Important Login Information MAIN LAB ACCOUNT - You must use the MAIN LAB account, not a personal account, to login to the Lab Center. When logged in with the MAIN LAB account, the User listed in the top right corner will be “Lab Administrator” followed by the name of your lab. OTHER ASHI ACCOUNT - If you are using a personal account (i.e., an account assigned to a person), you will NOT be able to proceed with your PT order. Using a personal ASHI PT account will not allow access to the PT ordering option. You must use the MAIN LAB account to place an order. Important Login Information

• MAIN LAB ACCOUNT - You must use the MAIN LAB account, not a personal account, to login to the Lab Center. When logged in with the MAIN LAB account, the User listed in the top right corner will be “Lab Administrator” followed by the name of your lab.

Step-by-Step Guide for Placing PT Orders in the Lab Center

Go to the PT page of the ASHI website (https://www.ashi-hla.org/page/PT)

Click the “PT Lab Center” link to access the Lab Center.

Login with the username and password for the MAIN LAB account.

If the “Profile Consent Needed” box pops up, click “Acknowledge & Agree” to accept ASHI’s privacy policy and continue.

After logging in, click on the blue Login button in the top right corner then click the drop-down arrow next to Orders.

Select Place Order. Please note: If the “Place Order” option doesn’t appear, a different login may be needed. If logged in with the correct MAIN LAB account, the name in the top right corner will be “Lab Administrator” followed by the name of your lab.

Click the Item name (in red) - or the View button (black) - for a detailed description of each item. Please note: All modules listed should begin with the same year. If a module for a prior year appears in your list, please contact [email protected] before proceeding.

Select the modules for the appropriate year - and select quantity of each.

Please note: A quantity of 1 means 2 shipments of that module will be shipped to your laboratory in one calendar year,

typically 1 shipment in the spring and 1 in the fall. See the description for additional details which include the month the

samples will be shipped, the number of samples that will be sent, and how many ml are included in each module.

If your laboratory is located in the United States or Canada and you wish to have your PT results sent to CAP, answer Yes

to the CAP question for each modules’ results you want sent to CAP.

Please note: CAP does not accept scores for laboratories outside the U.S. or Canada. CAP also does not accept scores for

laboratories that have not applied for CAP accreditation.

IMPORTANT:

After selecting “Add Item”, if you receive the message “ARE YOU A MEMBER?” and you find

yourself in a loop that keeps returning to this same message, please be aware that an easy

Cookie Update must be completed on your computer before continuing. Follow the next few

steps to update the cookie settings on your computer or the PT order will not go through.

If, however, you did not receive this message, please feel free to skip these next few

Cookie Setting steps (in blue).

Select a new tab on your computer.

In your search engine, type chrome://flags

This should lead you to the following screen entitled “Experiments”. Please don’t be alarmed

at this message. These steps were provided by our IT team and were followed by ASHI staff to

proceed with a PT order placed from the ASHI staff office. Once the settings were changed in

this manner, the process of placing a PT order could proceed.

On this page, look for two items: “SameSite by default cookies” and “Cookies without

SameSite must be secure”. You can use the search box to locate these two items quicker. Type

in samesite in the search box and the following results will appear.

Use the drop-down arrow in the Default box next to these two and change the option to

Disabled.

Only those two items need to be changed to Disabled. All others can remain the same. Once

complete, click Relaunch.

Once completed, you can close the extra tab.

Back on the order page tab, click the refresh icon to go back to the normal Ordering Page.

YOU CAN RESUME YOUR PT ORDERING BY FOLLOWING THE STEPS BELOW: To see the items currently in your cart, click Cart at the top of the page.

Review the items and amounts listed to ensure accuracy of your PT order.

Click Continue Shopping to add more modules or to add the $25 wire fee to the invoice (if paying by wire).

(See further wire information below.)

*Wire Transfer If paying by wire, after you have selected all modules, please select Continue Shopping. To add the wire fee, go to the module listing page, scroll to the bottom of the page and select Page 2.

Select the “Wire Processing Fee” on Page 2. (This will add the $25 wire processing fee to your invoice.)

Select “Checkout” to proceed to Payment page.

IMPORTANT: Your PT order will be shipped to the address as it is listed on the Order/Checkout

page under the Recipient Information so carefully review the lab address in this section. This

information must be updated to match the address the samples should be shipped to.

It is the sole responsibility of the laboratory to provide the correct address and business phone

number for ASHI to use for shipping purposes.

The Total Amount listed on the address/payment page should include shipping charges.

Choose “Bill Me” if you are using a PO# (Purchase Order#), are paying by check, or paying by wire transfer. If your

order has already been issued a Purchase Order# by your institution, please include it in the PO field located under the

billing address and phone number.

You may click “Check here” if the billing address is the same as the recipient address.

IMPORTANT: When selecting the Bill Me option, please include the email address for your

lab’s Billing Contact - or A/P Accounts Payable department – in the Special Instructions section.

Questions concerning invoice payments, including wire payments, will be sent to this email

address when necessary (i.e., if wire payments don’t include the invoice#, etc.)

If paying by credit card, choose “Credit Card” and enter the credit card and billing information in the appropriate fields. Update the “Name on Card” field to match the name on the credit card. Please note: The Billing address must match the address on file with the cc company or the card may be declined.

Please note: The Location field is a required field in the Billing address section. U.S. labs can enter the name of their

state. International labs can enter the name of their province, territory, etc.

Once payment information has been entered, if you selected the Bill Me option, include the name and email address of

your Billing contact or A/P department in the Additional Comments section.

Please note: Special requests for your order should be communicated by emailing [email protected]. Please put “PT

Order Request” in the subject line of the email to ensure the ASHI office is aware of special requests or instructions

specified during the placement of your order.

Click “Proceed to Confirmation” to continue processing the order.

System Time Out

After clicking Proceed, if you receive another message that says “Are you a member?..., this iteration of the message

does not have anything to do with cookies.

It is likely due to a system timeout in which the system automatically signed you out of your session if the ordering page

was left open for a prolonged period of time.

To continue ordering, click “sign in” and re-enter the username and password of the MAIN LAB account.

After entering the login for the MAIN LAB account, uncheck the Remember Me box, so your computer does not

“remember” you as the MAIN LAB account the next time you sign into the Lab Center.

Before proceeding with your order, click the Cart to see if any items were carried over from your last session.

Review the list of items in the cart and proceed accordingly. “Continue Shopping” will allow you to add more modules or the $25 wire fee to invoice if paying by wire. (See further instructions above.)

Select “Checkout” to proceed to the Address/ Payment page.

Please note: Information on the payment page may not have carried over from the expired session so please review and

make any necessary updates to the payment information.

As stated above, if paying by wire or check, select Bill Me option and click “Check here” if the billing address is the same as the recipient address. If not paying by wire, select Credit Card and enter the cc information. Update the “Name on Card” field. Billing address must match the address on file with the cc company for the card being used or the card may be declined.

Click “Proceed to Confirmation” after making the necessary updates.

The confirmation page allows you to review and/or edit information as needed before submitting the order.

To edit, click the page icon on the top left of the section you wish to edit.

To submit the order, click Complete Order.

If you receive a “blank” page after selecting Complete Order, you may need to use the scroll bar to go to the top of the

page and view the Invoice.

To print or save a copy of the invoice, click on View Invoice/Receipt.

Please review your invoice for modules selected and to be sure the CAP question was answered correctly (for U.S. and

Canada).

Bill Me

IMPORTANT:

If your laboratory selected the Bill Me option and will be paying by wire, check, and/or is using

a PO#, please forward a copy of the invoice to your Billing Contact or Accounts Payable

department for payment upon completion of the PT order process.

If paying by wire, payment details – including account number for wire transfers, etc. - are included on a copy of the

saved invoice which is available once the order has been completed.) Please include the invoice# when providing

payment by wire, otherwise, the payment for your invoice may be delayed.

Click the Log Out button to fully logout of the PT Ordering session of the MAIN LAB account.

If you exit without logging out, your computer may keep you logged in as the LAB which may prevent you from accessing

the options necessary for entering and viewing PT results.
