Quality Improvement and Management Project – Fall 2020
You will work on this project in groups of 3 or 4. This paper should at least 10 content pages, APA style, and using peer reviewed journals for references (See grading rubric). Please use headings (hint: follow this outline and make sure you include an introduction and summary. No abstract is necessary). **Papers must be submitted through Safe Assign on Blackboard. Because this is a group project, one group member will submit it in Blackboard.
1. Introduction: Identify the aim of the paper. Introduce reader to what the paper will address.
2. Problem: Identify one area at your clinical site that is not meeting desired quality outcomes or may be considered a potential problem. Give specific information regarding who, what, where, when as it relates to the problem (key players and situations). Discuss in terms of which Joint Commission quality indicator it relates to. In addition, document relevance using another indicator from a health (Health People 2020), nursing (NDNQI), or educational (QSEN) database or initiative. Describe why it is a problem or a concern (Potential problem) at this clinical site (i.e. is it related to technique, staffing, education, etc…).
3. Literature Review: Conduct a literature review on the topic. Identify sources from professional literature that discuss why this is a problem. Identify suggested solutions, how they were implemented and what were the results. You are establishing evidence here.
4. QI/QM Plan: Create a Quality Improvement plan that can be initiated at this clinical site. It should be based on the information you obtained and reported in the “Problem” section of this paper. Describe your plan for improvement and management. Each student must design a different audit (structure, outcome, or process). For instance, who are the involved players (disciplines), how often will the indicator measured, how will the outcomes be measured (tool or data collection sheet), how often will the data be reviewed.
5. Analysis: How does the literature support the plan? Within the literature, identify three (3) topics we discussed this semester in relation to leadership and/or management that would support the plan.
6. Summary: Summarize your paper with some thoughts on relevance to population health, diverse populations and cultural competence, and nursing.

Introduction (5pts)
Sepsis is one of the most life-threatening medical emergencies and hence not mentioned a
lot in hospital care settings. It is the body's extreme response to an infection, which can
deteriorate the condition more leading to tissue damage, organ failure and even death. According
to the CDC, each year at least 1.7 million adults in America develop sepsis and 270,000 people
die as a result of sepsis. In hospitals, 1 in every 3 patients dies due to sepsis. Due to an increasing
number of cases, Sepsis has become a significant health care burden in hospitals accounting for
more than $24 billion in hospital expenses, representing 13% of total US hospital costs (Paoli et.
Al). Early prevention and recognition for sepsis is the key to management. Health care providers
and nurses should be provided with adequate training to recognize the sepsis early possible to
prevent any adverse effects. The World Health Assembly (WHA) and World Health
Organization (WHO) in May 2017 made sepsis as a global health priority and urged to improve
the prevention, diagnosis and management of sepsis to improve patient outcomes (Seymour et.
Al.). In this paper we will talk about the importance of early sepsis recognition and its
management.
