Please be advised that even though your Mattress and Box Spring have a longer warranty for an exchange if deemed necessary, there is only a twelve month warranty for service, which begins at the date of purchase. The paperwork attached is the first step in the mattress claim process that works as follows:

1. Read the directions and fill out the claim form. 2. Mail it to the address shown. 3. The form will be reviewed. 4. If a decision is reached right away, you will be contacted. 5. If we need further information, or measurements, we will set up a service date. 6. If it has been over one year since the date of purchase, there will be a $60 trip charge for this service.

7. There are no exceptions to the trip charge.
