innovative and sustainable activities and services which will help to reduce health inequalities in East Merton.

1. Be Active: aims to increase physical activity levels

through increasing access to low cost and sustainable community based exercise and activity sessions.

2. Stay Well:aims to support people with long term

conditions to maintain their health and independence, through targeted community based activity.

3. Community:aims to build community groups capacity to

support residents to lead healthier lifestyles and promote physical and mental wellbeing.

4. Business:aims to support partnership working to

engage the business community in promoting the health and wellbeing of residents, and support small health related social enterprises.

How Much & How Long?Year one of a three year programmeFunding is available for projects lasting up to one

year. Grants of up to £10,000 are available, but we expect

most grants to be in the region of £5,000.£70k available in year one

Proposals must increase the health and well- being of families and communities in the following wards :

Abbey, Colliers Wood, Cricket Green, Figge's Marsh, Graveney, Lavender Fields, Longthornton, Pollards Hill, Ravensbury and St Helier.

Decision MakingAll applications will be assessed and

decisions made by a grants panel with representatives from Merton Council, NHS, Clinical Commissioning and the voluntary sector
