•45 accidents or incidents were declared in 2005, 14 were followed by an inquiry, and probably many more were not declared.

•Manual handling, falls, incidents or accident of electrical origin, etc.

•Procedures badly applied, changed at the last minute, not followed or non existing.

•Personnel involved: sub-contractors, interim or experts who are over-confident.

Recurrent Problems

•PPSPS not done, or at the last minute, generic, copy-paste, not adapted to the situation

•Works not declared, or continuing beyond what is declared or authorised

•Safety perimeters not respected

•Lack of supervision and preparation

•No smoking and personal protection

•Bidders’ conference - rules are presented

•PPSPS - 30 days before start of work with precise and full task description

•Work Package Analysis, experimental zones

•Joint inspection (VIC) - one week before start of work

Work at height

•Accident of the Queen Mary in St. Nazaire: 15 dead (Endel)

•Decree of Sept. 2004 : Scaffolding, platforms, work at height

•Structure calculations, qualification of personnel (erection, control, etc.), training, work with harness, etc.

•New European rules in 2006

Working rules for firms

•Night and Sunday work is forbidden in France and Switzerland

•Derogation only for safety reason (transport, X-rays) or continuity of work (Site guards)

•Strictly forbidden to lend personnel (CERN, company, collaboration, etc.)

Consequences of an accident at SLAC

• Electrical arc when installing a circuit breaker in an electrical cabinet.

• 2nd and 3rd degree burn of a person from a outside sub-contractor.

• Stop of all activities during a month and all accelerators for 6 months.

Conclusion

•An accident may have catastrophic consequences on the whole project

•The level of safety revels the respect of the schedule, of quality and of the environment

•Everybody shall be involved

•Compression of the planning must be done without compromise on safety
