Create a survey using Google Forms

Set up a google account and go to this link - https://docs.google.com/forms

Give your survey a title and choose the type of question

Preview the survey by clicking on the eye.

Click Send via email or create a link to share on Facebook or email.

To view the responses open Google Forms and click on your form. You can view responses in a graph or see

individual responses.
