PURPOSE In 2000, Alzheimer’s disease was the 7th leading cause of death for all ages, a ranking that changed since then, as the disease was 5th leading cause of death in 2017. Alzheimer’s disease is a progressive degenerative and increasingly debilitating condition characterized by progressive loss of memory, difficulty in finding right words or understanding what people are saying, difficulty in performing previously routine tasks, and changes in personality. Alzheimer’s disease gradually destroys vital nerve cells in the brain. At later stages of the disease, persons with Alzheimer’s disease are bedridden and susceptible to developing other medical conditions, especially pneumonia. These secondary conditions may also result in death. The purpose of this report is to provide information to individuals interested in Alzheimer’s disease mortality among Arizona residents. The data for 2017 are placed in a temporal context by comparison with the data for the preceding years. METHODS AND SOURCES Data on the number and characteristics of Alzheimer’s deaths were obtained from the death certificates filed with the Arizona Department of Health Services. Population denominators for Arizona residents, used to calculate rates, are the latest available estimates by the Arizona Office of Economic Opportunity and census enumerations from the U.S. Census Bureau (www.census.gov/). Beginning with the 2000 data year, two major changes have occurred in Arizona, affecting the computation of mortality rates, tabulation of leading causes of death and analyses of mortality data over time. First, a new revision of the International Classification of Diseases (ICD), used to classify causes of death, was implemented. The Tenth Revision (ICD-10) has replaced the Ninth Revision (ICD-9), which was in effect since 1979. Second, a new population standard for the age adjustment of mortality rates has replaced the standard based on the 1940 population and used since 1943. Both changes have profound effects on the comparability of mortality data and continuity in statistical trends. Age-adjusted rates can only be compared to other age-adjusted rates that use the same population standard. Breaks in comparability of mortality statistics effective with deaths occurring in 2000 also result from the implementation of ICD-10. Any comparison of causes of mortality in Arizona from the 2000-2017 period and prior years needs to take into account the changes in statistical trends that can be attributed to changes in the classification system alone.
This report includes tables where small cells and related totals are suppressed. Using suppression rules similar to those used by the National Center for Health Statistics (NCHS), this report now attempts to maintain the anonymity of the individuals whose vital records are summarized herein. In cell suppression, the first task is primary suppression, or removing non-zero counts in the body of a table that fall below a certain number. Primary cells that were less than six but greater than zero were suppressed and identified with an asterisk (*). Next, secondary suppression is used to obfuscate the totals or sums with components, or addends that fall below the threshold for primary suppression. These totals are typically reported in the margins of table rows and columns. Column or row totals that contained a non-zero addend less than 6 were rounded to the nearest tens-unit and identified with a dagger (†). In certain cases where these rules would have dictated the rounding of a row or column total but the value of the information contained in the total was identified as important or attainable from other sources, these rules were relaxed and the original value was reported.

The number of deaths from Alzheimer’s disease in Arizona in 2017 made Alzheimer’s disease the 5th leading cause of deaths for all ages. (Figure 1). More Arizonans died in 2017 from Alzheimer’s disease than they did from other chronic diseases, such as diabetes, chronic liver disease and cirrhosis, or nephritis (kidney disease).

The trend in mortality from Alzheimer’s disease (Figure 2) reflects a number of factors such as 1) changes in attitudes ofphysicians and the public aboutattributing Alzheimer’s disease as acause of death, 2) availability ofimproved diagnostic procedures, 3)increase in the number of deathsattributed to Alzheimer’s diseasedue to changes in coding andclassification of causes of mortality,4) decreases in several otherleading causes of death (homicide,diabetes, suicide, atherosclerosis).

The age-adjusted mortality rate for Alzheimer’s disease increased by 61.5 percent from 21.8/100,000 in 2000 to 35.2/100,000 in 2017 (Figure 3). Among males, the age-adjusted mortality rate for Alzheimer’s disease increased by 66.9 percent during that time.
In 2000, the risk that a female will die from Alzheimer’s was 38.3 percent greater than the male mortality risk. In 2017, however, the female-to-male mortality ratio has diminished to 35.3 percent.

The 2017 death rate for Alzheimer’s disease among White non-Hispanic females was the highest among race/gender groups in Arizona (Figure 4). Black and White non-Hispanics had the highest death rates for Alzheimer’s disease among males.
White non-Hispanic females accounted for 1,667 deaths from Alzheimer’s disease (or 84 percent of all Alzheimer’s deaths among Arizona females), while White non-Hispanic males accounted for 937 deaths (or 87.1 percent of all Alzheimer’s deaths).

Risk of death from Alzheimer’s disease increases sharply with age (Figure 5). In 2000, nine out of every ten deaths from Alzheimer’s disease were to persons aged 75 years and older. Similarly in 2017, the risk of death from Alzheimer’s was greater for Arizonans aged 75 years or older.

The number of Alzheimer’s deaths has increased from 2007 to 2017, in part due to a growing population of older adults. Mortality rate from Alzheimer’s disease follows the same trajectory, marked by a 5.2 percent increase from 2007 to 2017 (Figure 6).

The death rate for Alzheimer’s disease, the third leading cause of elderly female mortality in Arizona in 2017, almost doubled from 191.6/100,000 in 2000 to 306.0/100,000 in 2017 (Figure 7). For elderly males, the mortality rate for Alzheimer’s disease, their fourth leading cause of death, increased by 79.4 percent from 108.0/100,000 in 2000 to 193.7/100,000 in 2017. In 2000, the Alzheimer’s disease mortality risk of elderly females compared to males was 77.4 percent greater, while in 2017 the risk was 58.0 percent greater.

Mortality rates for Alzheimer’s disease among Arizonans 75-84 years old were higher among females than males from 2000 to 2017 (Figure 8). In 2000, the Alzheimer’s disease mortality risk of elderly females compared to males in this age group was 17.4 percent greater (168.8 deaths per 100,000 females vs. 143.8 deaths per 100,000 males). In 2017, the female-to-male difference was 22.6 percent, a ratio greater than that of 2010.

Among Arizonans 85 years and older, the mortality rates for Alzheimer’s disease among females exceeded the male rates in every year from 2007 to 2017 (Figure 9). In 2007, the Alzheimer’s disease mortality risk of elderly females compared to males in this age group was 60.6 percent greater, while in 2017 the risk was 42.5 percent greater.

In 2017, Alzheimer’s disease had the highest average age at death of 87.0 years, exceeding the average age at death for all causes by 14.4 years (Figure 10). Only 16.1 percent of deaths from Alzheimer’s disease occurred before the age of 78.8 years, i.e., before the expected years of life were reached.

White non-Hispanic elderly residents of Arizona disproportionately contributed to total mortality from Alzheimer’s disease. In 2017, White non-Hispanics accounted for 80.9 percent of all elderly 65 years and older, but 85.6 percent of all deaths from Alzheimer’s disease (Figure 11).
White non-Hispanic residents of the State may be at higher risk for Alzheimer’s disease because they live longer than the other race/ethnic groups. In 2017, White non-Hispanics were the only group with about 50 percent of all deaths occurring before the expected years of life were reached. The 2017 premature death ratio was 43.3 percent for White non-Hispanic females and 57 percent for White non-Hispanic males. In contrast, at least 70 percent of deaths among ethnic minorities occurred prematurely.

In 2017, the mortality rate for Alzheimer’s disease was 8.2 percent higher for urban (35.7/100,000) than rural (33.0/100,000) residents of the State (Figure 12). The mortality disadvantage of the urban compared to rural residents was particularly pronounced among females. The 2017 rate of the urban females was 11.3 percent greater than the rate of the rural females (40.3/100,000 vs. 36.2/100,000).

In 2017, mortality from Alzheimer’s disease among the fifteen Arizona counties ranged from a rate of 11.7 deaths per 100,000 persons in La Paz County to a rate of 46.6 deaths per 100,000 persons in Yavapai County (Figure 13). Maricopa and Yavapai counties, which accounted for approximately half of the total population of the State, made a substantially higher contribution to mortality from Alzheimer’s disease (2,015 out of 3,050 deaths, or 66.1 percent of all deaths from Alzheimer’s disease). These two counties accounted for 60.4 percent of all Arizonans 85 years and older in the State, the group at the highest risk of death from Alzheimer’s disease.
