Each prospective CM/Contractor must have the following California General Building Contractor’s License: B, current, active and in good standing with the California Contractor’s State License Board on the date and time of the Prequalification Questionnaire submittal is due. This Prequalification Questionnaire with all portions completed, including required attachments must be submitted.

Each prospective CM/Contractor must answer all of the following questions and provide all requested information, where applicable. Any prospective CM/Contractor failing to do so may be deemed to be non-responsive and not responsible with respect to this Prequalification at the sole discretion of the University. Each prospective CM/Contractor must submit three (3) printed sets and one (1) flash drive with complete submittal of the questionnaire. All CM/Contractor’s which have submitted a Level I Prequalification Questionnaire will be notified in writing of either successfully or not



successfully achieving Level I prequalification status. The decision of the University is final and is not appealable within the University of California system.

6. CONSTRUCTION EXPERIENCE

Submit Project Data on a minimum of three (3) and a maximum of five (5) comparable projects successfully completed within the last ten (10) years constructed in the United States of America one of which was constructed in the State of California.

A comparable project is defined as having a construction cost at the bid date of at least $2,000,000 or a total of $8,000,000 for the projects submitted, and one of the following example building types:

Physical Sciences and/or Engineering research laboratories, specifically laser laboratories or cleanroom facilities.

Other facilities that contain a high degree of technical complexity and

Such projects should have possessed the following construction challenges:

ISO cleanroom construction including the installation and commissioning of complex mechanical systems with extremely tight temperature and humidity controls.

Laboratory facilities utilizing displacement ventilation systems. Occupied campus site with limited construction and staging areas. Project complexity requiring tracking of multiple functions and phases. Relocation of major MEP/FS utilities in an occupied building with no/minimal

downtime. Project complexity requiring construction management services. Proactive preconstruction services including constructability reviews, productive

design team relationship, robust subcontractor prequalification process, accurate cost estimating, and effective procurement/scheduling strategies.

Construction coordination leading to the successful completion of projects within critical time frames on an accelerated schedule.

Documented critical path scheduling used to complete projects on time and within budget without compromising the aesthetics or construction quality of the project.

Two (2) of the projects submitted must include experience as CM/Contractor providing both pre-construction services and acting as General Contractor during construction phase on the same project.

9. STAFF EXPERIENCE AND PROJECT SAFETY

The Project Manager, and Project Superintendent listed will be considered qualified only if he/she has successfully completed at least two (2) comparable projects under a CM/Contractor Construction Contract.

A. CM/Contractor hereby commits as a minimum to assignment of the specific field staff as outlined below. Interviews with these individuals will take place during Level II of the Prequalification. CM/Contractor to submit a complete staffing chart as part of this package.

One Project Executive (part-time) during pre-construction and constructionThe Project Executive will be on site part-time for construction (i.e. Option Phase 2) and on site for construction meetings.

One Project Manager (a minimum of 2 full days per week on site) during constructionThe CM/Contractor will assign one project manager assigned to oversee, manage and coordinate the overall project as a whole. The magnitude and complexity of the project will necessitate that this position manage the entire construction process. This position will need to be involved with all aspects of the project including but not limited to all pre-construction scheduling and budgeting meetings, overall construction process development and execution, multi-staff operation management and coordination. The project manager will take the lead position in all architect and University issues including overall contract administration, constructability and logistic reviews, RFI, RFP and contract change order negotiations, campus and community relations. The project manager will maintain the point position on behalf of the general contractor and will be the key person in charge guiding the construction development process to successful closure, both budget wise and schedule wise.

One Project Superintendent (full-time on site) during constructionThe CM/Contractor will assign one project superintendent to manage, coordinate and facilitate the field supervision staff for each of the various components of the project construction process. The major function of this position will be in addition to the normal superintendent’s daily workload, subcontractor interaction and production, and various field related coordination issues.

One Project Engineer (part-time on site) during constructionThe Project Engineer shall perform Quality Control duties, and MEP coordination.

One Pre-Construction Manager (part-time) during pre-construction. The CM/Contractor will assign one (1) pre-construction manager assigned to oversee, manage and coordinate the pre-construction services. The magnitude and complexity of the project will necessitate that this position manage the entire pre-construction process.



This position will need to be involved with all aspects of the project including but not limited to all pre-construction scheduling, budgeting, value engineering, constructability review, bid package strategy and preparation, and attendance at University-Architect-Contractor meetings. The pre-construction project manager will maintain the point position on behalf of the general contractor and will be the key person in charge guiding the pre-construction project development.

One Clerical Assistant/Receptionist (part-time) during construction

B. At the time of fee bid, the successful CM/Contractor will be required to reconfirm staff assignments to the project based on this submittal. If any of the named staff submitted are no longer employed by the firm at the time the project starts, or are otherwise unavailable, the firm's fee bid may be considered non-responsive. Substitution of other individuals with equivalent experience may be considered by the University, however, resumes; comparable project history and other relevant information must be submitted to the University prior to the determination of the fee bid results.

C. Project ManagementThe CM/Contractor shall keep on the job throughout its duration a competent Project Manager and Project Superintendent, all of whom must be satisfactory to the University. The Project Manager and Project Superintendent shall be the same individuals proposed by the CM/Contractor during the procurement process for this project. The Project Manager shall represent the CM/Contractor, and all communication given to the Project Manager shall be as binding as if given to the CM/Contractor. The CM/Contractor shall not change either the Project Manager or the Project Superintendent on the project from those originally proposed for the project without the prior written consent of the University. The University will only grant written consent for such change in the case of undue hardship on the individual or if the Project Manager and Project Superintendent shall leave the employ of the CM/Contractor.

By submitting a proposal for this project, the CM/Contractor agrees to pay a training fee of $5,000 should they change Senior Project Manager/Project Executive, Project Manager, or Project Superintendent without the written consent of the University.

D. Safety ProgramThe safety of the CM/Contractor employees, employees of the Design Team and other visitors to the Project are of the utmost importance to the University. The CM/Contractor shall take whatever steps are necessary to maintain a clean and safe work environment for their employees, the employees of their Subcontractors and vendors, and any other visitors to the project.

Definitions

A. Schedule (Assume preparation of 1 preliminary schedule and 1 detailed schedules).

The successful CM/Contractor shall prepare a preliminary project schedule using Primavera Project Planner with no less than 50 activities. The successful CM/Contractor shall prepare detailed project schedules using Primavera Project Planner with no less than 150 activities. These schedules shall include phasing required for the work and any major milestones indicated by the University. Included in the schedules shall be allowances for normal delays caused by weather, etc.

B. Cost Estimating / Cost Control (Assume 2 detailed estimates).The CM/Contractor shall prepare a detailed estimate at Design Development, at 50% and 90% construction documents. The estimate shall be detailed to the extent that it includes quantities, unit prices, and extensions. This detailed estimate will be broken into the various bid packages that the CM/Contractor proposes to issue, and this estimate shall represent the CM/Contractors best estimate of the probable low bid received on bid day for that bid package.

C. Value Engineering Review and Constructability ReviewConcurrent with estimate, the CM/Contractor will also perform a detailed value engineering and constructability review of the project. It is the intent that this review provides recommendation to the University and the Design Team on possible alternate design schemes that could result in a similar final product but at a reduced cost or schedule for the University. Additionally, the University expects the CM/Contractor to review the document for any constructability issues such as hoisting, access, conflicting information on the documents, coordination issues between disciplines, and the like so these issues can be resolved prior to the start of construction. While it is not the intent of the University that CM/Contractor become a designer of record, it is the intent of the University that the University receives benefit of the CM/Contractor’s experience in the construction industry.

D. Bid Package Strategy and PreparationThe CM/Contractor shall develop for the University’s review a strategy for packaging the various elements for the project for bid to trade contractors. While the University may comment on the strategy developed by the CM/Contractor, the ultimate responsibility for the bid packaging and completion of the project and schedule rests with the CM/Contractor.



E. University-Architect-Contractor Meetings (Assume 1 meeting per week for 6 months)

CM/Contractor shall identify and prepare a budget based upon hourly rates for attending University-Architect-Contractor Meetings during the pre-construction phase of the project.

The prospective CM/Contractors shall propose a budget for each of the above activities to be preformed during the pre-construction phase of the project. This budget shall include title or function of individuals working on the above pre-construction activities, hourly rates for each title or function, and extended costs.
