HOSPITAL CAPITAL EXPENDITURES AND FINANCIAL INFORMATION REPORT FOR 1999
The purpose of this report is to present information concerning the past, current and anticipatedcapital expenditures of hospitals in the State of Illinois, as well as financial information for thereporting hospitals.
At the direction of the Illinois Health Facilities Planning Board (State Board) the Department ofPublic Health (State Agency) developed a survey instrument and mailed it to all licensed hospitalsin the State of Illinois. The University of Illinois was the only State of Illinois operated hospitalincluded in this survey. No federally operated hospitals were included in this survey. The surveyrequested that hospitals provide capital expenditures for calendar year 1999 and estimated capitalexpenditures for the years 2000 and 2001. The hospitals were also requested to provide revenuesby inpatient and outpatient source as well as capital costs (depreciation plus interest) for calendaryear 1999. Finally, the hospitals were requested to provide financial statements (audited orunaudited) for the latest year available.
Not all of the reporting hospitals answered all of the survey questions. Therefore, differentcategories have different response numbers.
Highlights 1990-1999
1990-1999
From 1990-1999 the hospitals reported more than $11 billion in capital expenditures. Thirty-two hospitals reported spending more than $100 million in capital expenditures during this10-year period. Capital expenditures after adjusting for inflation increased more than 57%over this period of time. Average capital expenditures during the period of time 1990-1999ranged from $4.9 million in 1991 to $7.8 million in 1999 when adjusted for inflation.
1996-1999
Over the past four years patient revenues at hospitals within the State of Illinois haveincreased approximately 3.4% annually when adjusted for inflation. The reporting hospitals(214) averaged $4.9 million in net income per year over this four-year period. Nearly 77%(165) of the hospitals reported making a profit for the period 1996-1999. Total hospitalprofits (165) amounted to more than $4.4 billion for this period of time. Nine hospitals mademore than $100 million. Eighty hospitals had profits more than $10 million during thisperiod. Investment income and other income totaled more than $2.3 billion and $4.9 billionrespectively for this four-year period.
Forty percent of the hospitals reported making money from operations during the period1996-1999. Operating profits totaled $1.2 billion over this period of time. Sixty percent(129) of the hospitals reporting had operating losses for the years 1996-1999. These losses

amounted to $3.8 billion. Of that amount the hospitals owned/operated by Cook County(refer to Table IV for listing) accounted for more than one third of the total or $1.3 billion.
The period 1996-1999 saw the average length of stay (ALOS) decrease approximately 7%.Average daily census and average occupancy percentage of inventory beds remainedunchanged as inventory beds decreased approximately 4%. Average occupancy percentagewas less than 50% over this four-year period. Medicare and Medicaid as a percentage oftotal patients served remained unchanged over this four-year period. Significantly, privatepay patients as a percentage of the total patients served has increased 26% from 1996-1999.
1999Capital expenditures by Illinois hospitals (211) totaled $1.64 billion in 1999, with projectedspending of $1.6 billion in 2000 and $1.63 billion in 2001. Twenty percent (41) of allhospitals reporting incurred approximately 75% of all capital expenditures reported to theState Agency in 1999.
The reporting hospitals had average net income of $3.06 million in 1999. Nearly 65% (138)of the hospitals reported making a profit in 1999. Total hospital profits (138) amounted tomore than $1.03 billion for this period of time.
Thirty percent of the hospitals (64) had operating profits of $263 million in 1999. Seventypercent (150) of the hospitals reporting totaled $1.3 billion in operating losses in 1999. Morethan 25% or $330 million of the operating losses are attributable to the hospitalsowned/operated by Cook County.
Findings
I. Capital Expenditures
Since 1990 average capital expenditures have increased more than 57% when adjusted forinflation. Capital expenditures at the hospitals have increased even though review of theseexpenditures by the State Board has decreased.
Total capital expenditures as reported by 215 hospitals to the State Agency for 1999 was$1.664 billion. Of that amount $1.064 billion (63.9%) was funded by equity and theremainder approximately $600 million was funded by debt (36.1%). The average capitalexpenditure for the reporting hospitals for 1999 was $7.6 million per hospital. The mediancapital expenditure was $ 2.24 million
Twenty percent (41) of the hospitals reporting provided approximately 75% or $1.3 billionof the total capital expenditures for calendar year 1999. Three hospitals reported capitalexpenditures of more than $100 million: Northwestern Memorial Hospital ($203,571,785);

Evanston Hospital ($120,102,526); and Edward Hospital ($114,697,000). The next highestamount was reported by Cook County Hospital which reported $89,588,578 in capitalexpenditures in 1999. Nationally hospitals averaged approximately $5.1 million in capitalexpenditures in 1999.
Table I provides the total capital expenditures by year for the past 12 years as reported by thehospitals to the State Agency. The State Agency notes that the sample size (the number ofhospitals responding) varies for most years; therefore, the amounts are not totals. (e.g., only124 hospitals reported in 1995).

II. Patient Revenues
Total patient revenue reported by 211 hospitals totaled more than $32 billion in which $20.4billion (64%) was inpatient revenue and $11.6 billion (36%) was outpatient revenue.Nationally, hospitals averaged 62% in inpatient revenue and 38% in outpatient revenue in1999. Ninety-nine hospitals reported revenues of more than $100,000,000. The largestpatient revenue reported was Rush-Presbyterian-St. Luke’s Medical Center at $940,034,652.Table II illustrates the number of facilities by total revenue.

III. Capital Costs
Capital costs (depreciation plus interest) reported by the hospitals totaled $1.6 billion in1999. The average capital cost per hospital was $7,341,716 and the median was $3,606,000.Forty-eight hospitals reported capital costs of more than $10,000,000. Table III shows thetop ten hospitals’ capital costs. Of the top ten hospitals listed in Table III, eight are teachinginstitutions. Holy Cross Hospital and Bethany Hospital are non teaching institutions. Of the48 hospitals reporting capital costs more than $10,000,000, 22 or 47% are teachinginstitutions.

IV. Operating Income/Loss
Of the 214 hospitals reporting 30% (64) of the hospitals had an operating profit of $263million in 1999. The average operating profit for these 64 hospitals was $4.1 million. These64 hospitals had on average an occupancy percentage that was 2.65% higher than thosehospitals that reported operating losses in 1999. The median difference was 2.3%. Onehundred fifty hospitals reported an operating loss of more than $1.3 billion in 1999. Theaverage operating loss was $8.6 million for these 150 hospitals.
Hospitals owned/operated by Cook County accounted for more than 25% of the totaloperating losses reported by the hospitals (150) in 1999. If the hospitals owned/operated byCook County were not included, the average operating loss per the 150 hospitals losingmoney would have been $6.4 million.

For all reporting hospitals (214) net profit was $656 million in 1999. Average net profit forthe 214 hospitals reporting was $3.1 million and median net profit was more than $500thousand. Of the 214 hospitals reporting 138 (64%) reported more than $1.01 billion inprofits.
V. Profit Margin
Profit margins or the percentage of net profits to patient service revenue in 1999 averaged1.4% and the median profit margin was 2.4%. This means that for every dollar generated inrevenue the average hospital had net income of a little more than one cent. If the hospitalsowned/operated by Cook County were eliminated from the analysis, the average profitmargin would be 2.4%. Profit margins nationally for 1999 were 2.9%.
VI. Investment Income
The period 1996-1999 saw investment incomes increase more than 11% per year. Totalinvestment income for the period 1996-1999 totaled more than $2.3 billion. In 1999 there

was more than $623 million in investment income reported by the hospitals. The averageinvestment income reported by the hospitals was $2.9 million. The median was $338,106.There were 15 hospitals that had investment income of more than $10,000,000 in 1999.Forty-one hospitals reported no investment income in 1999, and three hospitals reportedlosses. Table V lists the 15 facilities that reported investment income of more than$10,000,000 in 1999.

VII. Other Income
The period 1996-1999 saw other income increase approximately 2% per year. Total otherincome for the four-year period was $4.9 billion. Other income totaled $1.3 billion in 1999.Other income includes such items as parking lot income, income from meals sold toemployees, vending machine rental, and revenue from coffee shops and canteens. Twenty-six hospitals (12%) reported other income of more than $10,000,000 in 1999.
VIII. Long-Term Debt
Total long-term debt reported to the State Agency was $5.6 billion. Fourteen hospitalsreported long-term debt of more than $100,000,000 with the largest being NorthwesternMemorial Hospital at $303,748,000. Average long-term debt for the hospitals reporting was

$29,471,502. For the 190 hospitals reporting, the average percentage of debt tocapitalization ratio ( the amount of debt to total equity) was 29.9%. Nationally the averagepercentage of debt to capitalization ratio for 1999 was 32%. Table VI provides a breakdownof the number of hospitals that reported long-term debt.

IX. Fund Balance
The total fund balance for the reporting hospitals for 1999 was over $12.5 billion. Thelargest fund balance was reported by Northwestern Memorial Hospital of $1,000,775,000which is approximately 9% of the total amount reported to the State Agency. From 1996 to1999 the average fund balance has increased approximately 2%. Table VII documents thetotal fund balances, averages and medians for the past four years.

X. Solvency Ratio
Net Assets/General Fund Balance as a percentage of net patient revenue is an indication ofthe solvency of the entity. The higher the percentage of net assets/general fund balancedivided by net patient revenue means that the hospital has a better solvency position. Thehospitals with the smallest percent represent a higher risk of insolvency. Average netassets/general fund balance to net patient revenue averaged 78% in 1998 and 75% in 1999.What this means is that the average hospital has approximately nine months to fundoperations from their general fund even if no revenue were received in that year. In 1998,70 (33%) hospitals reporting had sufficient fund balances to fund operations for six monthsor less. In 1999, 82 hospitals or 39% had sufficient fund balances to fund operations for sixmonths or less. In 1998, 10 hospitals reported a negative solvency ratio. In 1999, 18hospitals reported a negative solvency ratio. Of the 18 hospitals that reported negativesolvency ratios in 1999, one closed; two are proposing to close; and seven are members oflarge health care systems and as a such spread their loses over a larger number of facilitiesmaking them more likely to survive. In addition to the hospitals listed here, there are anadditional 19 facilities that have a solvency ratio of less than 20%.

Over this four-year period patient revenue has increased 3.4% when adjusted for inflation.ALOS have decreased 7% over this same period. Occupancy percentage for CONinventoried beds remained relatively unchanged as occupancy percentage for staffed bedsincreased 1.5% annually. Average daily census (ADC) remained unchanged over this four-year period. Private pay patients as a percentage of total patients served increased almost26% from 1996-1999.

XII. Data Limitations and Descriptions:
All information has been provided by the hospitals and is appended to this report. Howeverthe following points should be recognized when evaluating this information.
• Accounting practices vary by type of institution (not-for-profit vs. for-profit). Thereexists the possibility that cost and revenue data may not be calculated in the samemanner (cash vs. accrual or modified accrual). In many instances, facilities providedincome statements for year ending December 31, 1999 that may not reflectappropriate accruals.
• Comparisons between hospitals might not reflect financial information for the sametime period. In some cases the data reported is based upon the hospital’s fiscal yearand not on a calendar year. In some cases, hospitals reported financial informationat different times during the calendar year. Some hospitals reported data endingDecember 31, 1999, others reported information ending September 30, 1999 or June30, 1999.
• Total patient revenue in most cases does not include revenue from investments, rentalproperties or other revenue derived from non patient services.
• Unrealized gains/losses from the requirements of FASB 124 that require institutionsto value their investments at the end of the financial year at market value are notincluded in the operating income and excess of revenues over expenses figurespresented. Unrealized gains or losses flow directly to the equity section of thebalance sheet and in many instances were grouped as increases/decreases in total netassets.
• Operating profit excluded non-operating income/loss, contributions, and realized andunrealized gains and losses on investments. Overhead costs related to non-operatingrevenues are included in costs of operations. Also in some instances operating profitsand excess of revenues over expenses will be the same if the entity did not have non-operating income, investment income, or realized gain/losses from the sale ofsecurities.
• Net profit includes operating income, realized income or loss on investments, andother non-operating income. Net profit excludes unrealized gains and losses oninvestments, and transfers of net assets from or to a related organization, andcontributions of long-term assets.
• Profit margin indicates the proportion of patient service revenue retained as profit.This ratio focuses on the results of operations and includes non-operating sources ofincome.
• Net Assets/Net patient revenue is an indication of solvency of the entity. This showsa relative rank of net worth.
