Social Aid

Social aid services are among the most essential tasks in psychiatry hospitals. As a member of the healthcare team, a social worker plays a key role in the provision of care services for the patients. Some of these tasks may begin upon the admission of the patient and continue during all the stages of admission, even after the discharge of the patient from the hospital.

A summary of social aid services at the psychiatry hospital is as follows:

- Interview and guidance of patient companions upon primary admission

- Specialized social aid counseling with the rehabilitation patients

- Specialized social aid counseling with the families of patients, including foreigners, before admission

- Counseling with the families of pediatric patients, children with special needs and guidance regarding the use of education and training services for these cases

- Finding the family of unidentified patients and delivering the patient to them

- Home visit of patients in order to facilitate diagnosis through collecting cultural, economic, social and familial information, as well as offering financial and discount bonus services

- Introducing poor patients to charities for financial help

- Finding suitable residence for unidentified patients (without family) or those without guardians, follow-up of their paperwork and accompanying them to centers for chronic mental patients

- Collecting gifts, clothes and food for poor patient families and those with several psychiatric family members

- Providing clothes for discharged patients without family or absent families upon discharge

- Follow-up of residence for poor patients, as well as their legal and municipality affairs

-Home visit of discharged patients

- Introducing poor patients to charitable pharmacies

- Collaboration with holistic care centers in the city

- Coordination for in-service training courses for social workers in the health centers affiliated to Mashhad University of Medical Sciences

- Membership of one of the social workers at the Holistic Psychiatry Committee

- Training of social workers and psychologists in Ibn-e-Sina and Hejazi hospitals

Psychology

The Department of Psychology provides various services, including history taking, mental examination, consultation and setting psychiatric tests. The department is active during the morning shift, and psychological tests are among the main equipment of the department. Some of the main responsibilities of psychologists are history taking, mental examination, psychiatric consultation and testing.

Psychological tests include the following:

Intelligence test: To determine the intelligence quotient of the tested individual (e.g., Raven and Wechsler Intelligence Test)

Personality test: To evaluate the clinical patterns of personality and diagnose psychiatric disorders in patients aged above 16 years (e.g., MMPI and MCMI)

Depression test: To determine the depression level in the patient and obtain the accurate diagnosis of the disorder for beginning the proper treatment (Beck and Hamilton Depression Inventory)

Anxiety test: To determine the level of anxiety in the patient (Beck and Hamilton Anxiety Inventory)

Projective test: To assess the personality, motivations, needs, pressures, contradictions, failures, interests, relationships and fears of the patients (Rorschach and TAT tests in adults and CAT in children)

Speech Therapy

The Department of Speech Therapy is a paraclinical section in Ibn-e-Sina Hospital, which operates to diagnose, evaluate, and provide treatment and counseling for all the patients with disorders of the tongue and speech.

Admitted patients in the department include:

- Referred patients from psychiatry specialists and other specialists inside and outside the hospital

- Referred patients from the military service and forensics

- Patients with disorders of the tongue and speech who refer to the clinic independently

Among the other services of the speech therapy department are performing neuro-feedback services to evaluate patients with anxiety, hyperactivity, attention deficit disorder and obsessive compulsive disorder, which are offered to inpatients and outpatients.

Some of the main disorders that are diagnosed and treated at the department are:

- Dysfluency disorders (stuttering and steep bounce)

- Manufacturing defects (cleft palate, hearing impairment)

- Language disorders (delayed speech development, aphasia, learning disorders, autism)

- Voice disorders (vocal nodules, impairment in the frequency and tone of voice)

Occupational Therapy

In general, occupational therapy is a specialized field that acts as a bridge between the hospital and community. In the Department of Psychiatry, the patients are trained on the skills needed in life and different vocations. Services in this regard are offered to a wide age range of patients (children to the elderly), as well as patients with physical and psychiatric disorders, children with cerebral palsy, patients with mental retardation and the blind. The main objective of occupational therapy is to make patients independent.

The Department of Occupational Therapy was founded in 1984, beginning its scientific performance after the conversion of the hospital into a psychology education center.

Patients referring to this department are classified into two main categories, as follows:

- Patients who are referred daily from other wards, providing a performance report at the end of their activity

- Day patients who are introduced by the clinic's physician to encourage the participation of the hospital and patient families in the process of treatment

- Feeling worthy and increasing self-confidence

- Continuing treatment until complete recovery and even after discharge

- Distancing the patient from stress-causing environments

- Group activities by displaying films, drawing conclusions and discussions about the film

Returning to the Community (CRP)

Social skills training and preparation of patients for returning to the community are among the main tasks of the department for the rehabilitation of the patients. Moreover, the department endeavors to pave the way for the patients by using CRP collections, especially in the case of patients with schizophrenia, bipolar disorder and recurrent depression.

The program of returning to the community is implemented for patients with disabling and recurrent psychiatric disorders in order to achieve the following goals:

- Patients must pay attention to the symptoms and behaviors before discharge from the hospital to have an independent life

- Patients must be able to participate in their hospital discharge plans or transition to different real-life situations that require less supervision

- Patients should be able to communicate well with the providers of social services

- Patients must comprehend the benefits of their prescribed medication

- Possibility of recurrence should decline in the case of psychiatric patients

The CRP aims to familiarize the participants with the skills and information that help them during transfer from the mental health system, hospitals and controlled residence centers, thereby enabling them to achieve an independent social life. Furthermore, patients would be able to receive rehabilitative social-psychiatric services in the long run.

Currently, there are two niches (500 meters each) for the mentioned purposes of the patients, which were pleasantly designed and provided by respected benefactors in June 2016. Efforts of the benefactors are appreciated and honored by the officials of the department.

Sleep Clinic

The Sleep Clinic is equipped with the latest scientific devices and instruments for the diagnosis and treatment of various sleep disorders. Such examples are:

- A polysomnography device (Stellate-Harmoni) for different sleep tests (e.g., PSG and MSLT)

- CPAP Titration devices for the proper treatment of interruptions in breathing during sleep

- EEG device for the evaluation and diagnosis of seizure disorders, particularly during sleep

- Respiratory dysrhythmia, such as breath interruption during sleep and obstructive sleep apnea

In the research-diagnosis laboratory of the sleep clinic, which is the first laboratory for this field in the eastern part of Iran, the primary specialist visit is followed by patient admission, and the following tasks are performed within overnight hospitalization:

-Persistent assessment of sleep-specific electroencephalography

-Continuous evaluation of eye movements using electrooculogram

-Constant monitoring of ECG

-Constant monitoring of oximetry pulse to assess blood oxygen saturation through the pulse

-Performing continuous surface electromyography of three organs (chin and two feet)

- Continuous evaluation of snoring

-Constant evaluation of inspiratory and expiratory effort with oral nasal flow sensors, chest and abdomen

-Constant evaluation of body position changes during the study

-Constant video recording during sleep

 

Electroencephalography (EEG)

EEG is performed to evaluate patients with seizures. In addition, EEG helps physicians in determining the effectiveness of anti-epileptic drugs and withdrawal time of the medications. EEG is also recommended in cases such as dizziness and reduced consciousness, concussion and other issues that are likely to cause disorders in brain function.

EEG is an effectual modality in the diagnosis of some brain disorders that might deteriorate personal performance (e.g., Alzheimer's disease) or disrupt proper cerebral function. Moreover, this method could be applied for some renal and liver conditions that are associated with disordered cerebral function (e.g., encephalopathy). In some cases, EEG has been applied to confirm conditions such as brain death.

Department of Brain Stimulation Treatments

One of the most important treatment methods for neurological and psychiatric disorders involves the stimulation of the brain, which is widely practiced across the world. Some of these approaches are as follows:

Electric Shock Therapy

In Ibn-e-Sina Hospital, the department of electric shock therapy is active with the presence of psychiatry specialists, anesthesiologists and nursing personnel, admitting 25 patients daily. In addition to the hospitalized patients, these care services are also offered to the outpatients referred from other psychiatrists.

Neurofeedback and Biofeedback

These methods are used in the treatment of anxiety disorders, symptoms of hyperactivity and attention deficit syndrome, and tension and migraine headaches.

Repetitive Transcranial Magnetic Stimulation (RTMS)

RTMS is performed through the skull and is a non-invasive, safe approach for the stimulation of brain neurons. In this method, weak electrical currents, which rapidly change under the influence of magnetic fields, are generated in the brain to stimulate brain neurons. As a result, cerebral activity could be stimulated with the lowest amount of pain and facilitate the examination of the performance of the interactions within the brain. RTMS has been recognized as an effective therapeutic measure in the treatment of psychiatric disorders such as major depression and obsessive compulsive behavior (OCD).

Dentistry

The Department of Dentistry has two general dentists and one dentistry assistant who offer healthcare services to the admitted patients in this hospital, as well as the hospitalized patients in Dr. Hejazi Hospital during the morning shifts. Services include visits, scaling, repair, pulling the teeth and implants. It is noteworthy that the benefactor ladies donate an approximate amount of 600$ USD each month in order to pay for the expenses of the dentistry clinic in Dr. Hejazi Hospital, which contributes to the treatment and service of more than 70 patients.

Laboratory

Clinical laboratory is crucial to the healthcare system in the health monitoring, disease diagnosis and treatment control. To this end, the officials and managers of hospitals and care centers are dedicated to developing specialized services proportionate to the advancement of medical sciences and new community needs. On the other hand, scientific and professional standards have coordinated the performance of clinical laboratories with the needs of the hospital and patients.

The laboratory is 200 square meters, with different sections for hematology, biochemistry, microbiology, serology and hormonology.

-The hematology department is equipped with two automatic cell counter systems (Sysmexkx, t810, made in Japan), which is able to count and calculate 18 hematological parameters

-The biochemistry department is equipped with an auto-analyzer device, which is applied for biochemistry, serology, immunology, electrolyte and data storage purposes

-There are two automatic electrolyte analyzer devices plame photomerty (phf108 Hycel, made in France) and a GDV automatic film photometer to measure serum sodium, potassium, lithium and urine

-The serology and immunology departments are equipped with an automatic ELISA plate reader to perform examinations for viral, bacterial and protozoan infections

-The reception section is equipped with HIS to efficiently serve the physicians and patients

Radiology

The Department of Radiology has one plain radiography device without a scapular tube, which has been active for 20 years to meet the therapeutic needs of admitted patients and outpatients during hospitalization and after discharge from the hospital. The processing systems are analogue, and there is an automatic OPTIMAX processor that suffices the requirements of the department to prepare all the exposed films. In addition, a portable Siemens radiography device has been purchased and the necessary measures to establish are under way.

The majority of the referred patients to this department are admitted to the hospital, who receive radiography at the Radiology Ward after admission. The Radiology Department of this center is located at the central clinic of the hospital offering care services during morning hours, as well as other shifts (on-call). Number of personnel in the department are three individuals, including two experts and one technician.
