Mentoring Club

4th-5th Grade students Mentoring and tutoring program that provides students additional instructional support designed to target the needs of the students. Mentoring sessions engage students in the topics of health and wellness, group building, and team sports.

Girl Scouts of West Central Florida K-8th Grade students Deliver the Girl Scout Leadership Experience (GSLE) to build courage, confidence and character in the girls that participate in the program. GSLE program topics include: Girl Scout “Journeys” and skill-building in the areas of STEM (science, technology, engineering & math), financial literacy, healthy living, and career exploration. Our core curriculum “Journey’s” is differentiated by grade level and geared towards the three keys to girl leadership: discover, connect and take action which are based on research from the Girl Scout Research Institute that supports girl academic and leadership growth.

YMCA After-School and Summer Day Camp Program

K-8th Grade students The Sulphur Springs YMCA works with the SSK8 school and community partners to promote academic success through expanded learning programs and enrichment clubs that encourage students to develop new interests and skills. The Sulphur Springs YMCA provides out-of-school programming year-round support to Sulphur Springs K-8 Community School students. YMCA services are delivered daily during the school year and during the summer.

YMCA Mini-Camp SS YMCA After-School Participants K-8th Grade

The Sulphur Springs YMCA is hosting off-site mini-camps for currently enrolled after-school participants. Mini-camp groups are limited to ensure we are following social distancing protocols. Mini-camp will include rotations of YMCA staff-led academic enrichment, physical activities, social emotional activities and breakfast/lunch.

YMCA Reads! K-3rd Grade students (50 Students) The YMCA Reads! Program, a reading remediation and mentoring program designed to close the achievement gap in students’ reading performance and aims to increase students’ reading skills. Daily tutoring is provided to selected students during school time and out-of-school time.

Color Me CONFIDENT, Inc 10-25 students A non-profit organization dedicated to building CONFIDENCE in young ladies through expressing the value of self-worth, education, family values, life skills, and mentoring through discussion and enriching activities.

Form 2 Fashion 10-20 students Is a non-profit organization that we are partnered with that conducts workshops once a semester about the fashion industry and how economics operate. This workshop encourages students to think critically about products and consumerism and the math that goes along with this trillion dollar industry.

Men of Royalty 10-15 students Their mission is to improve the quality of life of youth who face delinquency, abandonment, poor academics and many other facets that decrease their chances of becoming productive citizens. The goal is to transform youth by teaching them to exhibit “King & Queen-like”mannerism and to live elite lifestyles. Men of Royalty promotes an environment that encourages youth to live a life of distinction and to be men and women of valor and does so by discussion, guest speakers, and activities.

Microsoft Store at Florida Mall 10-20 students We collaborate with Elevate Orlando to transport students to Microsoft Store at Florida Mall once a month. The workshop is from 3:30-4:30pm. Microsoft conducts workshops of all sorts such as coding games, starting a business, starting a nonprofit, professional building, and more.

Professional Opportunities Program (POPs) 10-15 students Provides workshops throughout the school year dedicated to specific topics. Students learn the basics of resume writing, interviewing, workplace etiquette, professional dress attire, money management, deciphering a paycheck; finding the best college match, paying for college and applying for scholarships, civic responsibility and engagement, career exploration, majors and curriculum and networking. Many of the workshops are interactive so that students have an opportunity to practice the skills learned through role play. When faced with a real life situation our students will be ready.

Queen of Roots 10-20 students A partnership with a local professional Orlando natural hair salon, Queen of Roots. She comes in once a month to conduct natural hair workshops and goes over topics such as the science of hair, safe products to use, how to style natural hair, braiding, skin regimen, etc. We want to encourage our African-American students to be proud of their natural hair and take care of their hair and skin in a healthy manner.

The Future Leaders United (FLU) Program 50-60 students Exposes the students to the STE(A)M education, college readiness, life beyond high school, and leadership through service to the community. Bridging the gaps in both educational and mentoring needs of the Pine Hills Community using a holistic approach.

Trojans Do Good 10-20 student A partnership with Central Florida Urban League where we devise monthly community service opportunities in house that will positively affect our local community and allow students to receive community hours

Wells Fargo Workshops 10-20 students A partnership with 2 Wells Fargo sites in Pine Hills that come to our campus once a month and provide financial literacy workshops that high school students can relate too and want to learn about. They go over how to budget, types of loans, bank jargon, how to save to buy a car, how to start a business, the value of money and more!

YMCA Teen Achievers 25 students Expose and provide high school students the assistance to unlock academic and career doors. The goal for students is to achieve success in all aspects of life within their school years. Through this program students volunteer regularly every month and complete activities that will help them in the college or career realm

Parent and Family Engagement Parents PEP is a program that empowers parents to share ideas, resources and information to other parents in their network and neighborhoods. Over all the program’s goal is to empower parents to assist with change in other parents’ lives through engagement

ECPS Volunteer (Non-Mentor) Entire Assist with the day-to-day administration of The HUB, and provide support for regular Evans Community Partnership School staff. Volunteers are also encouraged to follow, share, and repost information from the official The HUB social media account.

On-Campus Mentor Program 83 Students Provide a positive role model for students through the Evans Community School Mentor Program.

Parent Teacher Student Association (PTSA) Parents, Teachers and Students This group provides members with education, leadership training and a powerful voice of advocacy. Overall the group aims to better the lives of the students who attend the school.

School Advisory Committee (SAC) Parents, Teachers, Staff, Students and Community Members

Members learn what is happening with the School Improvement Plan and let their voices be heard for important decisions that impact students. This council is a group that brings representatives of stakeholders together to assist the school principal with developing and evaluating a school improvement plan. Each SAC is composed of the principal and representatives of teachers, education support employees, parents, community members, and students. Teachers, education support employees, students and parents are elected to the SAC by their peer groups. Community members are appointed by the principal.

TITLE SERVING DESCRIPTION

Clothing 9th-12th Students who are in need of clothes are able to receive assistance in the HUB. Name, size, and other materials are needed for the Social Worker to put in the request through the Kids Closet. Once clothing is received from the Social Worker, a student is to be called from an elective class to pick up the items.

Food Pantry 9th-12th Food services are provided to the students through the Food Pantry on campus. Food is ordered through Second Harvest and picked up. *Families who are not receiving Food Stamps services-Wellness Coordinator can get with them and do an ACCESS application with them through the DCF website. DCF will determine whether the student is eligible or not.

Vision 9th-12th All 9th graders receive a vision screen through Vision Quest. Vision Quest schedules the date and the coordinator finds a location on campus for the screening to take place. Students from 10th-12th grades can receive vision service by providing them with a Vision Voucher depending on the students’ situationThose who have insurance can attempt going through their insurance first. Have a conversation with the students’ guardian to make an appointment for the student. Vision Vouchers can be obtained through the Nursing Association and this is by partnering with them through a Nurse that is on campus.

Wellness Center (Dental) 9th-12th and community Dental services are provided by Orange Blossom Family Health. Dental Unit is open on Tuesdays only. The Mobile unit is able to do screenings while the regular unit will provide services such as screening, extractions, and fill-ins. If a student is experiencing pain, appointments should be made immediately with Rae-Nesha for one of their off campus locations.

Wellness Center (Medical) 9th-12th and Community Medical services are provided by Orange Blossom Family Health. They are open Tuesdays and Thursdays.*Students who don’t have insurance can get with Wellness Coordinator to do an ACCESS application with them through the DCF website. DCF will determine whether the student is eligible or not.

Southeastern University (SEU) Reading & Diagnosis

2nd & 3rd Grade students A tutoring program that occurs during the school day, three students per class are selected by their teachers to participate. SEU education majors provide the instruction.

Mobile Food Pantry Student’s families and community members Food pantry providing each family a weeks’ worth of fresh food (variety of fruits, vegetables, meats) that will feed a family of four.

One More Child Students Weekend food bag program that provides two breakfast and two lunch meals for students identified as in need of food support.

SALT: Sensory Academy of Learning & Technology

K-5th Grade students SALT provides sensory activities to assist students in learning self-regulation in a calm, relational setting and to further assist them in transferring their knowledge to the classroom and family settings.

School Meals Distribution Students and community children under the age of 18

Grab and go lunch/breakfast meals provided through the Polk County Public School Nutrition Department.

This site has a school and summer program, but offers extended day services to other sites.

Dental Sealant Program Prek-5th Sealant program in collaboration with the Health Department.

Supplemental Nutrition 200 students. Blessings in a Back pack has been our program for a number of years to provide weekend food bags. Services are now being provided in a drop off format.

Vision Prek-5th Screening and glasses for students in need. In collaboration with the Lions Club.

Wellness Care (Tele Med) Students and families Wellness services are coordinated and provided through Flagler Health Plus
