Neuroendoscopic treatment of the midbrain abscess: a case report
Introduction

• Midbrain abscesses are rare and were fatal until recently. • The optimal treatment is not yet defined. • It can be managed by pharmacological treatment alone, stereotactic
aspiration with pharmacological treatment, or surgical removal/drainage of the abscess.
• We applied a neuroendoscopy to treat a large midbrain abscess. • .Pathohistological examination confirmed cerebral abscess. Conclusion:
Purely neuroendoscopic procedure can be a permanent solution for treating large midbrain abscess without complications and with good outcome. Neuronavigation is a useful tool,
Case report
A 50-years old male presented with progressive pupil-involving third nerve palsy, contralateral hemiparesis and dysarthria.
Magnetic resonance imaging revealed a 2.6 cm x 1.8 cm ring-enhancing lesion in the left side of the midbrain and left cerebral peduncle with associated edema

Case report - operation
Neuroendoscopic removal of the lesion was performed through the right lateral and third ventricle using a rigid endoscope with neuronavigation.
Pathohistological examination confirmed cerebral abscess.

His neurological condition gradually improved, no new neurological deficits appeared.
MRI 2 months after the operation showd no sign of abscess.

Conclusion
• Purely neuroendoscopic procedure can be a permanent solution for treating large midbrain abscess without complications and with good outcome.
• Neuronavigation is a useful tool which increases the accuracy and safety of the surgical procedures.
