Quality Improvement and Management Project – Fall 2020

4. QI/QM Plan: Create a Quality Improvement plan that can be initiated at this clinical site. It should be based on the information you obtained and reported in the “Problem” section of this paper. Describe your plan for improvement and management. Each student must design a different audit (structure, outcome, or process). For instance, who are the involved players (disciplines), how often will the indicator measured, how will the outcomes be measured (tool or data collection sheet), how often will the data be reviewed.

5. Analysis: How does the literature support the plan? Within the literature, identify three (3) topics we discussed this semester in relation to leadership and/or management that would support the plan.

6. Summary: Summarize your paper with some thoughts on relevance to population health, diverse populations and cultural competence, and nursing.

         Sepsis is one of the most life-threatening medical emergencies and hence not mentioned a

lot in hospital care settings. It is the body's extreme response to an infection, which can

deteriorate the condition more leading to tissue damage, organ failure and even death. According

to the CDC, each year at least 1.7 million adults in America develop sepsis and 270,000 people

die as a result of sepsis. In hospitals, 1 in every 3 patients dies due to sepsis. Due to an increasing

number of cases, Sepsis has become a significant health care burden in hospitals accounting for

more than $24 billion in hospital expenses, representing 13% of total US hospital costs (Paoli et.

Al). Early prevention and recognition for sepsis is the key to management. Health care providers

and nurses should be provided with adequate training to recognize the sepsis early possible to

prevent any adverse effects. The World Health Assembly (WHA) and World Health

Organization (WHO) in May 2017 made sepsis as a global health priority and urged to improve

the prevention, diagnosis and management of sepsis to improve patient outcomes (Seymour et.

Al.). In this paper we will talk about the importance of early sepsis recognition and its

management.

 Problem (15 pts)

The problem we chose for this paper is Mortality related to sepsis and how can we aid to reduce

the deaths associated with sepsis. Joint commission on Accreditation of Hospitals states that

outcomes are measured, and if there have been any documented changes based on these

outcomes and evaluations.  Once again, you may want to use secondary headings if you discuss a

new idea.

Analysis (15 pts)

Start writing here.  Using 3 topics you have learned and read about during lecture, use the

literature, with citations, as a confirmation and comparison of how leadership and management

aspects would support the plan.  For example, how would delegation influence the Q.I. issue?
