Purpose The purpose of this transaction is to Upload all Medical reports.

Business Scenario In this scenario the Health Care Provider (HCP) Uploads the First Medical Report into the CompEasy System.

Prerequisites The following prerequisites are applicable when processing this transaction:

• A Valid Claim.

1.1.1. Home - Google Chrome

The Health Care Provider will use the Medical Report custom APP (Application) to upload Medical Reports.

Click the Submit Medical Report tile to access the transaction.

[2] Click the Service Provider Number possible entries button to search for the required value.

Enter the Service Provider number or use the Search functionality available. In this example the Search functionality will be demonstrated.

• The claim number.

• ICD10 codes (according to the national standard rules).

• The consultation date.

• Relevant date from and to.

• Return to work dates (optional).

The Date of Loss is pre-populated, the system defaults this from the existing claim.

When a report is not listed, select the option "Medical Additional Supporting Documents". It is important to add the name of your Report in the Notes field below.
