Concierge Appointment Hotline

The AvMed Jackson First concierge hotline offers Jackson employees under the Jackson First and Jackson Select health plan an opportunity to make appointments at Jackson Facilities and have their questions answered in a quicker and more convenient way. Some of the benefits include:

Hospitality and concierge services available 24 hours a da y, seven days a week. A direct appointment line and email address for all medical appointment needs. Appointment line business hours: Monday - Friday, 7 a.m. to 6 p.m. (holidays excluded) Three campus locations available to you for your convenience:

Pre - Appointment

A dedicated outpatient coordinator will confidentially assist and facilitate all arrangements needed to link Jackson employees with the outstanding medical at tention they require and deserve. The outpatient coordinator will support the employee through each step and confirm their requested appointment(s). In order to begin the appointment process, you will need to provide:

Registration forms Medical history an d respective reports Information indicating special needs (dietary, religious, special acces)

Once the information is received, our team obtains the earliest available appointment with the requested medical specialist. When the appointment has been confi rmed, the patient is notified, receives financial clearance, and is pre - registered in our system.

Your employer offers you the opportunity to save in a 403(b), a 457(b) deferred compensation plan or both. Because you can choose to contribute to one or both, you can select the plan with features that best suit your situation. What is the most you can contribute? How do the plans differ? There are some significant differences between the plans, especially when it comes to withdrawals. Consider these differences when deciding which plan will suit you best.

403(b) 457(b)

Withdrawals prior to age 59½ may be subject to a 10% federal early withdrawal tax penalty, unless an exception applies. Unlike the 403(b) plan, the 10% federal early withdrawal tax

penalty for withdrawals prior to age 59½ does not apply to distributions from 457(b) plans except on amounts rolled into the plan from non-457(b) plans — including IRAs.

Less stringent hardship withdrawal restrictions while you are employed.

More stringent unforeseeable emergency withdrawal restrictions while you are employed.

Examples of financial hardship include: • Certain unreimbursed medical expenses • Payments to purchase a principal residence • Qualifying expenses for higher education • Payments to prevent eviction from or

foreclosure of a mortgage on a principal residence

Examples of unforeseeable emergency include: • You or a dependent suffer an accident or

unexpected illness

• Loss of property due to casualty • Other similar extraordinary circumstances

arising as a result of events beyond your control Sending a child to college or purchasing a home, two common reasons for 403(b) hardship withdrawals, generally are not considered unforeseeable emergencies.

• Click “Employee Resources” tab in the Net Portal Website. Then click “Lawson Application Employee Self Service” tab.

• Type in your user name and password to login. User name and password is usually your network ID that you use to log in to windows.

• Select “My New Hire Process,” and then select “New Hire Enrollment.”

Before You Start Your Web EnrollmentPrior to enrolling in your benefits online, it is to your advantage to thoroughly review your enrollment materials.

If you are ready to enroll, but need assistance or have questions regarding your benefits, contact the on-site FBMC Service Center at 305-585-6512.

For assistance with your ESS log in, contact Jackson Health System IT Customer Service Center at 305-585-6789. Once you have the answers you need, you may begin the enrollment process.

Be sure to have the following information available before you begin the enrollment process:

• Social Security numbers (SSN) for all your dependents.

• Dates of birth for all your dependents.

• Proof of eligibility for all your dependents.

• Primary Care Physician (PCP) if electing health insurance.

Verify your demographic information and add dependents & beneficiaries, if applicable.

Click “Update and Enroll” and then “Continue” once you finish adding “your dependents’ and beneficiaries’ profiles.

4 Coverage

Choose your coverage level or election amounts for each benefit (e.g., Medical, Dental, Vision). Click “Continue” and “Add to Elections” unti l the enrollment is complete. * Note: You may save your enrollment session progress and return later to complete the enrollment

at any point by clicking on the “Save Elections.”

5 Review Elections

Click “Review/Submit Your Election” once you complete your benefits election. You will be given a description of your benefit selection.

6 Submit

Click “Submit Elections” to confirm your enrollment. Please print your Benefits Election page for your records. You will receive a confirmation email prior to your plan effective date

You may access the web enrollment 24 hours a day, 7 days a week, to make any changes to your benefits election during your new hire enrollment period (45 days). For questions, please contact the on-site FBMC Service Center at 305-585-6512.
