PSYCHIATRY

WITH RESPONSIBILITY FOR PROVIDING AN ADDICTION PSYCHIATRY SERVICE TO NORTHEAST SECTOR,

GLASGOW HSCP

Job Description

General introduction

Greater Glasgow and Clyde (GG&C) Addiction Services are recruiting an enthusiastic and committed specialist to a post of Consultant Addiction Psychiatrist to work in Northeast Sector, Glasgow HSCP.

The addiction services in GG&C are organised within 6 HSCPs. The Glasgow HSCP is further split into 3 sectors: NorthEast, NorthWest and South. Community services in all areas include integrated community addictions teams/drug and alcohol teams with contracted services providing community rehabilitation and GP shared care scheme. The community services share access to 2 dedicated NHS addiction in-patient units- Kershaw Unit (Gartnavel Royal Hospital) and Eriskay House (Stobhill Hospital). In addition, day services for all areas are provided from these units, with the exception of South Clyde (Inverclyde and Renfrewshire) where more local day services exist. A network of local community mental health teams and an ARBD service are examples of other relevant services organised on a similar geographical basis.

The medical workforce within GG&C addiction services is currently over 50 doctors from diverse backgrounds working as medical officers, senior medical officers, consultant addiction psychiatrists and SAS doctors in addiction psychiatry. In Glasgow, the addiction psychiatrists at present work from secondary service settings reaching into community teams, with geographical areas of responsibility. This post holder will be based in Parkhead Hospital but will have a community role working with 2 community addiction teams (CATs) in Northeast Glasgow.

The Drug and Alcohol Services across Greater Glasgow and Clyde are concluding Clinical Services Reviews. Consideration is being given to possible reconfiguration of in-patient and day services, and to a more community orientated role for consultant addiction psychiatrists. Any proposed changes to the northeast consultant role will be discussed with the post-holder as part of the job planning process.

Clinical

This postholder will provide a tier 4 psychiatric service to patients with both alcohol and drug problems for all of the northeast area . This will involve-

- Inpatient assessment and treatmentBeds are available in the 15 bedded Eriskay House, Stobhill Hospital. It is likely that consultant addiction psychiatrists across the partnership will need to refashion their job plans to reflect the probable consolidation of all inpatient care and partial hospitalisation to dedicated consultants. This might result in a separation of hospital and community function with some addiction consultants taking an inpatient responsibility and others a community responsibility

- Partial hospitalisation / day hospital assessment and treatmentFollowing referral from the CATs, patients attend for this at Eriskay House, Stobhill Hospital. A partnership wide review of partial hospitalisation and day hospital addiction services is currently being undertaken and any future services will be shaped by the review and its results. Again collaboration with the north partial hospitalisation staff would be essential – agreeing a unified approach to unit use and function.

- Community outpatient clinics and home visitsThis is a community based job and the post holder will be expected to carry a consultant caseload, provide face to face routine and emergency psychiatric outpatient assessment and treatment for referred patients with complex substance misuse problems often in the context of co-morbid physical and mental illness. Referrals will generally come from the community addiction teams and other consultants.

- Liaison and consultation with the Northeast sectorThe postholder will regularly provide liaison and consultation to northeast addictions services. There may also be occasional liaison input to general adult inpatient psychiatric services for east patients and to relevant acute services.

Community and residential rehabilitation placement is available through a variety of contracted providers. The consultant would be expected to take a role in the decisions about placement of patients in residential rehabilitation facilities in coordination with the local head of service

- Mental health Legislation usePost-holder will be put forward for section 22 approval by NHS Greater Glasgow and Clyde under new mental health act to allow participation on the senior on-call rota for north and east Glasgow mental health sector and in order to be able to deal with all cases requiring mental health provisions arising within the Addiction Services. In order to do this post holders must have obtained day 1 and 2 Mental Health (Care and Treatment)(Scotland) Act 2003 approved medical practitioner certification from the Royal College of Psychiatrists and submitted these forms to the medical staffing department of HR. Use of the Adults with Incapacity Act and Adult Support and protection Act would also be part of the job.

- On callOn call on the combined general adult psychiatric senior rota in Northeast Glasgow is coordinated by the local General Adult Psychiatry Clinical Director. The frequency is around 1 in 26 for which a level 1 on call availability supplement of 3% is currently payable.

- EmergenciesThe Consultant may be required to perform duties in occasional emergencies and unforeseen circumstances at the request of the Associate Medical Director (Addictions). It has been agreed that such duties will be exceptional and that this kind of arrangement would not be for prolonged periods of time or on a regular basis in keeping with contractual arrangements.

- Clinical cover of colleaguesThe 10 pa job plan would include prospective cover for a colleague’s leave. Any leave requested would require cover to be arranged in advance from another consultant. Cover for short term sudden absences e.g. sickness in a colleague consultant would also be included in the job plan.

Job plan

The exact nature of clinical commitments assumed by the post-holder will be a matter of agreement between the successful candidate and the Associate Medical Director for Addictions (or a Lead Clinician) in an annually renewed job plan.

The post is for a standard 10 programmed activities (pas) made up of 9 pas of direct clinical care and 1 pas of supporting professional activity. Applications for less pas would be considered.
