at the Sawgrass Marriott. The evening will feature cocktails and

hors d’oeuvres, a seated dinner, silent and live auctions, and Fund A Cure Program.

The evening of celebration will also include music and dancing.

Michael A. Mayo, FACHE, currently serves as hospital president ofBaptist Medical Center Jacksonville where he is responsible for theoperational and strategic direction of the 676-bed flagship facility ofBaptist Health. Under his leadership, the tertiary and regional referralcenter was named to the U.S. News & World Report Best Hospitals in2012, 2013 and 2014; #1 Hospital in Jacksonville market in 2014 and #4in Florida. Mr. Mayo is board certified in Health Care Management anda fellow with the American College of Healthcare Executives. He servesin various community leadership positions, including chairman of theUNF Healthcare Administration Advisory Board and a member of theACHE Regent’s Council for North Florida. Now, Mr. Mayo is contributinghis time and talent to help JDRF in our efforts to improve lives andcure Type 1 Diabetes by serving as Honoree for the 2016 JDRF Gala,where it is our honor to present him with the Living and Giving Award.

What is JDRF?JDRF is the leading global organization funding type 1 diabetes (T1D) research. Our strengthlies in our exclusive focus and singular influence on the worldwide effort to end T1D. Foundedby parents determined to find a cure for their children with T1D in 1970, JDRF expandedthrough grassroots fundraising and advocacy efforts to become a powerhouse in the scientificcommunity with more than 100 U.S. locations and six international affiliates. To date, JDRF hasfunded nearly $2 billion in research and made significant progress in understanding andfighting the disease.

MISSION: Accelerating life-changing breakthroughs to cure, prevent and treat T1D and its complications.

• Research: As the leading global organization funding type 1 diabetes (T1D) research, JDRF isaccelerating life-changing breakthroughs in the area of cure, prevent and treatment for T1D.

• Advocacy: $300M secured in 2-year commitment in federal funding for the special diabetesprogram (SDP) in 2015.

• Community:We provide information to help those newly diagnosed understand and bettermanage the daily burden of T1D to stay as strong and healthy as possible. In 2015, 12,000Bags of Hope were distributed to newly diagnosed children and 3,000 care kits weredelivered to adults in communities where JDRF is present. Additionally, JDRF outreachprovides an opportunity for individuals living with T1D to connect with others in theircommunity for support when they need it most.

Type 1 diabetes (T1D) is an autoimmune disease in which a person’s

pancreas stops producing insulin, a hormone people need to get energy from food.

T1D strikes both children and adults, and its onset has nothing to do with diet or lifestyle.

There is currently nothing you can do to prevent it, and there is no cure.
