SCIENTIFIC MANAGEMENT FLORIAN BAUER, MD, DDS, PHDFlorian Bauer, MD, DMD graduated from the University of Munich (Lundwig-Maximilian-Universität) in
2002. He continued his study and graduated a second time from the Medical School (Ludwig-Maximilian-
Universität) of Munich and the Technical Universität of Munich in 2007.
He did scientific research, developed a new sinusfloor elevation system, called “The balloon-assisted-
sinusfloor elevation system BLC”. In addition he wrote two thesis one in 2004: Thesis: „The Influence
of platelet-rich-plasma (PRP) on sinus floor bone grafts” and the other in 2010: Thesis: „Investigation
of functional loaded dental implants in humans“. Dr. Bauer is since 2008 resident at the department of
maxillofacial surgery at the Technical University of Munich. Since 2002 he is lecturing internationally,
in Europe, Asia and the USA. Dr. Bauer is the cofounder of a very successful foundation for dental
students. He won several awards, did a lot of bone management lectures and workshops and founded
his own dental clinic in Miesbach, Bavaria.
DR. ALEXANDER ZASTERADr. Alexander Zastera graduated from the University of Münster in 2016, receiving his D.M.D with
magna cum laude. In 2017 he started his residency program in oral surgery in a private practice in
Germany and continued his residency in 2018 at Prof. Dr. Khoury’s private clinic in Olsberg, Germa-
ny. From 2018 until 2019 he successfully attended the postgraduate Master of Science program in
implantology and dental surgery at the International Medical College Münster.
FOUAD KHOURY, DMD, PHD, PROF. DR. MED DENT.Dr. Fouad Khoury received his dental education at the St. Joseph University in Beirut, Lebanon. Speciali-
zation in Oral Surgery at the Universities of Freiburg and Muenster, Germany. He was assistant Professor
at the University of Muenster completing his PhD and scientific habilitation Degree. He is since 1994 full
professor at the Department of Oral & Maxillo-Facial Surgery of the University of Muenster and Chairman
of the Privatklinik Schloss Schellenstein, Olsberg, Germany. Since many years he is the Chairman of the
examination comity for Oral Surgery and Board member of the German Society of Oral Surgery.
Prof. Khoury received several Prices (e.g. Pioneers in Dentistry 2016 AUB, W. Laney Award 2017, Ame-
rican A. Osseointegration) and has many Patents. He is author and editor of 3 bestseller Textbooks trans-
lated to more than 10 languages. He is member of Editorial Board of different journals and has more than
135 Publications in peer-reviewed Journals as well as more than 1000 Lectures / Courses worldwide.
MICHAEL S. BLOCK, DMDMichael Block graduated from the University of Rochester in 1975, attaining both a BA in Biology and a
BS in Biomedical Engineering. He completed his dental training at the Harvard School of Dental Medi-
cine in 1979, receiving his D.M.D. cum laude in a special field. He completed his residency program in
Oral and Maxillofacial Surgery at the LSU School of Dentistry in 1983. He remained at the LSU School
of Dentistry achieving the faculty level of Professor in the Department of Oral and Maxillofacial Surgery.
He was teaching one day a week in the dental school’s implant program but now dedicates full time to
taking care of his patients in his private practice.
ISTVAN URBAN, DMD, MDIstvan Urban received his DMD degree and subsequently his MD degree from Semmelweis Univer-
sity School of Medicine and Dentistry (Budapest, Hungary) in 1991 and 1996. He completed a full
time program in oral surgery at St. Istvan Hospital in Budapest, Hungary (1992-1996). He completed
his internship program in Periodontics at UCLA. After he graduated from the Fellowship Program
(1999-2000) in Implant Dentistry at Loma Linda University in Loma Linda, California, he was appointed
assistant professor the following year. Dr. Urban teaches implant dentistry in the graduate program
at Loma Linda University. He is licensed in the state of California (USA) and has a private practice in
Budapest, Hungary. Istvan Urban received his PhD degree in Periodontlogy in 2012 at the University of
Szeged, Hungary. He is currently an honorary professor at the University of Szeged. Istvan Urban has
published scientific articles and text book chapers on bone regeneration and soft tissue reconstructive
surgery around dental implants.
STAVROS PELEKANOS, DDSStavros Pelekanos graduated School of Dentistry, National and Kapodistrian University of Athens
in Greece and received Doctoral promotion Dept. of Prosthodontics, School of Dentistry, Albert-
Ludwigs University of Freiburg, Germany. He was a clinical instructor, Dept. of Prosthodontics, School
of Dentistry, National and Kapodistrian University of Athens from 1994-2001. He has published many
scientific articles and abstracts, 1 chapter in a book, and received 2nd prize at the scientific award
competition of the EAED in Madrid, Spain, 2008.
GILES B. HORROCKS, DDS, MSGiles Horrocks completed his specialty training in Periodontology in the Department of Medicine and
Surgery at the Veterans Administration Medical Center in Milwaukee, Wisconsin. He is board certified
and a Diplomate of the American Board of Periodontology. He has written a chapter on Piezosurgery
Assisted Controlled Ridge Expansion and has a recent publication in the International Journal of Perio-
dontics and Restorative Dentistry on a technique he developed called Controlled Assisted Ridge Expan-
sion (CARE), which is a minimally invasive surgical approach to alveolar bone volume enhancement. He
has also recently published a chapter on Periodontal and Peri-Implant Soft Tissue Surgery in Minimally
Invasive Dental and Implant Surgery by Wiley Blackwell.
RAWAD SAMARANI, DDS, DES, MRESRawad Samarani graduated as Doctor of Dental Surgery in 1998, and in 2002 he graduated in
Periodontology (Diplôme d‘Etudes Supérieures) from the Saint-Joseph University in Beirut. Since 2002,
he is a member of the Periodontology Department of the Saint-Joseph University where he is serving
as a senior lecturer and a clinical instructor. In 2013, he obtained from the same school a Master
of Research. He published 13 papers in international peer-reviewed journals including the European
Journal of Oral Implantology, the International Journal of Oral and Maxillofacial Surgery, the Journal of
Oral Implantology and the Journal of Maxillofacial Surgery. He is the author of the chapter “Revisiting
Guided Bone Regeneration in the Esthetic Zone” in the textbook „Advances in Esthetic Implant Dentistry“
published by Wiley-Blackwell. He is lecturing and giving courses in several international venues on
guided bone regeneration, immediate loading and immediate implantation in the esthetic zone.
MR. ASHOK SETHI BDS, DGDP (UK), MGDS, RCS (ENG), DUI (LILLE), FFGDP (UK)Ashok Sethi has pioneered, in the UK and internationally, the art and science of surgical and restorative
implant dentistry for 25 years. His practice is totally dedicated to implants, achieving outstanding success
rates in terms of predictability as well as aesthetic and functional outcome. He conceived and taught
the Diploma in Implant Dentistry of the Royal College of Surgeons of England. He is the director of the
recently established PID – Academy providing postgraduate education and training in practical implant
dentistry. He is a founding member and past president of the ADI and has been elected as an honorary
life member of the Association. He has written a book “Practical Implant Dentistry” jointly with Thomas
Kaus, which is published by Quintessence International and has been translated into 10 different langua-
ges. The 2nd edition, “Practical Implant Dentistry–the Science and Art”, is now available.
DANIEL CULLUM, DDSDaniel Cullum completed his DDS with distinction at the University of Alberta, Canada, and residency
training at Westchester Medical Center, New York. Daniel Cullum is a Diplomate of the American
Board of Oral and Maxillofacial Surgery. He speaks internationally and has contributed publications
and textbook chapters on implant reconstruction. He has served in leadership roles with the American
Association of Oral and Maxillofacial Surgeons (Dental Implant Sub-Committee), Academy of
Osseointegration (Newsletter Committee), Kootenai Medical Center (Chairman, Department of
Surgery), and at state and local societies. At Implants Northwest (Coeur d’Alene, ID), he practices Oral
and Maxillofacial Surgery with emphasis on immediate and minimally invasive techniques in esthetic
implant reconstruction. Daniel Cullum is also president of Implants Northwest LIVE Learning Center
which emphasizes training in advanced techniques for surgeon/restorative teams using LIVE surgery and
hands-on application in a small group environment.

WORKSHOPDR. GILES B. HORROCKS– Controlled Assisted Ridge Expansion with Simultaneous Implant Placement in the Esthetic Zone

Minimallly invasive surgery means something different to each generation of surgeons. Reducing the surgical events, cost and patient trauma, however, remains constant. This discussion will focus on the placement of dental implants simultaneous with the correction of the horizontal bone deficiency. By manipulating the bone with select releasing incisions and respecting the periosteal/endosteal perfusion, the labial bone can be mobilized and an osteotomy prepared to allow simultaneous placement of the implant. The resulting augmentation follows a healing mechanism of Bone Guided Regeneration.The mobilized cortex is a va-scularized bone pedicle that can allow regeneration of native bone in the space held by the implant. Since membranes and bone fillers are often unnecessary, many complications are avoided. Happy patients.
Educational Objectives:• Gain an understanding of a technique of controlled non-malleted bone manipulation for ridge expansion in the maxilla.• Knowing what anatomic parameters are necessary to provide this treatment alternative that minimizes treatment time, cost and common surgical morbidities.• Understand the biologic rationale of this surgical technique.• Gain an understanding of the limitations in this technique for predictable treatment planning. • Review specialized instrumentation.• How to manage possible complications.• Review cases with 5 and 7 year CBCT follow up.• Look at what implant designs of the future can assist this minimally invasive procedure.
The participant will gain an understanding of the technique controlled assisted ridge expansion. Though this technique is innovative it will be presented in a literature based fashion. Concepts of biologic containment and intrabony subperiosteal bony releasing incisions will have been outlined in the lecture and re-inforced in the workshop.

An immediate restoration of implants is from patient’s side well accepted. It cuts down the treatment time and helps to develope the soft tissue’s profile from beginning.Prerequisite is mechanical firmness of the implant within the existing bone that will be normally measured by a torquewrench. It‘s said that 35 Ncm torque for single implants is safe for immediate loading by a fixed restoration.
To facilitate reaching that level of initial stability we have two options: using implants with extended threads and carrying out undersized preparation of the osteotomy. The MyPlant two-system combines these options by providing a progressive thread design and a range of instruments for dimensional (hard bone) and underdimensional (soft bone) preparation. Actually, 80% of our cases will get an immediate provisional restoration. Clinical application on different implant indications will be demonstrated.
WORKSHOP
DR. DANIEL CULLUM– Advanced Crestal Sinus Elevation “Hands-on Workshop”

Synopsis: Attendees will complete “Hands-on” training in advanced and minimally invasive techniques for Crestal Sinus Elevation. This workshop will develop skills with highly predictable sinus floor elevation techniques at delayed and immediate implant sites. Procedures using crestal side-cutting reamers, piezosurgery, contiguous sinus floor elevation (CSFE) and crestal window (CW) techniques will be undertaken. im-plant placement at sites with larger elevations of 5-9+mm and residual bone heights of 2-4mm will be completed and contrasted to traditional osteotome techniques and lateral sinus grafting. Attendees will use this training to enhance patient care, avoid complications and reduce the healing interval.
Learning Objectives: • Discuss the advantages and limitations of minimally invasive sinus elevation
techniques. • Develop “hands-on” skills with crestal drills, piezosurgery, CSFE, CW and modified
osteotome techniques.• Apply anatomic findings and CBCT planning in crestal technique selection to avoi-
ding complications.
WORKSHOP PROF. DR. ISTVAN URBAN– Vertical Ridge Augmentation

Vertical and horizontal augmentation presents one of the greatest challenges of bone regeneration in implant dentistry. This is primarily due to the difficulty of the surgical procedure and its potential complications.
These complications include postsurgical swelling, bruising, and potential neurosen-sory disturbances.
Minimally invasive flap management that can minimize the chances of these complica-tions will be revealed during this hands-on course.
Graft harvesting, membrane fixation for vertical augmentation as well as for the Sau-sage Technique TM will be practiced.
A step by step approach how to protect the neurovascular bundles such as the mental nerve and to achieve a safe, tension free closure will be learned during this practical.
