G November 2018 data is now available within the Primary Care Team for childhood immunisation. The dashboard is being used to target practices where rates are below expected. Performance reports are produced, shared and discussed at practice visits and locality meetings on a regular basis.

G All midwives at WMUH are level 1 trained for Behavioural Therapy Treatment to help pregnant women stop smoking. One midwife is trained to level 2. Practices who have low immunisation rates are being targeted by the Primary Care Team. Vaccination rates are increasing month on month. Some practices have engaged with the PCPC team in call/recall for non-responding patients.

The 'One You' national brand has been applied in Hounslow, with regular community engagement to encourage cessation of smoking. Testing for vaccination dashboard is taking place with one practice, with the ambition to roll out to all Hounslow practices once testing is complete.

(2) Reduce admissions in under 5s by 5%

20/12/2018 Rosalyn King G Reviews of all urgent care activity is continuing and the audit process will be extended to Childrens attendances and admissions. Paediatrics activity which moved across to NHS-E is to be investigated to discover whether this is responsible for the favourable movement in activity.

G The number of admissions for under-5s has reduced in line with performance standard. NEL activity within the last 12 months has reduced by 33% compared to the previous 12 months.

Engagement is being undertaken with all care providers as part of Integrated Care.

(3) Contribute to the delivery of London Borough of Hounslow's plans for reduction of obesity in children

G Healthy Weight sessions and Child Weight Management services are meeting targets. More referrals from GPs to the 0-5years and 5-19 year s services is desired. A new, targeted Health Visiting component has been added in 2018 called Maternal Early Childhood Sustained Home-visiting (MESCH). A new contract is out to tender for new additional support to promote breastfeeding in the community. Delivery is expected to start January 2019 to meet UNICEF Stage 2 Community accreditation.

G One You Hounslow had 329 families complete a child weight management programme at the end of 2017/18 with 82% having maintained or reduced their BMI z-score. Last year 297 young people had a reduction in BNMI at their 6 month follow up. In Quarter 1 2018/19 43 families have completed a CWMP (currently below target for the year). In 2017/18 UNICEF Baby Friendly Stage 1 Commitment was achieved by LBH and HRCH NHS Trust. Hounslow has no published data for breastfeeding due to data validity, however for the 59% of parents with a valid breastfeeding status recorded at 6-8 weeks, 75% were partially or fully breastfeeding.

Working with schools and school nursing service to target CWM services PHE met with Health Visiting Service and Public Health to support around breastfeeding data. Health Visiting have been improving the recording of breastfeeding status at 6-8 weeks, but the strict timescales applied by PHE has anecdotally resulted in a reduction in the number of parents attending the 6-8 week check. Hounslow Young Parents in the Family Nurse Programme 87.9% initiated breastfeeding (national programme average 61.8%), 45.2% were still breastfeeding at 6 weeks compared to national programme average of 22.1% .

A Selected to be a Trailblazer for developing Mental Health Support Teams in schools following submission of successful bid. A Project Initiation Group has been established. Recruitment of new staff underway in partnership with West London Trust and Berkshire Healthcare Trust. Working with NHSE, HEE and Reading University on staff training. Parent trainers have been recruited and trained for Empowering Parents, Empowering Communities (EPEC) programme - first groups due to launch in January 2019. Waiting list initiative implemented in Tier 2 CAMHS to address on-going demand and capacity issues, including use of Child Wellbeing Practitioner posts.

G Formal announcement of Trailblazer sites has been delayed several times, due imminently. New teams will start training in Jan/Feb 2019 and the full service offer will be fully established by Dec 2019. Partnership Agreement established for Tier 2 CAMHS - regular reports on activity, outcomes and waiting times due from January 2019. Reduction in waiting time needs to be closely monitored in 2019. Regular reporting on impact of the Waiting list initiative for the Tier 2 CAMHS will be coming through the CAMHS Partnership Group from January 2019.

Engagement with schools, LA and voluntary sector as part of Trailblazer project. WLNHST is engaging with parents through the EPEC programme.

(5) Implement Prescribing Wisely across all Practices in Hounslow

G Number of items has decreased marginally month on month since August 2018. The Medicines Management Team continues to work with the GP practices.

A GP Prescribing Wisely Survey completed regarding their views on the Prescribing Wisely program. Hounslow CCG has purchased NHS-E leaflets which have been shared with GP practices.

GP Prescribing Wisely Survey completed Medicines Management team continues to participate in Patient Engagement activities in liaison with assistance from the Communication and Engagement Team. Medicines Management engages with individual practices on an on-going basis.

A Locality workshops undertaken in early December jointly between the consortium and HRCH to develop the locality working structure for the delivery of primary care at scale alongside the development of network based allied health care professional roles.

G Draft network overview developed for Physiotherapists, Pharmacists and Respiratory nurses as well as a specification for home visiting with a business case to be written in January 2019. Locality operational structure agreed with funding in place for delivery via primary care at scale.

Aforementioned workshops, Programme board held regularly between consortium and HRCH for future working and redesign of PCPC service into a community/primary care joint model. Regular contract meetings are taking place with both the consortium and HRCH

(7) Reduce inequality of access to services in Feltham

Apr-18 17/12/2018 Chris Jack A New APMS provider mobilised at Feltham Health Centre. Healthcare assistants in place at Age UK in joint Community/health endeavour. Feasibility study delayed due to inconsistent floor plans for Feltham Health Centre.

G Monthly programme for healthcare assistant now in place at a community site in Feltham working with Age UK.

GP practice sessions undertaken including 1-1 floor walks. Property services met with to discuss building redesigned and a feasibility study commissioning,. Age UK a new APMS provider facilitated to develop local community/health solution.

(8) Develop integrated community and primary care provision for long term conditions, using commissioning tools to support integration

A Conversations between Hounslow CCG and Hounslow Consortium are continuing with respect to the Primary Care Wrap-Around Contract. Five locality workshops were held during the week of 3rd December to develop the integrated community and primary care locality teams.

A A set of priorities have been agreed between HCCG and Hounslow Consortium, including infrastructure and service delivery. A revised plan is expected in January 2019.

Regular contract meetings with Hounslow Consortium and monthly Board to Board meetings are carried out.

(9) Improve use of technology including further development of call and re-call systems, use of WSIC dashboard at practice and federation level, and care planning

May-17 19/12/2018 Chris Jack A PCPC has access to the WSIC dashboards for the practices they work in. Dashboards are generated for the practices. These are used to case find for MDTs but this is challenging due to the time lag in data, which has to be verified on SystmOne. Draft rising risk dashboards have been created within WSIC to be rolled out to pilot sites in January.

A Call/recall process has been updated across approx. 50% of practices to operate under a new system which involves calling patients in the month of their birth for all their reviews.

Locality PPG groups have been attended to have the WSIC process explained. Feedback was received on content and frequency of letters and texts. The system can be tailored to the practice’s requirements so engagement with them is on-going as part of PCPC's overall relationship with the GPs/PMs. Locality PPGs informed of the WSIC and their ability to opt out. Concerns raised at contract meeting regarding the utility of the dashboard for which there is a data lag.

Improve delivery of annual Learning Disability health checks in Hounslow by 20%

A Included as a mandatory service in the Hounslow Primary Care Contract (wraparound). Key KPIs within the contract includes that 95% of the LD register have a completed health check. Hounslow CCG have reconciled LD health check activity at practice level and have provided Hounslow Consortium with practice target numbers.

R All practices have signed up to provide the Learning Disabilities Health Check DES. Activity reports for Month 8 only show 23% of patients have on the LD register have had a completed health check. Hounslow Consortium are in conversation with practices with regard to identified gaps in activity.

The CCG continues to work with Hounslow Consortium and individual practices to increase the % of health checks undertaken through the year.

(11) Use all 6000 new PAM licenses to inform personalised approach to care

G The Primary Care Patient Coordinators have targeted patients with long term conditions to distribute PAM licences to those who may benefit the most.

G 3,805 PAM licences are reported to be in use for the reporting period April-November 2018. This is 4.9% below the target of 4000.

(12) Increase Dementia diagnosis rates and improve follow up care including use of early help

Apr-18 20/12/2018 Bilal Dunoo G HCCG is working with WLMHT to improve follow-up care by facilitating better care pathways across the health and social care system. Additionaly, a project is being worked up for an Enhanced Dementia Care Service aimed at improving the health and wellbeing for dementia patients across the health economy.

G Dementia diagnosis rates have improved from 70.8% as of March 2018 to 70.4% as of October 2018.

Dementia diagnosis data will be shared with GPs as part of wider strategy to encourage higher levels of dementia diagnosis in line with expectations.

(13) Develop support and training offer for carers including implementing a carers support group, implementing health related training with carers (using 7 key issues checklist), and trial IAPT support to carers

Jun-18 21/12/2018 James Hearn G We are currently in discussion with a provider to run a bespoke training course for carers called ‘taking back control’ which will cover a range of health and wellbeing issues planned for the first quarter of 2019.

G An IAPT trial was held by Anchor Counselling during carers week 2018. These sessions were initially set up as a 2 hour group therapy session facilitated by Jacqueline McCouat, Chief Executive & Qualified Psychotherapist and Ila Patel, Qualified Psychotherapist. Following on from the trial we are now working to further promote the existing free 1-2-1 counselling offered through IAPT to ensure access to carers in Hounslow.

The commissioned Hestia Integrated Support Service are running monthly carers meetings. These meetings are used to ensure that any health related carer issues are discussed. These issues are then collated and fed back to the carers commissione r,via our contract monitoring processes. Once there is an appropriate level of reporting on health issues from carers, a report will be compiled on the issues being raised and any intended actions/developments planned for the commissioned services currently offered or new services to carers for consideration by the CCG.

(14) Agree in-reach and co-ordinated working between hospital pathways, ICRS and CRS to deliver a reduction of non-elective admissions and readmissions, including introduction of HomeFirst

R Non-elective admissions continue to increase. Prior to full implementation of pathway 3, a financial review and further pilots will be undertaken. Specific approaches to tackle locality performance are underway and some localities are seeing a reduction in the increase of attendances. ICRS input into A&E and medical admissions pathways is being scoped.

R Audits have been completed on 0 LOS medical admissions including AEC. Pathways are currently under review and further work is planned to support reductions and earlier discharges for longer LOS. A review of the first patient on pathway 3 has been undertaken and a 'lessons learned' document has been drafted.

A&E Delivery Group discussions across all providers are scheduled. Targeted audits and reviews for consultants and GPs are underway. Additional engagement with GPs for pilot patients has been identified as required.

(15) Implement Enhance Care in Care Homes and the new Care Homes specification and reduction in LAS conveyances

G 116 members of staff have completed part 1 of the RASD training programme. LAS Conveyance callouts to care homes showed percentage reduction compared to 17/18. August 2018 - 9% reduction compared to August 17/18. September 2018 - 21% reduction compared to September 17/18. October 2018 - 22% reduction compared to October 17/18.

Engagement with GPs, including a learning event, workshop and as part of contract monitoring meetings. Engagement with care homes including conducting visits with care homes and as part of contract monitoring meetings.

(16) Complete development of specification for support of those with severe Mental Illness in Primary Care

The CCG continues to work with Hounslow Consortium and individual practices to increase the % of reviews undertaken to meet the end of year target

(17) Complete implementation of the Recovery Model

G The Community Rehabilitation and Recovery Service (Cherry Tree) opened in April 2018 (20 beds), run in partnership between London Cyrenians and WLNHST. Good progress made so far in establishing this innovative new service for people with complex needs, patients are admitted from the acute care pathway, specialist inpatient beds and high cost community placements. A MH Nurse practitioner has been in post since April, providing extra capacity to review spot-purchased MH placements, leadership to increased movement through the accommodation pathway.

G A total of 11 patients have moved to Cherry Tree. In 4 cases these placements proved to be short term, while 7 are on-going. There are 5 more potential placements in the pipeline and further referrals are being identified. The new service is filling up slower than anticipated due to the complexity of the target client group, however the overall level of savings will be substantially higher than planned over the first two years in operation. A business case for enhanced capacity in the WLMHT Clinical Team was approved by the F&P Committee in Nov 18.

Cherry Tree interfaces closely with a number of key services and engagement has been undertaken with these to develop effective working relationships - key examples are the inpatient service at Lakeside MHU, local Recovery Teams and the Crisis Assessment and Treatment Team, WL Rehabilitation Service and the new substance misuse service (ARC) run by CNWL. Effective working relationships will take time to establish but a good start has been made. Engagement is also on-going with service users at Cherry Tree through regular, well-attended forum supported by a dedicated member of staff - service users actively involved in decisions about the running of the service.

(18) Ensure the Crisis pathway is delivering appropriate support for both adults and children, including resolving LPS, place of safety, and the use of the SPA by LAS

A WLNHST continue to make improvements with regard to the LPS service at WMUH. The LPS Contract Improvement Notice has been lifted. The service priorities remain recruitment and sustainability. This is being taken forward through strong partnership working between WMUH and WLNHST. Work is focussed upon developing an Alliance Partnership between the two providers with the aim that this will form part of the CCG’s long term integrated care vision. Providers and commissioners are actively involved in the London wide HBPoS (Health Based Place of Safety) Strategy. Locally contract negotiations for 2019/20 include interim plans to assure adequate local provision of Section 136 places of safety.

G Aim to deliver an LPS service in line with the Core24 service as set out in the 5YFV. 24/7 LPS and CATT response in place. Informal Alliance Contract Approach included in the 2019/20 commissioning intentions with the vision this will be a formal agreement in 2020/21. The LPS Contract Improvement Notice has been lifted.

Engagement with WMUH, WLNHST, Ealing CCG, Richmond CCG. HBPoS partnership working across NW London which includes patients representatives. A patient representative is a member of the Hounslow Integrated care Working Group.

G NWL Out Patient Transformation Scheme is in progress NWL Outpatient Change proposal presented at F&P on 6th Nov Contract models will be reviewed to ensure savings for both CCGs and Providers Hounslow continues to utilise Clinical Guidelines and SMART forms via RFS pathways. CCG and Providers are developing implementation plans. Hounslow CCG are reviewing where there are gaps in the programme.

A 4 NWL acute Providers have adopted the NWL Referral guidelines. The 2019/20 QIPP Programme will include savings in the following specialties: PHASE ONE: Cardiology,Gynaecology, Gastroenterology, Dermatology. PHASE TWO includes Urology, Respiratory, Ophthalmology, MSK

Outpatient Transformation Board's membership includes representatives from all NWL acute trusts.

(20) Continue progress on Heston, Brentwood and Chiswick developments

A Progress on Heston has stalled. LiftCo in partnership with Community Health Partnership (CHP) are exploring ways in which the Heston Health Centre can be built without breaching the Affordability Cap All parties have agreed that the Heston Health Centre will be built. It is estimated that the outcome of the LiftCo/CHP investigation will be known before Christmas (2018) and will be communicated to all parties as soon as it is known. Heston - NHSE decision on OBC dependant on outcome of interest rate issue Chiswick and Brentford - Awaiting agreement on Heads of Terms.

A Heston: Planning permission granted The outcome of the LiftCo/CHP investigation will be known before Christmas (2018) Brentford and Chiswick: PIDs and Options Appraisals have been agreed at GB 10th July 2018 Once HoT has been agreed, the OBC will be developed.

Heston Project Board membership includes a lay member and a patient representative. The Heston User Group includes GP's, Practice Managers, Councillors and Patient representatives.

(23) Deliver 100% of the required QIPP (£16.4m) and ensure CCG meets its control total

A 2018/19 Target has been adjusted down to £11.981m which has been fully identified. Delivery includes a risk. So far £1.46m of savings has been identified for delivery in 2018/19. These savings are mainly non-recurrent savings.

A Year to date (M8), HCCG has achieved 75% (5.8m) of target £7.7m. Full year forecast as at Month 8 stands at 77% (9.2m) of target of £11.98m. Scheme forecasts have been adjusted following NWL QIPP Leads Steering Group reviews. Risk on further slippage has therefore reduced. Schemes with a delayed start and delayed achievement are included in the 2019/20 QIPP plan.

QIPP plans are regularly discussed with operational and finance leads at providers Regular contract performance at operational and strategic meetings including the CWFT Transformation Board, the Performance Contract Executive (PCE) Meeting and the Finance and Information Group (FIG). The monthly QIPP Board Meeting, the Clinical Board, the F&P Committee and the Governing Body. Meetings with HRCH to develop joint CIP/QIPP initiatives for 2019/20.

(24) Through developing the organisation ensure Hounslow CCG is a fit-for-purpose commissioning organisation and develops collaborative working across NWL CCGs and providers establish clinical review of all follow-up patients booked more than 6 months in advance

G Office move to New Civic Centre planned for March 2019. Smart Working to be implemented prior to Office Move. Business Information working collaboratively with WLCCG on the CWFT contract and across NWL on Primary Care reporting. HCCG and WLCCG working collaboratively on management of the CWFT Contract. Commissioning leads working across NWL QIPP initiatives e.g. Outpatient Demand Management, CHC and PPwT.

G Project Plan, Risk Register, Meetings Register, IT Asset Registers are in place. A SMART/Agile working Policy has been agreed. A new IT Implementation sub-committee has been set up. Monthly meetings including representation from all parties held addressing all outstanding issues. Council have confirmed they will physically move the CCG. Licence to occupy being finalised between solicitors with CCG and Council input. Car parking solution to be finalised in January.

Office move: Staff consultation documentation is in completed and staff will be consulted on in January. Weekly staff updates at Wednesday morning team meetings. Communication to external partners in development. Lunch and Learn sessions on Smart / Agile working have been completed.

(21) Commission community incontinence service for children

Engagement undertaken with HRCH & WMUH to get service up and running. Both HRCH and WMUH staff make up the service. Further engagement planned with families and patients.

(22) Deliver Integrated Care Business Case by July 2018

Jul-18 17/12/2018 Chris Jack G In September, the Governing Body gave HCCG a mandate to pursue Integrated Care objectives within the scope of the outline business case. Development of Integrated Care as a system is on-going.

The table below shows the number of admissions for under-5’s broken down by admission type. From April to August 2018, there have been 1,930 admissions for under-5’s in Hounslow. The full-year forecast for 2018/19 is 3309 admissions, which is an annual reduction of 949 admissions, or 22.3% compared to 2017/18. Between 2016/17 and 2017/18, some paediatric activity moved to NHS England which accounts for the movement between those years.

The graphs below shows the monthly number and cost of Over the Counter Medicines items since April 2017. This shows that there has been a decrease of 4,002 items year to date from April –September 2018 compared to the number of items over the same period in 2017/2018. the total cost over this period of time has increased by £7,594.

(12) Increase Dementia Diagnosis Rates and improve follow up care including use of early help

The graph below shows the number of patients over the age 65 who have been diagnosed with dementia.

The number of patients diagnosed saw an increase between May and June 2018 which became stable before diagnosis numbers saw a dip in October to 70.9%. The diagnosis rate target for HCCG is 66.69%, in November 2018 the number of patients diagnosed saw an increase with performance reported at 71.6%.

The table and graph below show the number of LAS conveyances carried out for patients within Hounslow care homes targeted by the Enhanced Primary Care service. From June 2018 onwards, the number of LAS conveyances from nursing homes decreased.
