A COPD Action Plan is written instructions on what to do when you have a flare-up.

Your COPD Action Plan will tell you which medications to start taking, when you should contact your doctor and when to seek emergency care.

The Action Plan is designed to help prevent and treat COPD flare-ups.

The COPD Action Plan is an important part of learning to self-manage your COPD. It allows you to get treatment for flare-ups based on orders from your doctor without actually needing to go to the clinic.

This important tool has shown to decrease emergency room visits and hospitalizations in people living with COPD.
