Our Post-Discharge Services aim to provide the following supports:
• Security reassurance checks• Friendly visiting• Connecting to community resources for patients discharged from Etobicoke General Hospital
Visits are arranged for home- bound clients at their homes, hospital or nursing homes for a three-month period after the referral.
If you are interested in receiving friendly visits, you can arrange that during a reassurance call and schedule a preferred time between 9:00 a.m. and 9:00 p.m.
ABOUT THE PROJECTIf you are eligible for this program and are interested in participating, please contact Etobicoke General staff prior to discharge for a referral.
With your consent, your contact information will be shared with project staff within 24 hours of discharge.
REFERRAL FRIENDLY VISITING
Referrals to community programs and services will be based on your identified needs and situation.
You will be connected to communityresources including: family physicianscommunity health centres, dayprograms, housing supports, ethnicprograms, financial assistance and more.
CONNECTING TOCOMMUNITY RESOURCESYou will receive your first phone
call within 48 hours of referral. The purpose of the call is for program staff to:• Check on your health and security• Assess your needs• Discuss services and• Create a tailored service plan
Regular follow-up calls will be conducted subsequently.
SECURITY REASSURANCE CHECKSPatients of the Etobicoke General who are either senior or immigrantand:
• are lacking support• are isolated• have complex needs• have language or cultural barriers• are uninsured• need assistance with accessing community resources
