Mackenzie Health, formerly York
Central Hospital received designation
as a District Stroke Centre in 2005. We
participate in ongoing education and
training in stroke care, providing
coordinated stroke services for York
Region. We work with providers across
the health care continuum. We receive
leadership, support, and education from
our Regional Stroke Centre, Royal
Victoria Hospital in Barrie.

PREVENTION CLINIC
The Stroke Prevention Clinic is an
outpatient clinic for people who have
experienced signs and symptoms of a
recent stroke or transient ischemic attack
(TIA). The goals of the clinic are:

Services– Mackenzie Health The Cardiovascular Rehabilitation
Services promotes overall cardiovascular
wellness. An exercise program will be
tailored for each individual. Individual
and group counseling sessions are also
available to provide nutrition and

Cardiovascular Prevention and

to inspire, educate, monitor, and support

Ontario Stroke Network The Ontario Stroke Network (OSN) was created in 2008 – funded by the Ministry of
Health and Long-Term Care – to provide provincial leadership and planning for the
continuum of stroke care in Ontario. Its key function is to enable the advancement of the
Ontario Stroke System – consisting of all Ontario Stroke Centres and Stroke Prevention
Clinics – primarily through leading provincial initiatives, credible advisor, evaluating and
driving system performance, as well as coordinate research and knowledge management
programs.

STROKE SURVIVOR GROUPS Meetings are for stroke survivors and their caregivers. The groups generally meet once a
month and often have guest speakers. All meetings are wheelchair accessible, and all

Community Care Access Centre
CENTRAL COMMUNITY CARE ACCESS CENTRE
(CCAC)
The Central Community Care Access Centre (CCAC) is part of a province-wide
community care network that connects people of all ages to the care they need at home
and in the community.
Central CCAC staffs are nurses, occupational therapists, social workers and other
regulated health professionals with specialized clinical expertise, who are trained to
assess people’s health and care needs, and connect them to a wide range of CCAC
services and other community resources.
Services provided by CCAC include nursing, personal support and therapy services, as
well as specialized services such as rapid response nursing, palliative care and child and
family services. Central CCAC also helps many people explore other care options such as
adult day programs, retirement homes, short and long-term care and supportive housing.
Through personal health assessments, in-home visits and regular check-ins, Central
CCAC manages the care and community supports people need to return home from
hospital faster, to heal at home, and to live at home longer, independently and with
dignity.

aphasia, apraxia, dysarthria, and other social communication disabilities due to stroke,
traumatic brain injury, or neurological diseases.

MENTAL HEALTH SERVICES
Mackenzie Health – Richmond Hill Hospital Mental Health Programs
Aftercare Clinic Offers long term care to adults living with a chronic and persistent mental illness.
Components of the clinic include: medication management, administration of depot
neuroleptics, and monitoring compliance for Clozaril by a Nurse and Psychiatrist. A
Social Worker and Occupational Therapist offer a weekly group program intended to
promote community living skills.
Crisis Intervention Services Crisis Intervention Services provides assessments to the Emergency Department and
consultations to the medical floors for individuals experiencing a mental health crisis. We
no longer provide out-patient assessments or follow-up care (these services are provided
through the Urgent Clinic and/or The Shaw Clinic).
The focus is to resolve the immediate crisis to assist the individual to return to a pre-crisis
level of functioning.

Day Program Provides daily, short term, group treatment designed to stabilize mood and improve day-
to-day functioning. Appropriate for clients requiring intensive treatment and for those
safe to return home each day.
Outpatient Adult Services Adult Services provides treatment to adults 18 years and older who are experiencing
acute or significant difficulties related to their emotional and psychological health. The
Adult Outpatient Services has a broad range of assessment and treatment options for
reducing symptom distress and dysfunction in most areas of mental health including
depression, anxiety and relationship difficulties. Psychiatric consultation is available in
conjunction with ongoing therapy as appropriate. Referrals can be made by calling
Psychogeriatric Clinic The Psychogeriatric Assessment and Outreach Service is comprehensive assessment and
consultation service to physicians in York Region for those 65 years and older
experiencing mental health difficulties and who are not requiring crisis or urgent
intervention.
Urgent Clinic Psychiatric assessment and /or brief treatment of adults, ages 19 and above whom
require urgent psychiatric intervention in an outpatient setting. Patients under age
19 may be referred from the Emergency room only.
Urgent Clinic is located at Mackenzie Health on 3West, Room 3931. Operational
hours are Monday to Friday from 0900-1700 hours.
Referrals are accepted for clients who:
a) reside in the catchment area (north to Bloomington Sideroad, south to
Steeles Ave. east to Hwy 404 and west to Hwy 400)
b) Whose physicians’ practice is within Mackenzie Health catchment area.
c) have a physician who is affiliated with York Central Hospital
Appointments are booked on a priority basis.
The nurse consults with the psychiatrist when required.
Clients are seen within a 24-72 hour period.
Length of service is typically 1-6 sessions.

Vaughan Community Health Centre
The Vaughan Community Health Centre’s (VCHC) Diabetes Program provides free
education for adults living with Type 2 Diabetes or pre-diabetes. The VCHC diabetes
education program helps you learn how to improve your quality of life by providing

We offer both individual counseling and group sessions with Registered Dietitians,
