Send your paperwork directly to the USCG. You are on your own with this method. Walk-ins for In person paperwork reviews at the USCG are not allowed by any offices that we know of. Turnaround time for the Coast Guard to notify you of errors is considerably longer than our review time. We do not recommend sending your paperwork to the Coast Guard until it has been reviewed. IMPORTANT INFORMATION ABOUT COSTS, FEES AND OTHER

EXPENSES ASSOCIATED WITH THE LICENSING PROCESS

If you attend a Coast Guard approved course or a license prep course, there will be costs for the training. The costs will generally include some of your training materials and a guarantee of success.

Additional required supplies could include a plotting gear (dividers and parallel rule), and perhaps a Time-Speed Distance or hand held calculator (costs vary from $30 upward).

There will also be costs associated with a physical exam and drug screen, as well as CPR and First Aid training (all required by the Coast Guard for original license transactions). Costs vary greatly depending on where you are, and where you shop (usual range is from $100 to $150). These things do NOT need to be accomplished before our course is taken.

There are fees for license evaluation and issuance by the Coast Guard ($145 for original license), and a fee for testing. Paperwork review by Sea School prior to submission to the USCG will be $35 for original licenses (not required, but a valuable service).

Testing fees are paid to the testing organization. For example, if taking an approved course, the approved school will collect the test fee ($80) for a full test for an original license. If the Coast Guard gives the test, they will collect a fee, usually a higher amount.

Test fees authorize the student to have three attempts at the exam, after which a waiting period may be imposed, and an additional test fee will be required. This applies to private approved schools and to the Coast Guard as mandated by the Code of Federal Regulations.

If testing at a remote location, the organization traveling to give the test will collect a fee (usually around $25) to cover travel expenses for the trip. If no travel is involved by the testing organization, there is no additional fee to pay. That means testing at a fixed school office offering the exams.

SEA SCHOOL provides the best paperwork package of forms and instructions for getting your license.From 6-Pack to 100 ton Master, we provide everything you need.

We strongly urge you to use our forms packages rather than any supplied by the Coast Guard. Theyusually provide many more forms than you will need for any given license, and explanations on theiruse are limited.

Have questions? Just call. We=ll provide accurate answers, or get them for you.

In order to expedite processing, you may file your paperwork directly with the Coast Guard. We urgeyou to check every line on every form carefully for errors and/or omissions before you do.

If you want your paperwork checked by our staff before filing, simply mail the forms to our office forquick and accurate review. There is a modest cost for this service at $35 for original licenses andraises of grade. Renewal forms are reviewed at a nominal $25 charge. Tonnage increases and otherminor endorsement requests are reviewed without charge.

If we find problems, we will advise you as to how to correct them. We=ll let you know if something youhave done will put you in a gray area of Coast Guard policy, and how to deal with it if a problem comesup.

Make sure your license or document application is correct the FIRST time

Many applications are returned due to missing information, unsigned documents, etc. Sea School’s checkoff lists and example forms have been provided to help you get your application correct the first time. But, if you want us to confirm that your application is ready, we provide an application review service for just $35. Here’s what you get:

If there is missing information, we will insure completion

We will deliver your application to the USCG

Your application will be stamped “REVIEWED BY SEA SCHOOL”. While this carries no official

status, it lets the Coast Guard examiners know that it has been reviewed by us for completeness

and accuracy.

While they may be willing to “put an eye” on your paperwork, they are not trained to review your application and they can make no assurance that it is correct. _____A copy of the front of your TWIC card or evidence of having applied for one is enclosed. In order to expedite processing at the USCG, they have requested that you contact TSA at 1-866-347-8942, press 5 for existing card info, and make sure that you are registered with the occupation code as “Merchant Mariner”. PHYSICAL STANDARDS FOR U.S. COAST GUARD LICENSES Based on the U.S. Coast Guard document entitled “NVIC 04-08"

1. Eyesight, at least 20/200 (the big E at the top of the eye chart), correctable to 20/40 or better, in

at least one eye. Waivers may be available in some cases. Uncorrected vision MUST be shown on the physical, and correctable vision must be shown if uncorrected is worse than 20/40. Monocular vision requires verification that the applicant has compensated for lack of depth perception.

2. Color perception - normal by any of the methods shown. Method used MUST be indicated on

the form, as well as number of errors when tested. Waivers available in some cases for color blindness.

3. Hearing - testing is ONLY required if physician believes hearing is abnormal. 4. Cardiac - any abnormal indications in the past will have to be explained. Stress tests may be

required. Contact Sea School for details if unsure. Hypertension - Blood pressure 160/100 or lower, regardless of whether or not medications are

used to accomplish the control. 5. Lung diseases - any incapacitating diseases or those requiring corticosteriod medications may

be disqualifying conditions. Pulmonary function tests may be required. 6. Orthopedic - amputations, deformity or arthritis resulting in impaired motion may require further

evaluation and demonstration of abilities. 7. Diabetes - Well controlled Insulin dependent diabetes or well controlled non-insulin dependent

diabetes may no longer be disqualifying. Contact Sea School for details if unsure. 8. Neurologic - any convulsive disorder regardless of control by medication requires further

evaluation. 9. Psychiatric - primary psychosis or use of psychotropic medications requires further evaluation. 10. Medications - Anticoagulants (Warfarin, Coumadin), systemic corticosteriods, psychotropic

medications, medications with debilitating side effects, and addictive painkillers require further evaluation.

Further evaluation means that the file with all supporting documentation may be sent to the Coast Guard’s National Maritime Center for additional review and consideration. physstds 1015

ACTIVE CONDITIONS

The USCG is very strict when it comes to information contained on the applicant’s physical

exam paperwork.

In order to shed some light on this situation, we call your attention to the Coast Guard

document NVIC 04-08 which is a policy explanation of their regulations regarding physical

exams required for license and document issuance.

On page 1 of enclosure (3), item 1 of the “Medical and Physical Evaluation Guidelines…” of

the document says: “Active Condition. If not specified as “history of” in this table, a condition

must be currently active to be subject to further review. For purposes of this enclosure, “active”

means that the applicant is currently under treatment for the condition, or that the applicant is

currently under observation for possible worsening or recurrence of the condition, or that the

condition is currently present.”

In other words, don’t allow the physical to be an exercise in your irrelevant personal history.

It is a current physical!

There are a few key words that, when used, will trigger a response from the USCG asking for

more information from the applicant and his or her medical care provider. Those key words

include, but are not limited to: Sleep Apnea, Anxiety, ADD/ADHD, Bipolar Disorder,

Depression, COPD, and the use of addictive and/or mood altering medications.

SIMPLIFIED DRUG TESTING PROCEDURES

A drug test from an approved lab, certified as negative by a licensed physician, is now required forany license or document transaction with the Coast Guard if that transaction requires a physical.Sounds simple! It is if you follow the step by step procedures:

1. Contact a licensed physician (MD or DO only) and ask if he or she can arrange for a drug testapproved by the Department of Transportation (DOT). The test must be by a lab authorized bySubstance Abuse and Mental Health Services Administration (SAMHSA).

2. Verify that the physician has been trained and certified by the appropriate federal agency in drugtesting procedures. A new Federal Regulation requires that physicians receive special trainingbefore they can act as Medical Review Officers (MROs). Physician must identify the qualifyingorganization and registration number in the lower right corner of the Periodic Drug Testing Form.

3. If yes to both 1 and 2 above, ask costs. If satisfactory, go have it done.

4. Make sure that the lab name AND location is on the list provided. If it is not, it is unacceptable.The test can only be for the 7 listed drugs (regulations changed from 5 to 7 during 2010). Nomore, no less! ONLY the 7 drugs listed can be tested for. The official USCG form still only shows 5,but all approved labs test for the 7 as required.

5. Collection personnel send the urine sample (urine, not blood) to one of the approved labs at oneof their approved locations.

This test must be done "Blind." That means the lab only has a control number and physicianinformation to return the results to. Labs cannot have your name or address. That's why you need tofind a physician or collection organization that knows what to do. Approved drug consortiums are anideal source to use. You can't go directly to one of the labs on the approved list.

6. Lab runs the test and returns the results to the doctor.

7. Doctor matches up the lab results with your information and fills out the proper form completely.

8. Doctor sends the form to you, not to the USCG. Photocopies are acceptable to the USCG.

9. You submit the form to the Coast Guard with the rest of your paperwork. Do not send the labreport to the Coast Guard. Drug tests must be less than 6 months old, physicals less than oneyear old.

SUMMARY:1. Find a doctor willing and ABLE to do the drug test2. Check the form received from the doctor to be sure it is properly and completely filled out3. Submit the form to the Coast Guard with other required paperwork

A list of approved labs and their approved locations is available for your physician by mail or fax. Thelist is for your physician’s use only. You may not contact the approved lab directly.

NOTE: If employed in the maritime field, mariners need to be subject to random testing. This can be acompany sponsored plan or outside consortium (a group of individuals banding together, sponsoredby an organization).

AUTOMATIC LICENSE RENEWAL NOTIFICATION OFFERED

National Captain's Institute, Inc., a not-for-profit educational institution, is offering anautomatic renewal notification program for Captain's licenses and Merchant Mariner'sDocuments.

Coast Guard Licenses and Documents are good for a period of five years, and thereis NO notification sent prior to expiration. The license holder must remember to applyfor renewal or the license or document may expire.

Licenses and documents may be renewed up to 360 days prior to expiration, but afterexpiration they cannot be used. In order to avoid having to go through the wholepaperwork and testing procedure for a new license or document, the prudent marinerrenews prior to expiration.

Recent regulatory and policy changes has caused the renewal process at the Regional ExamCenters to take considerably longer than was formerly the case. This is especially true wherephysical and other waivers have to be reconsidered by the Coast Guard.

This program will notify you approximately six months in advance of expiration of yourdocument, and will advise you of the renewal procedures currently in use by the Coast Guardalong with helpful hints on renewing with as little effort as possible.

The fee for registration is only $5.00.

If you hold both a license and a document, you may renew the document early so itsexpiration date will coincide with that of the license for ease of future renewals. Askyour Regional Exam Center for procedures and forms. Renewing both together alsosaves money as the user fees for evaluation are combined.
