The open enrollment period for voluntary benefits will extend from June 23 to July 25, 2014. The enrollment period and the online enrollment website will close at midnight on July 25, 2014. All benefit and payroll changes will be effective September 1, 2014. Only qualified life event changes,  per IRS guidelines, will be permitted after that date. 

You will find detailed information on all benefits offered in the Reference Center (www.benefitsolver.com). You will also find instructions at the end of this summary on how to enroll using the website and how to look at plan information in the Reference Center. The Reference Center is available to you year round as a resource.

TCG Benefits Advisors will be available for enrollment assistance at your campus from June 23rd to June 27th from 7:30 a.m. to 4:00 p.m.

STAFFORD MSD OPEN ENROLLMENT 2014 - 2015 PLAN YEAR

This year, we have some changes including NEW supplemental  benefits, rate changes and plan  changes. The new plans you will be able to elect this year include iLOCK360 (an identity  protection plan), a Colonial medical bridge plan and a MetLife accident plan. We also will  be  switching  vision  providers, going from Block Vision to Eyemed Vision.    We will have a slight rate increase on the Dental DHMO plan.  All other  rates will remain the same, and all other benefits will roll forward on existing plans to next year except for flexible spending accounts. Flexible spending accounts must be re-elected each year.  

You are able to enroll your eligible dependent child(ren) up to age 26, whether or not a full-time student. An eligible dependent child is an unmarried child that you claim as a dependent on your tax return and one for which you are providing 50% or more of their support. Only your medical coverage allows you to enroll your child(ren) up to age 26, whether or not married or a tax dependent. Please confirm that your dependent information is entered correctly in the system for all your dependents, including their Social Security numbers, as TRS ActiveCare requires SSNs for dependent coverage.

No benefit changes will be allowed after July 25, 2014. All insurance coverage selected during open enrollment will be deducted from the September paycheck and be effective September 1, 2014. Benefits selected during this Open Enrollment period will remain in effect until August 31, 2015.

Medical PlanMedical Insurance is handled by TRS

STAFFORD MSD OPEN ENROLLMENT 2014 - 2015 PLAN YEAR

Major medical expense coverage is available to all eligible employees and their eligible dependents. Employees who have questions on the medical insurance can refer to documents in the Reference Center on the benefits enrollment website (www.benefitsolver.com), or you can contact Stafford MSD Benefits Office at 281.261.9271. If your spouse is an educator at another TRS district, please contact your benefits office for your rates. Annual Enrollment is the opportunity for employees to enroll in a TRS-ActiveCare medical plan for the first time, add dependents, or move from one medical plan option to another.

For 2014-2015, the TRS-ActiveCare 1-HD and TRS-ActiveCare 2 plans are "Aetna Open Access Choice POS II" plans, which work very much like a PPO. NOTE: ActiveCare 3 plan will be discontinued for the upcoming plan year. Those participants currently enrolled in this plan will be automatically moved to ActiveCare 2, unless they enroll in another plan option.

There is a new plan option, TRS-ActiveCare Select, which is a network-only plan similar to an HMO. It is an “Aetna Select Open Access” plan. 

Use the Aetna DocFind to locate doctors, hospitals, urgent care facilities and other health care providers in your plan’s networks(s). The link is provided on the benefit website, www.benefitsolver.com.

Important Note regarding the NEW TRS-Active Care Select: For the Stafford, TX area choose the “Aetna Open Access TRS-ActiveCare Select” plan in the “Select a Plan” box on the Aetna DocFind link !Some of the new exciting changes for 2014-2015 plan year include:

• Aetna and Caremark are the new plan administrators replacing Blue Cross and Blue Shield of Texas and Express Scripts;

• A new plan option called ActiveCare Select; and • Aetna/Caremark TRS-ActiveCare Customer Service team available 24/7/365 via telephone 800.222.9205 or online

at www.trsactivecareaetna.com. Check to see which providers are in the new TRS-ActiveCare plan networks using this custom provider directory at

www.trsactivecareaetna.com. The online directory is available to search for providers in ActiveCare 1-HD, ActiveCare 2, and ActiveCare Select providers. !Some of the new and exciting plan features for the 2014-2015 plans include:

• Teladoc—24/7/365 phone access to a national network of physicians for non-emergency medical assistance. Teladoc physicians include general practitioners, internists and pediatricians. They can diagnose, treat and prescribe many medications for most common medical issues, such as the flu.

• Quest Diagnostics Lab Savings—Quest Diagnostics has agreed to lower lab rates for TRS-ActiveCare participants. The ActiveCare 2 and ActiveCare Select plans cover lab services at 100% when using Quest Diagnostics for lab work.

• New Maternity Program—Aetna’s Beginning Right Maternity Program nurses can help give babies a healthier start in life. Nurses offer free education about pre-natal care, pre-term labor, newborn care and more.

• New TRS-ActiveCare ID Cards—All participants will receive new ID cards from Aetna and Caremark. Cards will be mailed prior to September 1.

• TRS-ActiveCare is going paperless – TRS-ActiveCare Benefits Booklet will be available online only. The TRS-ActiveCare Benefits Booklet will be available September 1, 2014. Look for both at www.trs.state.tx.us and the Aetna TRS-ActiveCare website www.trsactivecareaetna.com. !

Medical Bridge Hospital Confinement/Medical Bridge Insurance by Colonial

The Colonial plan is a Hospital Confinement/Medical Bridge Plan designed to bridge the “gap” of the in-hospital deductible and co-insurance of your current health insurance with your out-of-pocket expenses. It offers financial protection for out-of-pocket costs that are related to a covered accident or a covered sickness, when being admitted to the hospital.

The plan offers two different types of benefits: a $1,500 Hospital Confinement Benefit with a maximum of one benefit per calendar year per covered person on the plan, and an Outpatient Surgical Procedure Benefit which can help cover the costs associated with covered surgical procedures, up to $1,500 per year per covered person on the plan. The plan will not pay for loss during the first 12 months after the effective date due to a pre-existing condition which means a sickness or physical condition for which a covered person was treated, had medical testing, received medical advice or had taken medication within the 12 months prior to the plan effective date. Dependent children are eligible up to age 26. There are two dental plan options from which to choose: !PPO Dental Plan by United Concordia The PPO dental plan requires you to pay a plan year deductible of

$50 per covered person and then share a percentage of the dental costs. The maximum benefit for the PPO dental plan is $1,500.  The plan allows for the freedom to choose any dentist or to select a PPO dentist from a list of providers and receive contracted lower pricing for procedures with no balance billing. The list does contain participating dentists in the Stafford area.

The deductible is waived for preventative and diagnostic care.  Preventive and diagnostic care are paid at 100% (includes cleanings, x-rays, and exams) and will not count against your plan year maximum, basic restorative care is paid at 80% (which includes fillings, root canals, gum treatments, and oral surgery), and major restorative care is paid at 50% (includes crowns, bridges, and dentures) for you and your eligible dependents under age 25. Orthodontics coverage is paid at 50% with a lifetime benefit of $1,000 on the plan for eligible dependents under the age of 19. !Dental HMO Insurance provided by MetLife

The DHMO plan is an affordable plan that features no deductibles, no pre‐existing conditions, and no claim forms. To use the plan, you must select a dentist from the providers list. Upon your visit, you pay a predetermined reduced fee directly to the dentist. DHMO providers are listed at www.metlife.com under plan SGX290.

Vision Plan by Eyemed

Eyemed is the new vision plan provider for this plan year. This vision plan offers an economical way to purchase eye examinations, eyeglasses, and contact lenses from member doctors. At the time of your visit, you will pay a $10 co-payment for your examination and a $25 co-payment for your glasses or contact lenses. There is an allowance of $125 for frames in addition to 20% off any amount over the allowance OR $150 for contact lenses in addition to 15% off any amount over the allowance. Cancer Plan by Allstate Insurance

This is a supplemental insurance plan that provides payments directly to you to offset expenses related to the treatment of cancer. Once approved for this coverage, the plan pays according to a schedule of benefits. Optional coverage is available for any condition when confined to an intensive care unit of a hospital. You are able to enroll eligible dependent children up to age 26.

Existing employees applying for this coverage for the first time, or those changing current coverage, will be subject to medical underwriting and approval by the insurance company. New hires—within 31 days of date of hire, and their eligible dependents, who enroll in the cancer plan—are guaranteed to be accepted without regard to prior cancer history. Certain pre-existing condition limitations apply for one year. A pre‐existing condition is any condition for which you received medical treatment, consultation, care or services, including medication, in the twelve months prior to the effective date of coverage. Pre‐existing conditions will not be

Disability Insurance by Standard

The disability coverage provides payment of up to 2/3 of your salary in the event you are ill or injured and cannot work. The cost depends upon the amount of coverage selected and on how quickly you wish benefits to begin. A variety of elimination (waiting) periods are available. The plan features elimination periods of 30 days or less that are waived when admitted to a hospital as an inpatient for at least 24 hours or more. Disability coverage will be issued to all eligible employees who apply during this open enrollment period without having to provide evidence of insurability. Pre‐existing condition limitations only apply on new levels of coverage elected or for any enhancements to the plan. A pre‐existing condition is any condition for which advice or treatment was received in the 90 days prior to the effective date of coverage. Pre‐existing conditions will not be covered until after twelve months of continuous coverage. !Group Term Life and AD&D Insurance by SunLife

The District pays for $10,000 of group term life insurance for each eligible employee. This Group Term Life and AD&D Insurance allows employees to purchase additional amounts of group term life insurance on themselves and their family members. An employee must have voluntary coverage on themselves to obtain coverage on their dependents. This plan is available through Sun Life Assurance Company of Canada. The plan provides for larger amounts of low-cost term life insurance while employed at Stafford MSD.

Existing employees that are applying for coverage for the first time or increases in their coverage will be subject to medical underwriting and approval by the company.

New employees and their dependents, within 31 days of hire, who apply for coverage are guaranteed five times their annual salary up to $150,000, $50,000 for a spouse, and $10,000 for all children in the family. To be eligible for coverage, employees and their family members must not be hospitalized or home-confined due to sickness, accident, or disability, or must not be diagnosed with a terminal illness. Additional amounts of group term life insurance are available subject to medical underwriting and approval by the company. Group Accident Plan Group Accident Plan by MetLife Insurance

MetLife’s accident insurance plan is designed to help protect against the high costs of unexpected accidents, both on and off the job. These affordable plans complement medical insurance and help protect your savings from unexpected expenses due to a wide variety of accidental injuries.

The plan provides a benefit for a wide variety of accidents, including fractured bones, third degree burns, concussions, coma, broken teeth, eye injuries, tor n/ruptured/severed tendons/l igaments, emergency room treatment, ambulance, hospital confinement (including intensive care), blood and plasma, and for catastrophic accidents. The plan is guaranteed to be issued to anyone who applies for coverage.

There are two options from which to choose: a Low Plan or a High Plan. The covered benefits are identical under each plan; however, the High Plan provides greater benefit payments. Individual Life Insurance Individual Life Insurance by Fidelity Life

Fidelity Life offers low cost life insurance that is designed to last a lifetime. Coverage is available for employees, their spouses, and their eligible dependents. The cost varies by age, gender, and the amount of coverage desired. The plan also contains a unique long-term care benefit. The policy allows for up to 4% of the death benefit to be used monthly for long-term care expenses. For example, a policy of $75,000 would pay $3,000 a month for up to twenty‐five months for long-term care expenses. Any amounts used for long-term care expenses are deducted from the death benefit.

Section 457 The 457 plan is a district-sponsored voluntary retirement savings plan that allows an employee to save money for

retirement on a tax-deferred basis. Your district has chosen to offer the Region 10 RAMS 457 Plan which has low-cost investment options. The Region 10 RAMS 457 Plan has investment options that do not contain any surrender charges or penalties upon distribution. Section 403(b)

Section 403(b) Tax-Sheltered Annuity Arrangements refers to Section 403(b) of the Internal Revenue Code of 1986. The 403(b) plan is a voluntary retirement savings plan that allows an employee to save money for retirement on a tax‐deferred basis. There are different types of investments and a number of representatives who are available to assist the employee in establishing a 403(b) retirement plan.

In 2014, you can contribute 100% of your includible compensation up to $17,500, whichever is less. Participants age 50 and over may contribute up to an additional $5,500 for a total of $23,000 for the year. Withdrawals are generally available after termination of employment. Distributions from the 403(b) Tax Deferred Savings Plan are subject to the 10% excise tax for early withdrawal. You can contribute to both a 457 and 403(b) simultaneously.

Consult the JEM website www.jemtpa.com for a list of approved companies for Stafford MSD. First time participants, please complete the following steps to set up your 403(b) plan:

• You must contact one of the approved companies or a representative for those companies to complete the 403(b) account paperwork with them.

• Then, you must go online to www.jemtpa.com to do your online enrollment. Your 403(b) deduction will only begin once you have completed the online enrollment. Your initial password to enroll online is staff403 (case sensitive) for enrolling in pre-tax deductions, sta403xR (case sensitive) if enrolling in Roth 403(b).

• Select the dollar amount to be deducted from your paycheck. • Indicate which company you are choosing for your new 403(b) account.

To make changes to your current 403(b) plan, go online to www.jemtpa.com sign in using your username and password, and follow the prompts. All transactions regarding 403(b) plans, including starting a new plan or making changes to contribution amounts to a current plan, must be done through the JEM website for it to become effective on your paycheck.

The Section 125 Cafeteria Plan Benefit refers to Section 125 of the Internal Revenue Code of 1986. Simply put, the Cafeteria Plan allows certain benefit premiums to be deducted from your gross earnings, before federal withholding taxes are calculated. The amount you elect to have deducted “pretax” lowers your taxable income. Please review the sample paycheck on page 17 for an illustration of how participating in the Cafeteria plan could work for you. Medical Expense Reimbursement Plan

This is a plan that allows for a tax savings on most medical, dental, and vision expenses not covered by insurance. Non-covered expenses apply to all dependent family members even if not covered by a particular insurance plan. The employee estimates an annual election based upon the amount of non-covered expenses to be incurred. The annual election amount is deducted in equal amounts from each paycheck, before taxes are calculated, and then set aside for the employee in a special account. A MasterCard debit card will be issued to you to pay for most expenses incurred. Where a debit card may not be accepted, the employee submits a claim and the money is reimbursed by check to the employee from the employee’s account. Please consult your employee benefits office or an Benefits Advisor for more details.

Note: Not all expenses are eligible for reimbursement. Any money in the medical flexible spending account that is not spent within the plan year will be forfeited. Claims for expenses incurred within the plan year must be submitted by the employee no later than ninety days after the end of the plan year or the money will be forfeited. The new maximum annual election amount is $2,500 due to the Healthcare Reform Act. The Reference Center (www.benefitsolver.com) has a list of eligible expenses. !Dependent Care Expenses

• NOT pre-funded

• $5000 maximum per household

This is a plan that allows for a tax savings on day care expenses for children under the age of 13 and for dependent adults unable to care for themselves. Section 125 Cafeteria Plan FAQsQ. What does this benefit program mean to me? This program means that current after‐tax payroll deductions for items such as medical insurance, dental, vision insurance, medical bridge plan, accident plan, dependent care costs, and some medical expenses not covered by insurance, can be paid with pretax dollars. The bottom line is that you may have more dollars available to purchase other benefits you may need, or increased take‐home pay. !Q. Must I make my election before the effective date of the Plan? Yes, you must make your benefit election during the annual open enrollment period. A newly hired employee will have 31 days to elect benefits. !Q. Can I make changes in my election during the plan year? Generally no. Once you have made your benefits election, you may not make changes during the plan year. The only exception is if you have a Family Status Change. Your request to make a change in a benefit must be consistent with the change in family status. Some examples of a family status change are marriage or divorce, the death of a spouse or child in your family, the birth or adoption of a child, or a change in the employment status of you or your spouse. STAFFORD MSD OPEN ENROLLMENT 2014 - 2015 PLAN YEAR

!Because we have selected a new benefits website and

providers, you will need to go through each benefit to select or waive each plan this year. After you have completed all the benefits you will be directed to the “Review Enrollment” page. You are still able to make a change to a benefit there also, click EDIT to the right of the benefit you wish to change or elect. After you review your benefits, please scroll down to the bottom of the page and click NEXT.

!When finished making your benefit change, scroll to

the bottom of the page and click NEXT. !You will return to the review page.

!When you are finished reviewing your enrollment

and making changes, click APPROVE at the bottom or top of the Review Enrollment page. Continue to the last step on the next page.

enrollment. If you do not click I AGREE, your enrollment will remain in progress and your changes will not be submitted. If you click I DISAGREE, your changes will be deleted.

After you click I AGREE, the website will ask you if you would like to print a benefit summary. Please print this for your records. A record of your enrollment will be available on the website all year.
