Today marks the second live session of the Million Hearts® Model Control Group Training Series.

There will be plenty of trainings to make sure you and your organization are prepared to successfully participate in the Million Hearts® Model.

If you missed the MH Model Onboarding event, please check your email for event follow-up materials.

Today’s session kicks off a series of training events designed to equip you with the knowledge and skills needed to use the Million Hearts® Model Data Registry. Our session today provides a high-level introduction into the tool. You will have the opportunity to learn how to use each of its features in the other training events listed here, occurring between now and mid-November.

Let’s start with defining the MHM Data Registry. The Registry is an interactive, web-based platform that will facilitate the exchange of data between you and CMS throughout the Model. The Registry is where you will submit demographic and clinical data for your eligible beneficiaries.

The data you enter into the MHM Data Registry facilitates both your organization’s payments and CMS evaluation of long-term model outcomes.

The MHM Data Registry will allow you to complete four key activities throughout the model:

First, you will enter clinical and demographic data into the Registry to submit eligible beneficiaries for enrollment in the Million Hearts® Model. When you submit data, CMS will confirm each beneficiary’s eligibility and align them to your practice so they will be tracked throughout the Model. To learn more about this process, please view the Understanding Beneficiary Enrollment and Alignment Video, which will be released next Tuesday, October 18th via email.

Second, you will continue to use the Registry to submit data for enrolled beneficiaries throughout the model. After receiving confirmation that your eligible beneficiaries have been enrolled in the model, you will submit data from annual follow-up visits as required by the model. This ongoing data entry will track your patients’ health data over time. You can learn more about how to submit beneficiary data for both enrollment and annual visits by completing the Web-Based Training titled “Using the MH Model Data Registry in Practice.” This is a

hands-on training module that will be released in early November.

We expect you will use the two main functions of the MHM Data Registry throughout the Million Hearts® Model. At any point during the model, you can submit your beneficiary’s demographic and clinical data for CMS validation to facilitate Enrollment.

You are also required to submit clinical data for each beneficiary annually during his or her “Enrollment Anniversary Window.” An “Anniversary Window” is a 4-month length of time that occurs 10-14 months following either the beneficiary’s Enrollment date or the most recent Enrollment Anniversary. Each year, the Registry will alert you that a beneficiary is ready for his or her anniversary visit; at that time you must submit that beneficiary’s clinical data to the Registry.

Completing this step each year for all of your enrolled beneficiaries will allow CMS to track health outcomes and provide payments to your organization. If you need a refresher on performance tracking and payment incentives, refer to your Onboarding Packet. The upcoming web-based training, Using the MH Model Data Registry in Practice, will provide more information about how to enter this data.

Now that we have learned about the importance of the MHM Data Registry and some of its functions, we will take a few minutes show you the key site features in a short virtual tour video. Please ensure that your volume is loud enough to hear the narration of the video.

Once you have access to the MHM Data Registry, you will log in from the CMS Secure Portal at portal.cms.gov. Click the button in the upper right hand corner to log in to the CMS Secure Portal. Then, accept the terms and conditions to continue. Enter your user name given to you when you were granted access to the site. Next, enter your password. Select the Multi-factor Authentication (or MFA) Device Type that you registered for when creating your account. Using the VIP Access program, copy your security code and paste it into the security code field. Click Log In. Once you are in the CMS Enterprise Portal, click the Innovation Center Dropdown menu and click the appropriate application console. Click “Launch MHDR App” to launch the MHM Data Registry.

When you login, you’ll arrive at the MHM Data Registry Data tab. The maroon toolbar along the top of your screen will help you navigate to the key areas of the site. The Data Tab is where you can upload, input, edit, and view your beneficiaries’ data at any point during the Model. You can submit beneficiary clinical and demographic data in three different ways: Enter data manually by typing data values directly into the registry for a single patient one at a time. Upload the data for multiple patients at a time using a spreadsheet, such as a CSV or Excel file. Finally, you can submit data by uploading a file exported from your EHR. Upload the data in bulk from a standard E.H.R. CCDA format.

Once you submit a beneficiary’s data in the Model, that data will appear in the list of beneficiaries at

the bottom of the Data Tab. You can see each beneficiary’s current enrollment status and the date their information was last updated. Once your patient is enrolled, you will be able to see their anniversary date. You can also filter your data page by provider or status and search for specific patients or patient segments. Use the Actions dropdown menu to edit a beneficiary’s information or delete a beneficiary.

Practice Administrators can access the Administration tab to add, edit, or delete providers from their organization or update basic organizational information. However, this page is not accessible to those in the Practitioner role. Anyone can access the FAQ tab to see answers to common questions about navigating and using the MHM Data Registry. And, if you have more questions, go to the Support tab to see contact information for others who can help. You can send a message to the Help Desk through the Support tab if you are experiencing any issues with the Registry.

Successfully navigating the Million Hearts Model Data Registry will allow you to quickly and accurately complete many of the required tasks of Million Hearts Model participants. For more information, contact the Help Desk or attend additional Million Hearts Model trainings on using the MHM Data Registry.

Two common questions that the Million Hearts® Model team hears from organizations participating in the model are: How should my organization integrate the Registry into our workflow? And who in my organization should be entering and monitoring data on an ongoing basis?For the purposes of the Million Hearts® Model, your organization is free to answer those questions yourselves based on what works best for you and your workflow. It is possible that members of your organization such as administrative staff and clinical staff and other care team members may share some of the responsibilities associated with enrolling patients, submitting data periodically, and managing beneficiary and organization data. We also imagine that some organizations may have a single staff member complete all necessary Registry activities. Let’s look at some examples of how each of the roles highlighted here may use the Registry.

Administrative staff can use the Data tab to enter beneficiary data manually, one by one, or upload multiple beneficiaries in bulk. They will simply have to select the NPI of the provider on whose behalf they are entering data. Beneficiary data can also be entered directly by clinical staff as it is collected during patient visits. Note that all beneficiary information will appear in the table at the bottom of the data tab immediately after it is manually entered or uploaded.

Administrators can use the Administration tab to manage their organizations’ list of providers and basic organizational data.

This page is only available to individuals who register for the Practice Administrator user role when requesting access to the Registry. Individuals who request the Practitioner user role will not have access to the Administration tab. You will receive more guidance on how to select a user role in future trainings and communications.

Providers can use the Data Page to view beneficiary data and stay up to date on the enrollment of beneficiaries assigned to them. The table at the bottom of the Data Page includes each beneficiary’s Anniversary Date, the date on which their data was last updated, and their enrollment status.

Now that we’ve discussed the main uses of the Registry and why it will be important to you while participating in the Million Hearts® Model, we would like to discuss the process for gaining access. Although you are not able to request access to the Registry yet, it will be very important for you and your team to understand how to complete this process when it is time to request access in December.

You may be wondering who in your organization should set up a Registry account. At a minimum, all members of your organization who will be entering beneficiary data into the Registry must set up an account. We also highly other clinical and non-clinical staff to register. Securing access to the Registry will allow you to access any of the features we’ve discussed today and help promote Million Hearts® Model success. The more individuals in your organization that have an account, the more you can integrate using the tool into daily workflows.

When should you request access to the Registry? You can begin to register for Registry access starting December 1st. Practice POCs must set up an account first, by December 10th. Once Practice POCs are granted access to the platform, they will be responsible for approving all access requests for other individuals in their organization.

All staff who are not Practice POCs are encouraged to register for an account by January 1st 2017. Feel free to use the month of December to log in and explore the site. Starting January 1st, your practice is required to begin submitting data to enroll

your patients and fulfill Million Hearts® Model requirements.

The process for gaining access to Registry involves three steps. Each of these steps allows for CMS to maintain the security of all data stored in the Registry and prevent unauthorized parties to access the platform. First, you must register for Enterprise Identify Management account credentials via the CMS Secure Portal. This involves providing basic information about you and your organization and allows CMS to ensure people entering the portal are who they say they are.Next, you will request access to the Innovation Center Portal and register your computer or device for multi-factor authentication. This allows CMS to make sure when you access the registry, the data and interface is secure and behind certain firewalls.Once you have access to the Innovation Center Portal, you will go to the portal to request access to the Registry. When requesting access to the Registry, you will be asked to choose a user role: either a Practice Administrator or Practitioner. We will provide more information on the distinctions between these roles in the upcoming web-based training, Using the Registry in Practice, and in written instructions

distributed via email. After completing these three steps, you should receive confirmation that your application was approved via email. You may then log into the MHM Data Registry and familiarize yourself with the site. If you have any issues throughout this process, you are encouraged to contact the Help Desk. We will provide more information and training opportunities to guide you through these three steps in the coming weeks via email and will post all reference materials to the MHM Connect Library following email distribution. As previously mentioned, you are not expected to complete any of these steps until December.
