At John C. Lincoln Health Network, our mission serves as a beacon to guide and inspire us as we work to meet the needs of our community head on. I am extremely proud of what the John C. Lincoln team has accomplished over the past year. Our dedicated team has worked together in our ongoing quest for continuous improvement, and we have achieved great things.

First, we have expanded and enhanced our physical locations to ensure that they continue to meet and exceed the needs of those who require our services. In 2012, we broke ground on our new state-of-the-art John C. Lincoln Sonoran Health and Emergency Center in far North Phoenix. We also created a new “Pathway to Healing” at John C. Lincoln North Mountain Hospital, a lobby and communal area that welcomes our patients and their families in the spirit of hope as they work toward achieving wellness.

We are evolving our services, too. In 2012, we added six new practices to our Physician Network. We also launched G-60, our new geriatric trauma services program at John C. Lincoln North Mountain Hospital, to provide effective and expedited care for our senior trauma patients.

In our effort to deliver more integrated and efficient care, we developed our Accountable Care Organization (ACO). This new entity consists of groups of doctors and other providers who work together to carefully coordinate wellness-focused care for Medicare patients. Our ACO employs retired military medical personnel as specially trained transition coaches who provide our patients with resources and assistance as they move from the hospital to their homes, rehab centers or skilled nursing facilities.

To streamline our services even more, we launched our new electronic health record system, JCL Connect, which allows our physicians, hospitals and even patients to confidentially and seamlessly share health information to enhance the quality of care.

As we prepare to meet the future health care needs of our neighbors, we will continue to look to our mission for guidance and inspiration. After all, we are here to help build healthy and strong communities — and together we’re up to the challenge.

ABOUT THE NETWORK

The John C. Lincoln Health Network is an award-winning system of hospitals, physician practices and community programs honored and recognized by local and national health care experts, by our own employees and, most importantly, by the people we serve.

At John C. Lincoln North Mountain Hospital and John C. Lincoln Deer Valley Hospital, patients and their families find advanced medical technology and clinical approaches along with physicians, nurses and team members who provide superior care in a compassionate setting. Both hospitals provide the highest level of cardiac care available.

Our physician practices serve tens of thousands with primary care and specialty care throughout the Valley. The John C. Lincoln Health Foundation creates

opportunities for those in the community to have a long-term impact by funding programs to help others and advance clinical care.

Through Desert Mission, founded in 1927, we stay true to our roots and reach out to provide services to the underserved in the community and improve our community through the Food Bank, Community Health Center, Children’s Dental Clinic, Marley House Behavioral Health Clinic, Adult Day Health Care, Lincoln Learning Center and Neighborhood Renewal.

JOHN C. LINCOLN DEER VALLEY HOSPITAL

The 204-bed hospital with all private rooms excels in emergency heart care, pediatric emergency and inpatient care, orthopedics, spine, and many other specialties. The hospital has a provisional Level III Trauma designation and a pediatric emergency certification and ranks among the top hospitals in patient care according to publicly reported data from the Centers for Medicare & Medicaid Services.

JOHN C. LINCOLN NORTH MOUNTAIN HOSPITAL

The 262-bed hospital serves as the Valley’s northernmost Level I Trauma Center, taking care of the most severely injured patients. It’s a two-time recipient of the prestigious Magnet designation for excellence in nursing care as well as the home of two nursing units honored with the Beacon Award for Critical Care Excellence. North Mountain is accredited as a Primary Stroke Center.

Growth and Innovation

“Pathway to Healing” at John C. Lincoln North Mountain Hospital: The transformation of the lobby and surrounding areas opened at the end of November as a “Pathway to Healing” for patients and families on a journey to wellness. David and Joan Lincoln and Lillian Lincoln Howell donated $4 million to the project. Additional major contributors included the John C. Lincoln Health Foundation Guild, Lincoln GIVES, the Network employee giving program, and the John C. Lincoln North Mountain Auxiliary. Their generosity allowed us to reinvent the patient experience and create a lobby that reflects the quality of care.

John C. Lincoln Accountable Care Organization: In July, the Centers for Medicaid & Medicare approved the John C. Lincoln Accountable Care Organization. This allows patients, physicians and hospitals to work more closely together and improve patient care by promoting better management of chronic conditions, wellness and collaboration.

JCL Connect: The hospitals and many physician practices launched JCL Connect, a sophisticated and technologically advanced electronic health record system that manages patient records. JCL Connect also allows the physician practices and hospitals to confidentially and seamlessly share health information. Patients can monitor their own health records – and their children’s or elderly parents’ – with JCLMyChart.

John C. Lincoln Sonoran Health and Emergency Center: Ground was broken for construction of the $18 million John C. Lincoln Sonoran Health and Emergency Center, situated on 22 acres near I-17 and Carefree Highway, in Phoenix. The emergency department, diagnostic imaging service center and breast center make up the first phase of construction that will be completed in 2013.

G-60: G-60, the geriatric trauma services program at John C. Lincoln North Mountain Hospital, is designed to provide effective and expedited care for trauma patients 60 and older. The program establishes a multidisciplinary

medical team, an accelerated treatment plan and a designated inpatient unit at John C. Lincoln North Mountain Hospital to improve outcomes for older trauma patients. The program is being expanded to John C. Lincoln Deer Valley Hospital in 2013.

Da Vinci Si Robotic Surgical System: John C. Lincoln Deer Valley Hospital added the da Vinci Si Robotic Surgical System. Now both hospitals have the capacity to perform minimally invasive robotic surgery. The system translates the surgeon’s hand, wrist and finger movements into precise, real-time, minimally invasive movements of laparoscopic surgical instruments inside the patient. Deer Valley’s system is the first in Arizona to have capacity for “Firefly” Fluorescent Imaging, a skill simulator and a vessel sealer. Each hospital is designated as a distinguished da Vinci Epicenter, providing new da Vinci surgeons from around the country with opportunities to observe cases and learn from John C. Lincoln physicians.

Specialty Surgical Care: John C. Lincoln North Mountain Hospital’s completely renovated third floor opened in January with all private rooms for post-surgical patients. The floor creates a new dimension in care that blends medical technology with comfort and convenience.

60th Anniversary of the North Mountain Hospital Auxiliary: In January 1952 — the same year the Desert Mission Convalescent Hospital was licensed by the Arizona Department of Health — 28 ladies met as the Network’s first volunteer group, the Women’s Auxiliary, to raise money and support the facility. Auxiliary members raise money for community and hospital projects as well as provide almost half of the North Mountain Hospital’s volunteer hours. Auxilians make up part of the 1,779 volunteers of the John C. Lincoln Health Network.

We served 39,805 unduplicated clients through all of the Desert Mission programs, a 15 percent increase from the year before. That’s almost 10,000 more clients than we served in 2010.

We reached hundreds of community members through free support groups for breast cancer, diabetes, stroke and caregivers and through a variety of free health screenings and physician education seminars.

Additionally, the Network provided $37 million in uncompensated care to patients who could not otherwise afford health care.

DESERT MISSION

Families need more than a box of emergency food in tight times. Through an integrated approach, Desert Mission’s programs – Food Bank, Community Health Center, Children’s Dental Clinic, Marley House Behavioral Health Clinic, Adult Day Health Care, Lincoln Learning Center and Neighborhood Renewal – work together to meet the needs of the whole family.

40,752 emergency food boxes distributed at Desert Mission Food Bank.

40,352 Snack Pacs (emergency weekend food for children) distributed to local schools.

8,550 students who received dental screenings at school and 3,420 visits to the Children’s Dental Clinic.

4,347 patient visits to the Community Health Center.

8,719 participant visits to Adult Day Health Care, with an average daily census of 32.

1,866 visits to Marley House Behavioral Health Clinic.

447 children enrolled at Lincoln Learning Center.

121 new homeowners created through Neighborhood Renewal.

VOLUNTEERS

Our volunteers served in many capacities throughout the Network, ranging from greeting patients and visitors to packing emergency food boxes for families in need. Together, they added $4.1 million in value to the Network, the equivalent of 91 full-time positions.

1,779 volunteers provided 189,052 hours of service to the Network.

34 pet therapy teams visited 23,522 people.

$201,482 raised for the Network, including $38,100 for scholarships for students pursuing degrees in health care and $17,164 for pediatric radiology equipment at Deer Valley.

44 volunteers visited 22,061 patients and provided hospital items.

43,888 volunteer hours were served at Desert Mission.

In 2012, almost $5.5 million in philanthropic contributions to the John C. Lincoln Health Foundation touched lives throughout the community by providing nutritional food for families and children, funding life-saving patient care, improving our facilities and technology, giving hope to breast cancer patients, counseling families and children, offering community screenings and support groups, and more.

Awards and Recognitions

Lifetime Achievement Award: David Lincoln and the Lincoln family were honored with AZ Business magazine’s first Lifetime Achievement Award in its 2012 Healthcare Leadership Awards for the family’s longtime and ongoing commitment to the health of the community through the John C. Lincoln Health Network.

MAP Award for High Performance in Revenue Cycle: John C. Lincoln Deer Valley Hospital is one of eight hospitals in the country and the only one in Arizona recognized for its top performing Revenue Cycle Department from the Healthcare Financial Management Association.

This recognizes Deer Valley’s best practices in achieving excellence and in ensuring patient care. It also recognizes that the hospital has a sustainable financial performance as well as innovative and effective practices that look at the future. North Mountain did not apply as the rules for hospitals with trauma care are different; however, the same standards are upheld throughout the Network with all Revenue Cycle practices.

Startup America Policy Challenge: “A New Dimension of Service to the Country” proposal was the national winner in the challenge sponsored by the White House, the U.S. Departments of Education, Energy, and Health and Human Services in conjunction with Arizona State University. The idea, which now is in place at John C. Lincoln Health Network, hires military medics as health transition coaches to assist Medicare patients after hospital discharge.

2012 Sylvia Moss Brown Award of Excellence: Shelly Diana, RN, CCRN, ICU charge nurse; Jodi Todd, RN, ICU nurse; Amanda McFarren, RN, BSN, ICU nurse; and Kari Johnson, RN, APRN-BC, MSN, clinical nurse specialist, critical care, all from John C. Lincoln North Mountain Hospital, received the national Sylvia Moss Brown Award for Excellence and a $10,000 grant for their work on early recognition of delirium in trauma patients.

Beacon Award for Excellence: The Cardiovascular Intensive Care Unit at John C. Lincoln North Mountain Hospital earned a silver-level Beacon Award for Excellence in Critical Care from the American Association of Critical-Care Nurses. This is the second Beacon Award for the unit.

Grassroots Advocacy Champion Award: The American Hospital Association, in partnership with the Arizona Hospital and Healthcare Association, has awarded Bruce Pearson, senior vice president and CEO of John C. Lincoln North Mountain Hospital, with the American Hospital Association Grassroots Advocacy Champion Award for exceptional leadership and championing of patient’s access to quality health care. A winner is selected from each state.

50 Most Influential Women in Business: AZ Business magazine named John C. Lincoln Health Network President & CEO Rhonda Forsyth one of the “50 Most Influential Women in Business” in Arizona.

NAEYC Accreditation: Lincoln Learning Center earned re-accreditation, its fourth consecutive, from the National Association for the Education of Young Children (NAEYC), the nation’s leading organization of early childhood professionals. Only 8 percent of the nation’s preschools and other early childhood programs are NAEYC accredited, showing they meet the highest standards of care paired with a child-centered teaching approach.

Forty Under 40 Class: Ann-Marie Alameddin, JD, Network attorney, was chosen as part of the 2012 Forty Under 40 class for Phoenix. She was selected from nearly 300 candidates based on professional and community achievements, commitment to making a difference and leadership.

Top 25 Workplaces for Women: AZ magazine honored the network for its support of women in a number of areas including women in management, efforts to move women into leadership roles and policies to help working women care for their families.

Trace3 IT Outlier Award Finalist: Robert Slepin, the vice president and Network chief information officer, was named a finalist for the Trace3 IT Outlier Award for 2012 by CIO magazine, a national award that honors technology executives for innovation, technological performance and personal commitment.

UnitedHealth Premium Total Joint Replacement Specialty Center: John C. Lincoln Deer Valley Hospital was recognized by UnitedHealthcare in recognition of its quality and outcomes.

Dr. Daniel T. Cloud Outstanding Practice Teen Award: Desert Mission Community Health Center received the 2012 Dr. Daniel T. Cloud Outstanding Practice Teen Award from the Arizona Partnership for Immunization for its 97 percent immunization rate for patients between 11 and 15 years old and for showing leadership and dedication in the field of immunization.

Midas+ Platinum Quality Award: John C. Lincoln Deer Valley Hospital was ranked as a top performing hospital, in the top 5 percent of hospitals, tracked in key clinical care areas in the Midas+ National Comparative Database.

U.S. News & World Report’s Best Hospitals: John C. Lincoln North Mountain Hospital has been ranked 12th in the state and 10th in the Valley the U.S. News & World Report Best Hospitals edition.

Gold Medal for Organ Donor Registrations: John C. Lincoln North Mountain Hospital received a gold medal for the Most Registrations for organ donations via DonateLifeAZ.org in the Hospital Donor Registry Challenge.

Sunnyslope Art Walk: Phoenix New Times in its Best of Phoenix Awards named the Sunnyslope Art Walk the Best Arts Festival 2012.

MVP Award, Most Valuable Phoenician: For her work and dedication to improving the community of Sunnyslope, Neighborhood Relations Manager Christina Plante received an MVP Award for being a Most Valuable Phoenician.

NMRI Annual Workshop Award: Alicia Mangram, MD, and James Dzandu, PhD, received the Clinical Translational Research Science award for their work on identifying diabetes in trauma patients and their presentation at the NMRI (Network of Minority Health Research Investigators) Annual Workshop during the Dr. Marco Cabrera Poster and Networking Session.

Aces of Hearts Award: Arizona Emergency Medical Systems honored Network Prehospital coordinator Tracy Moroney, RN, BSN, BCEN, for providing outstanding service in EMS or Trauma for the Central Arizona Region.

Commitment to Care Award: Nelson Faux, MD, EmCare Regional Medical Director and North Mountain director of Emergency Medicine, was selected out of 7,000 EmCare physicians in the United States as the outstanding emergency medicine physician for the year.
