INSTRUCTIONS TO RESPONDENTS
SECTION 1 – GENERAL INFORMATION & REQUIREMENTS
1.1 Request for Statements of Qualifications: The Missionary Oblates of Mary Immaculate (“OMI”
or “Owner”) is soliciting qualifications statements from qualified architectural firms for design and
construction administration in connection with the following Project:
1.1.1 Renovation of the OMI Novitiate – Godfrey, IL to facilitate the operation and goals of the on-
going Novitiate Program. The Novitiate is situated on a 200-plus acre tract of land overlooking the
Mississippi River and surrounding landscape. The property was originally assembled by the Levis
Family as “La Vista Estate.” Dating back to the early 20th Century when the Levis Family assembled the
La Vista property for their estate, the site has been a natural and historic resource for the local
community. Since OMI’s involvement with the property beginning in 1950, OMI has continued the proud
tradition caring for La Vista – adding to the important history of the site the use of the property and
buildings by OMI as the Immaculate Heart of Mary Novitiate and other OMI programs and activities. The
tradition continues in the 21st Century under the stewardship and guidance OMI – the Novitiate, Oblate
Ecological Initiative, and the OEI Learning Center, La Vista.
The project is further described as follows:
Renovation of the historic Levis House and associated additions, including:
As envisioned and programmed, the total Levis House facility at the conclusion of the renovation will
have approximately 12,752 GSF, including a small new addition to accommodate an elevator of 1,500
GSF. Drawings of the original Levis House and subsequent additions are limited, but can be made
available to the selected Respondent.
The overall project will also include exterior building improvements / repairs and improvements to the
Mechanical, Electrical & Plumbing systems. As well, exterior site improvements, such as landscaping,
additional paved parking, repair to existing garden landscape features, retaining walls and walks,
driveway upgrades and main utility service upgrades are also part of the approved program.

The approved construction budget for the project is approximately $2.67 Million.
1.1.2 The Owner will provide a facility program for the Novitiate project to the architectural firm
selected by the Owner for services.
1.1.3 The delivery method for construction of this project will be Construction Manager at Risk (CMAR).
1.l.4 The Owner has hired Jasmine Engineering, Inc. as the Owner’s Project Representative &
Commissioning Agent for the OMI Novitiate Renovation.

SECTION 2 – SCOPE OF SERVICES
2.1 It is the Owner’s Objective to enter into a contract with the most qualified architect and required
engineering consultants to provide all phases of design, assistance with bidding and negotiation, contract
administration and project close out.
A licensed architect will be assigned by the selected firm to act though out the project as the project
manager and point-of-contact for the Architect / Engineer team.
2.2 During the Design and Construction Document Phases, the firm or firms will work with the
Owner’s representatives, Owner’s Project Manager, and Stakeholders to understand and maintain the
project objectives for renovation of the facility.
SECTION 3 – QUALIFICATIONS
3.1 Required Qualifications: Firms submitting statements of qualifications must be represented by a
professional Architect licensed to practice in the State of Illinois and able to provide a professional
Architect to attend meetings of any official nature concerning the Project, including but not limited to
Scope Meetings, Review Meetings and other meetings as may be required for the Project. Firms
submitting statements must have been in business for a minimum of 10 years.
3.2 Desired Qualifications:
3.2.1 Demonstrated firm experience in the design of high quality and cost-effective renovation and
addition projects involving existing historic or vintage facilities.
3.2.2 Strong team work ethic and cooperation both within the firm and outside of the firm with
contractors, consultants, and Owner representatives.
3.2.3 A proven firm and/or team capable of providing creating, functional, flexible and technologically
sound design solutions with an economy of means and within the project budget.
3.2.4 Proven firm and/or team experience with sustainable design and construction, such as completed
projects which incorporate energy conservation and energy efficient design solutions.
3.2.5 Proven firm and/or team experience with religious worship or retreat facilities.
3.2.6 Proven firm and/or team experience with higher education and campus residential / dormitory
facilities.
3.2.7 Outstanding experience and reputation, based upon information provided by the Respondent as well
as information obtained by references; past experience of the Owner with the firm; past experience on
similar projects for other clients; record for delivery of design documents within schedule, and other
similar factors reflecting the experience and reputation of the Architect and its firm.

3.2.8 Experienced and capable key personnel to be used on the project(s), including principals of the
firm and the proposed internal project manager.
3.2.9 Proven past experience on similar projects coordinated by an Owner’s Project Representative.
3.2.10 Proven past experience on similar projects using the Construction Manager at Risk delivery
method.
SECTION 4 – FORM OF QUALIFICATIONS STATEMENTS
4.1 Statements of Qualifications must be received at:

Statements of qualifications must be submitted in a sealed envelope or box
plainly marked: “OMI NOVITIATE – GODFREY RENOVATION, RFQ-001-
Architectural Services: Design and Construction Administration.
Qualifications statements received after the deadline will not be accepted.
4.2 To achieve a uniform review process and obtain the maximum degree of comparability, it is
required that Statements of Qualification be organized in the manner specified. Ten (10) sets of the
Qualifications Statement are required. The Qualification Statement must include one (1) original copy
labeled “ORIGINAL” and containing original signatures, preferable in blue ink. The remaining sets are to
be copies of the original and are to be labeled “COPY”. Statements of Qualifications must be submitted in
3-with spiral wire or plastic binding and tab dividers. Respondent must also complete Statement of
Qualifications response in an electronic PDF format on a USB drive.
4.3 Respondents are expected to examine this RFQ carefully, understand the terms and conditions for
providing the services listed herein and respond completely. FAILURE TO COMPLETE AND
PROVIDE ANY OF THE REQUESTED ITEMS MAY RESULT IN THE RESPONDENT’S
QUALIFICATIONS STATEMENT BEING DEEMED NON-RESPONSIVE AND THEREFORE
DISQUALIFIED FROM CONSIDERATION. The Qualifications Statement shall be submitted in the
order set forth below:
4.3.1 TAB 1: COVER LETTER
a. Submit a cover letter, prepared on the letterhead of the Respondent, which summarizes the
relevant characteristics that distinguish the Respondent in its ability to provide the services
requested by the Owner.
b. The cover letter should identify each proposed team member and his or her role, including
proposed consultants for engineering standardly provided by the Architect / Engineer Team.

c. The cover letter should identify a principal contact person authorized to commit the
Respondent to a contractual agreement. In addition to providing the name of a principal
contact person, the address, telephone number, facsimile number, and email address of said
individual should be provided.
4.3.2 TAB 2: EXECUTIVE SUMMARY
a. Describe your organization’s background and experience in design and administration of
renovation projects similar to the project proposed herein.
b. List resources of the firm that would be made available to the Owner that would distinguish
your firm as most qualified to perform the design, administration, and closeout of the
project(s).
c. Provide an overview of the services your firm proposes to provide the Owner which address
the Project specified.
d. Provide a statement that the Respondent is able to meet the insurance requirements and
provide Certificates of Insurance coverage as specified in the Insurance Requirements.
e. Provide a statement on the availability and commitment of the Prime Firm and its principal(s)
and assigned professionals to undertake the project.
f. Provide a brief history of the Prime Firm and each consultant proposed for the project.
g. Provide a graphic representation of the project team, identifying the Prime Firm and each
consultant proposed for the project.
h. Provide verification of a minimum of 10 years of experience.
4.3.3 TAB 3: INFORMATION REGARDING SIMILAR WORK
a. List a maximum of ten (10) projects completed in the last five (5) years for which you have
provided services that are most related to this project. List the projects in order of priority,
with the most relevant project listed first. Provide the following information for each project
listed:
Project name, location, contract delivery method, and description
Color images (photographic or machine reproductions)
Final Construction Cost, including Change Orders
Final project size in gross square feet
Type of construction (new, renovation, or expansion)
Actual start and finish dates for design
Actual Notice To Proceed and Substantial Completion dates for construction
Description of professional services Prime Firm provided for the project
Name of Project Manager (individual responsible to the Owner for the overall success of
the project)
Name of Project Architect (individual responsible for coordinating the day to day work)

Lighting, and Special Consultants)
Any Termination for Cause or No Cause by Owner/ Client
References (for each project listed above, identify the following):
i. The Owner’s name and representative who served as the day-to-day liaison during
the design and construction phases of the project, including telephone number
ii. Contractor’s name and representative who served as the day-to-day liaison during the
Preconstruction and/or construction phase of the project, including telephone number
iii. Length of business relationship with the Owner.
References shall be considered relevant based on specific project participation and
experience with the Respondent. The Owner may contact references during any part of
this process. The Owner reserves the right to contact any other references at any time
during the RFQ process.
4.3.4 TAB 4: SERVICES
a. Provide a description of your firm’s overall approach to facility design and construction
administration, and explain how this approach will benefit OMI and the project. Describe
your firm’s experience working with the Owner, the Owner’s Project Representative, and
other consultants.
b. Provide a description of your firm’s overall approach to project administration and explain
how this approach will benefit OMI and the project.
c. Describe your firm’s experience accomplishing a project with an Owner’s Project
Representative and Construction Manager at Risk and your approach to coordinating
administration duties.
4.3.5 TAB 5: PROJECT TEAM AND EXPERIENCE
a. Provide an organizational chart and resumes of not more than one (1) page per person on the
capabilities, experience, qualifications of the firm management and each team member who
would handle any of the requested services for the Owner. Please include information for
proposed engineering consultants. Resumes should include the team member’s name, title,
and education, and brief overview of professional experience.
b. Provide a description of your firm’s past experience with projects using the Construction
Manager at Risk delivery method.
c. Describe the project team’s experience with projects using a formal Commissioning process
led by a Commissioning Authority / Agent.
d. Describe the project team’s experience in sustainable design and improved indoor air quality.
4.3.6 TAB 6: PAST LITIGATION AND CLAIMS HISTORY
a. Provide history of past litigation and claims associated with past projects during the last ten (10)
years, including any Termination of Cause or No Cause by Owner / Client

4.3.7 FELONY CONVICTION NOTIFICATION
Complete, sign and submit the Felony Conviction Notification Form, RFQ Exhibit “A”. This
form is required and is considered as part of the response to this RFQ.
4.3.9 ACKNOWLEDGEMENT OF ADDENDA
Complete, sign and submit the Acknowledgement of Addendums, RFQ Exhibit “B”. This form is
required ONLY if an addendum is issued for this RFQ.
4.3.10 INSURANCE REQUIREMENTS The Selected Service Provider and their consultants will be required to show proof of, carry, and
maintain the following insurance at time contract is executed for an awarded project:
A. Commercial General Liability Insurance: written on an occurrence basis, in the amount of
$2,000,000 aggregate, $1,000,000 per occurrence, with an excess limit of $5,000,000, and
covering personal injury, bodily injury, death, and property damage. The Owner shall be
named as an additional insured on this policy.
B. Commercial Automobile Liability Insurance, including hired and non-owned vehicles, if
any, and uninsured motorists: written on an occurrence basis, in the amount of $1,000,000
each person, $1,000,000 each accident, covering personal injury, bodily injury, and property
damage. The Owner shall be named as an additional insured on this policy.
C. Workers’ Compensation Insurance: as required by the law of the place of the Project with
an employer’s liability coverage of at least $1,000,000.
D. Professional Liability Insurance: written on a claims made basis, with a combined single
limit of $2,000,000 each claim and $2,000,000 annual aggregate.
a) Coverage shall be continuous (by renewal or extended reporting period) for no less than 60
months following completion of the contract and acceptance of the work by OMI.
b) Coverage, including renewals, shall have the same retroactive date as the original policy
applicable to this Contract.
The Selected Service Provider shall provide OMI with Certificates of Insurance and required
endorsements prior to the execution of this Agreement evidencing that the stated coverages have
been obtained.
Selected Service Provider is responsible for all premiums and deductibles under all of the
insurance policies required by this section. When there is a cancellation, non-renewal or material
change in coverage which is not made pursuant to a request by OMI, Selected Service Provider
shall notify the Owner of such and shall give such notices not less than thirty (30) days prior to
the change, if Selected Service Provider knows of said change in advance, or ten (10) days’ notice
after the change, if Selected Service Provider did not know of the change in advance. Such notice
must be accompanied by a replacement Certificate of Insurance.
In addition to any other remedies, OMI may have upon Selected Service Provider’s failure to
provide and maintain any insurance or policy endorsements to the extent and within the time

herein required, OMI shall have the right to order Selected Service Provider to stop work
hereunder, and withhold any payment(s) which become due to Selected Service Provider
hereunder until Selected Service Provider demonstrates compliance with the requirements hereof.
Nothing herein shall be construed as limiting in any way the extent to which Selected Service
Provider may be held responsible for payments of damages to persons or property resulting from
Selected Service Provider ’s or its subcontractor’s performance of the work covered under this
Agreement. It is agreed that Selected Service Provider’s insurance shall be deemed primary and
non-contributory with respect to any insurance or self-insurance carried by OMI for liability of
Selected Service Provider arising out of operation under this Agreement. Selected Service
Provider agrees to require, by written contract, that all subcontractors providing services under
this Agreement obtain the same insurance coverages required of Selected Service Provider, and
provide a certificate of insurance and endorsement that names the Selected Service Provider and
the OMI as additional insured. The insurance that is required herein shall be written so that OMI
will be notified in writing in the event of cancellation, restrictive endorsement or non-renewal at
least thirty (30) days prior to such action. The selected service provider shall be responsible for
obtaining Certificates of Insurance from the sub consultants, and upon request, furnish copies to
OMI. If sub consultant is unable to provide appropriate overages, selected service provider may
replace sub consultant with the consent of the OMI
4.3.11 SIGNATURE PAGE
Complete, sign and submit Signature Page, RFQ Exhibit “C”. The Signature Page must be signed
by a person, or persons, authorized to bind the entity, or entities, submitting the Qualification
statement. Responses signed by a person other than an officer of the company or partner of the
firm shall be accompanied by evidence of authority.
4.4 No Qualification Statements, or modification to a Qualification Statement, shall be made orally or
by telephone, e-mail, or by facsimile transmission (“fax”).
4.5 Qualification Statements may be withdrawn by written request received by the Owner prior to the
time fixed for opening via hand delivery, FAX, email, or US Postal Service.
4.6 The Owner reserves the right to request supplemental information of any and all Respondents to
aid the Owner in the evaluation process.
4.7 Time is of the essence, and the award of the contract to the successful Respondent is expressly
conditioned upon (i) the Respondent’s execution and delivery of the contract and evidence of
insurance, within ten (10) calendar days after the successful Respondent is notified of the
acceptance of its post-selection price proposal, and (ii) the Respondent’s timely fulfillment of any
and all other preconditions expressly set forth in the Contract. Should the Respondent fail to
timely execute and deliver the contact, evidence of insurance, or fail to fulfill any other such
preconditions, the Owner may, at is option and discretion, rescind the award and thereafter
negotiate with and award the contract to the next ranked Respondent, or may reject all Statements
of Qualifications.
4.8 By submission of its Statement of Qualifications, the Respondent agrees to waive all rights to
claims against the Owner, or persons authorized by the Owner, or any person’s or committee’s
evaluation of the Respondent’s qualifications.

SECTION 5 – SELECTION CRITERIA
5.1 Evaluation will be based on the Respondent’s Qualifications Statement provided in response to
this RFQ. The Owner will evaluate the submitted Qualifications Statements and will evaluate
and rank each response submitted based on the following evaluation criteria:
5.1.1 Prior experience in connection with projects of a similar size and nature to the Projects noted
herein, commitment to quality, overall experience of the lead Architects assigned to this Project,
reputation of the firm and past performance, including past litigation and claims associated with
past projects.
5.1.2 Capability of the firm in the areas of support personnel, technical staff and the spectrum
of services to be offered to Owner.
5.1.3 The firm’s overall plan for meeting the Owner’s needs and requirements, management
ability and demonstrated ability to effectively serve its clients on prior projects/
5.1.4 Clarity and quality of Qualifications Statement and supporting data.
5.1.5 Resources and support services provided by the firm to the Owner.
SECTION 6- AWARD OF CONTRACT
6.1 The Owner reserves the right to reject any or all Qualifications Statements. There will be no
contractual obligations on the part of the Owner to any Respondent, nor will any Respondent have any
property interest of other right in the contract unless and until a reasonable price for the services is
negotiated and the contract is unconditionally executed and delivered by all parties, and all conditions to
be fulfilled by the Respondent have either been so fulfilled by the Respondent or waived in writing by the
Owner.
6.1.1 Within a reasonable time, a committee of Owner representatives to whom the task has been
delegated, will evaluate each Statement of Qualifications submitted in relation to the criteria set out in
above in this Request for Qualifications.
6.1.2 Based on the evaluation criteria, the Owner’s Committee will evaluate the Respondents based
solely on the qualifications of each Respondent.
6.1.3 The Owner’s Committee may elect to interview or receive presentations from one or more of the
Respondents prior to approval of the rankings, but will base its evaluation on the same criteria set forth
herein.
SECTION 7 – INDEPENDENT CONTRACTOR
7.1 Respondent agrees and understands that, if selected, it and all persons designated by it to provide
services in connection with the Contract shall be deemed to be independent contractor(s), responsible for
their respective acts or omissions, and that Owner shall in no way be responsible for Respondent’s
actions, and that none of the parties hereto will have authority to bind the others or to hold out to third-
parties, that it has such authority.

SECTION 8 – THE FORM OF CONTRACT
8.1 The form of Architect Agreement utilized for this Project shall be the Standard Form of
Agreement Between Owner and Architect (AIA Document B133-2014) as amended by Owner along with
Supplemental Conditions amended by the Owner.
8.2 The Architect will be required to provide administration of the Contract between the Owner and
the Contractor in AIA Document A201-2007, General Conditions of the Contract for Construction, as
amended by Owner.
8.3 A copy of the above referenced AIA Document B133-2014 and Supplemental documents are
attached here as Exhibit D.

The Owner requires that Respondents give advance notice to the Owner if the person or an owner or

A. It understands that (1) this RFQ is a solicitation for Qualifications and is not a contract or an
offer to contract; (2) the submission of Qualifications by Respondent in response to this RFQ will not
