MUNICIPAL STRUCTURE INSPECTION FORM
INTRODUCTION
The attached Municipal Structure Inspection Forms show the information that is required for MunicipalBridge and Culvert inspections. Specific details on each data entry field are included in Appendix A ofthis document. The old Ontario Structure Inspection Manual (OSIM) forms have been updated tosimplify the process of using inspection information to estimate bridge rehabilitation needs and costs.The inspection process is quite similar to the old process except for changes in the way that inspectiondata is recorded. Material defects of various bridge components, as defined in the old OSIM, are stillvalid. The most significant changes are in the �Element Data� Table, which incorporates the new�severity and extent� approach described in the new manual. This approach requires that morequantitative data be collected and recorded on the Condition State of bridge elements.
A seminar for municipalities and their engineering consultants will be held on September 21, 2000to explain the new inspection process and to provide information on how to complete theinspection form. For information on the seminar, contact the Ontario Good Roads Association at905-795-2555.
The new forms include several tables that are to be completed:
i) The �Inventory Data� Table contains information on the bridge location, bridge geometry, andtraffic and roadway data.
ii) The �Historical Data� Table contains information on previous studies or repair/rehabilitationwork that has been done on the bridge.
iii) The �Field Inspection Information� Table contains information completed by the bridge inspectorfor each specific inspection.
iv) The �Additional Investigations Required� Table contains information on further studies that arerequired to assess the condition of the bridge as well as some general comments on the overallstructure.
v) The �Element Data� Table contains quantity information for each component of the structure aswell as the inspection information in accordance with the �severity and extent� approach of thenew Ontario Structure Inspection Manual (OSIM).
vi) The �Repair and Rehabilitation Required� Table contains a summary of the requiredrepair/rehabilitation work for the structure.
vii) The �Associated Work� Table contains information on additional work that is required as a resultof the rehabilitation that will be done to the structure.
viii) The �Justification� Table contains a summary of the rehabilitation options considered and thereasons why the specific rehabilitation option was chosen.

Additional Information
In addition to the written information requested, photographs shall be submitted of the following:
- One photograph shall be taken which clearly shows the deck cross-section and features such asnumber of traffic lanes, curbs and sidewalks, medians and railing system. Note: This photograph isnormally taken from the approach roadway looking along the length of the bridge.
- One photograph shall be taken of the elevation of the structure and which clearly shows the numberof spans and superstructure type.
- One photograph shall be taken of the underside (soffit) which shows the type and number of mainsuperstructure element(s).
- Individual photographs shall be taken of all areas in a poor condition state (areas with severe defectsand deterioration). These photographs shall be taken at sufficiently close range such that the type,location and extent of the defects and deterioration are clearly visible and apparent. Note: Wherethere are no areas of severe deterioration in an element, then a photograph shall be taken showing atypical area which represents the worst condition state in that element.

INTRODUCTIONDefinitions of the various data entry fields are listed below. Reference is made to the appropriatetable if a standard list of choices exist for that data field. For those tables, a Code Number can beentered into the appropriate field on the inspection form, rather than the entire description.
For the Element Data required on Page 3 of the inspection form, an abbreviated explanation ofthe various data entry fields is listed below. The complete explanation, including the definition ofthe various condition states and sample quantity calculations, can be found in the latest revision toOntario Structure Inspection Manual (OSIM).
DEFINITIONS OF DATA ENTRY FIELDS

INVENTORY DATA
MTO Site Number: The MTO Site Number for the bridge. The county number (derived from thelist of old counties, Table 3) should be entered along with a 4-digit sitenumber (e.g. 21-0031). Culverts should be denoted with the suffix �C� (e.g.21-0031C).
Structure Name: The common name for the bridgeMain Hwy/Road #: The Highway or Regional Road number. Indicate whether the road is �on� or
�under� the bridge.Crossing Type: Indicate the type of crossing. Choose all that apply from either a navigable
waterway, non-navigable waterway, railroad, road, pedestrian or othercrossing.
Road Name: The name of the road on which the bridge is located.Structure Location: Text to describe the location of the bridge including distance from major
intersections or physical landmarks and/or lot and concession numbers.Latitude/Longitude: Enter these values if known. Values can be obtained using a hand-held
Global Positioning System (GPS) device. The reference point should be theNortheast corner of the bridge.
Owner(s): The owner or owners of the bridge. (Current municipality name � County,town, etc)
MTO Region: The MTO Region in which the bridge is located. (Table 1)MTO District: The current MTO District in which the bridge is located. (Table 2)Old County: The original County or Regional Municipality in which the bridge is located.
(Table 3)Geographic Twp: The original geographic surveyed township in which the bridge is located.

Heritage Designation: The heritage designation for the bridge. Either one of the following: Notdesignated, Considered but not Approved, Listed but not Designated,Designated but not listed, or Designated and Listed.
Road Class: The class of roadway (either Freeway, Arterial, Collector or Local road).Posted Speed: The legal posted speed limit across the bridge.No. of Lanes on Bridge: The number of lanes on the bridge.AADT: The Average Annual Daily Traffic on the bridge (Total for both directions)% Trucks: The percentage of truck traffic in the AADT.Special Routes: Indicate whether the bridge is on a transit, truck, school bus, or bicycle route.
Each box that applies should be marked.Detour Length: The detour length around the bridge.Fill on Structure: The amount of fill above the culvert or bridge structure.Skew Angle: The skew angle of the bridge.Direction of Structure: The direction of the structure (either North to South or East to West).No. of Spans: The number of spans in the bridge.
HISTORICAL DATA
Year Built: The year in which the bridge was built.Current Load Limit: The current legally posted load limit on the bridge.Load Limit By-Law #: The by-law number enacted to permit the load limit posting.By-Law Expiry Date: The date on which the by-law expires on the bridge.Min. Vert. Clearance: The minimum clearance on the bridge. Either the clearance to the overhead
members of a through-truss, or the clearance to the road under the bridge.Last Biennial Insp: The date of the last biennial inspection. A biennial inspection is a detailed
inspection of all bridge components, performed under the direction of aprofessional engineer, at a maximum of 2-year intervals.
Last BridgeMaster Insp: The date of the last complete and detailed inspection performed using theBridgeMaster, Scissors lift, or Cherry picker.
Last Evaluation: The date of the last structural evaluation.Last Underwater Insp: The date of the last underwater inspection.Last Condition Survey: The date of the last condition survey.Rehab. History: The relevant rehabilitation history of the bridge including repaired items and
dates of repair/rehabilitation.

The data for each element on the bridge shall be shown on one table. The tables can be duplicated for asmany elements as exist for each bridge.

Condition Data:
This table is used to record the �severity and extent� of the material defects of the various bridgecomponents. The condition of bridge elements is defined to be in any one or more of the four Condition

States (Excellent, Good, Fair and Poor). At any given time, areas within a bridge element may be indifferent Condition States, or the whole of the element may be in the same Condition State. For eachbridge element, the inspector assesses and records the amount (area, length or unit as appropriate) of theelement in each of the four Condition States. This assessment is based predominately on visualobservations, however, some non-destructive testing, such as hammer tapping of concrete fordelamination, may be required to determine or verify areas in poor condition.
• Inspection quantity units are Sq. m., m., Each, All - Circle unit used (Units defined in Table 6)• For Elements with units of Sq. m, or m.
• Enter the estimated quantity in the 3 Condition States (Good, Fair, Poor). See the appropriateCondition State Table in the latest revision to OSIM.
• Quantity in Excellent = Total - Quantities in other states• Percent should not be used unless more than 50% of the element is in Poor Condition state

Suspected Performance Deficiencies
Suspected Performance Deficiencies should be recorded if a follow-up action is required (eg. strengthevaluation, specialized inspection, etc). Select deficiencies from pick list on page 2 of the form andenter the appropriate Code number. If no suspected performance deficiencies exist, enter �none� or �0�.
Maintenance Needs
Select the Maintenance Needs from the pick list on page 2 of the form and enter the appropriate Codenumber.
Comments• Comments should provide information on the poor condition state (eg. How the quantity was estimated,
location of defect, etc)• Comments can also be used to provide general information on the element
Recommended WorkPriority:
1 to 5 years: A rehabilitation that can be done in a normal time frame. Therehabilitation is to be designed and preferably completed in one tofive years.
Within 1 year:A rehabilitation that is required with some degree of urgency but canstill be can completed under a reasonable time frame. The design phaseshould be started within a few months, with construction beingcompleted by the end of the next construction year
Urgent: A rehabilitation that is required immediately. The design is to bedone immediately and construction shall also be in the currentconstruction year. (eg. fatigue crack in steel girder)
None: No work expected in the next 5 years

REPAIR AND REHABILITATION REQUIRED
The inspector has already recorded most of this information when each element was inspected. Thisarea summarizes all the recommended repair/rehabilitation work and also provides a cost estimate forthe work. The table can be expanded if space is required for more element repair/rehabilitation work.
Element: List the element that is to be rehabilitated.Repair and Rehabilitation Required: The recommended rehabilitation for that element.Priority : See explanation under �Recommended Work� on Page 3.Estimated Construction Cost: The estimated construction cost for the rehabilitation of the
element.Total Cost: Indicate the total estimated cost of all rehabilitation work.
ASSOCIATED WORK
Approaches: The work required immediately adjacent to the bridge structure.Detours: Any construction costs associated with required detours.Traffic Control: Cost required for temporary barriers, line marking, or protection for the purpose
of Traffic control.Utilities: Any costs associated with relocating utilities.Other: Any other costs required to complete the bridge work.Contingencies: Contingency for cost overruns.Comments: Provide details about the type of associated work that is required.Estimated Cost: The estimated construction cost for each type of associated work.Total Cost: Indicate the total estimated cost of all associated work.
JUSTIFICATION
Provide a description of the recommended rehabilitation option chosen and options that were consideredand the reason for their dismissal. The purpose of this section is to justify the expense and to justify thatthe most economical rehabilitation was chosen. If a total bridge replacement is selected, the reasonsshould be described in this area.
