INTRODUCTION

Ethiopia is one of the least urbanized countries in the world; with as much as 80% of the population residing in rural areas. The

dispersed dwelling of the population and lack of utilities in these areas (Electric power, water supply, and Waste disposal) has made

it challenging to deliver health services such as Maternity care, Vaccination and Reproductive health. Exceptionally pressing is the

difficulty of providing Perinatal and postnatal care for women and infants in rural areas (including delivery).

Usually, patients are carried to health centers either on horseback or carried by people. This is not a safe way for a pregnant woman;

nor is it taken as appropriate [by the society] to carry a ‘healthy’ woman regularly to health center. So women have to travel long

distance (>5km) to nearby health centers if they seek prenatal care. Meeting a doctor (an obstetrician) requires travelling even longer

to nearby towns. After birth, also, a woman has to travel the distance carrying the infant on her back. This is a serious

discouragement for mothers in rural areas and significantly contributes to maternal and child morbidity.

Under the Health Extension Program, the Ministry of Health has been putting huge effort to avail health care and ambulance services

to rural communities. Due to the highly dispersed dwelling pattern and poor transport infrastructure, however, it has become difficult

to increase access and utilization of health services. It is even more challenging to address the health problem in pastoralist areas,

where the population is highly mobile. So the traditional approach of opening clinics alone is not sufficient to make health services

accessible nationwide.

MoCLAV is a mobile clinic built on a heavy-duty truck and furnished with all the facility required to provide medical service inside. The

mobile clinic can accommodate a doctor, a laboratory technician and a patient inside. It enables medical team to travel to remote

areas easily and deliver health services. With all equipment inside, the vehicle can be used as diagnostic or treatment room at any

location. Health officers can use MoCLAV to reach out to patients in remote villages and provide health service in-house or in-

village. Since MoCLAV is equipped with proper communication and signaling equipment, it can be used as an ambulance in the

event of emergency.

FACILITIES

The Mobile Clinic is manufactured locally by retrofitting a standard truck in Cab-

Chassis form. The first work is to create a comfortable treatment room on the

truck cargo. Then after, it is fitted with required Clinical Furniture, Medical

Equipment, Electrical System and Sanitary System. The following are major

facilities provided by the mobile clinic.

1. Room (Floor, Wall and Roof): Exterior of the

compartment is built from sheet metal and painted.

For the interior, various options are available

depending on the customer’s need (PVC, Fiber glass,

timber or composite). The wall is built to have proper

thermal and sound insulation. Additional treatment is

made to make all walls water proof.

2. Door: A gate with double door has been provided as

primary entrance to the clinic for patients as well as

medical staff. One of the doors is used only move

stretcher/ patient bed directly into and out of the

mobile clinic. There is a foldable aluminum stair with

four steps which can be collapsed during travel and

stretched when parked.

3. Furniture: Once the compartment is established on the truck, the required

furniture is put in place. The following furniture are required.

4. Room Conditioning: To make it appropriate for medical service, the room should have conditioned environment (Temperature, Ventilation,

and Lighting). Two lockable windows have been provided in order to allow air circulation and natural light in the room. Electrical lamps are

also provided for additional lighting. In case the need arises, there is a provision for a small air conditioning unit.

In order to allow medical service delivery even in the absence of utilities, the Mobile Clinic has been fitted with standalone Electrical Power, Water

Supply and Waste Disposal Systems. Tailored solutions that can be fitted to the vehicle have been developed.

I. Electrical Power supply: Electrical power is required to operate

some Diagnostic and sterilization devices. Power is required for

communication, signaling and Data recording also. A laptop is

expected to be available for use by medical personnel. In order to

power all these equipment, Custom-made Solar Power System has

been developed. Solar panels on the vehicle roof collect energy and

store it in a battery bank. The energy stored in the battery is used for

buffering as well as for night time operation. All required elements

such as charge controller, Inverter and power outlets have been

installed and tested.

II. Water Supply & Waste Disposal: To satisfy mandatory need for clean

water, a food grade water tanker has been built from stainless steel sheet

and installed. Water flow from the tanker is controlled by a gate valve.

Three water outlets have been provided – one at laboratory desk, one for

hand wash and one is a hose for washing the patient. Waste accumulation

and disposal mechanism is equally important. Aluminum sink and sewage

tank for waste water and other fluid waste are basic requirements. The

sewage tank should be air tight with no risk of contamination. We have

designed a compact sewage tank with safe intake and disposal

mechanism.

III. Signaling, Communication and Data recorders (Optional): Another category of

devices required in MoCLAV are electronic equipment that improves its accessibility

and safety. Siren and beacon light are installed to MOCLAV as signaling

mechanism, especially when it is used as an ambulance. The primary means of

communication to be installed in MOCLAV is cell phone with solar charger. Radio

and Satellite communication can also be installed to allow communication at remote

locations. Other optional features include GPS tracker and Data logger.
