QUALITY PATIENT CARE drives everything we do

Because excellent patient care is our priority, we registered as a participant in the Home Health Quality Improvement National Campaign. This is a grassroots movement that unites home health stakeholders and multiple healthcare settings under the shared vision of reducing avoidable hospitalizations and improving medication management. We are focused on improving communication and care from one healthcare setting to another. This brings peace of mind for patients and their families.

Here are some of our other activities that benefit patients.

Assessing patient needsWhen we ask standard questions to assess our Community Home Health patient’s progress, we go beyond the initial answers. For example, our patient may be able to walk – but is it safe? We listen and focus on what is important for each individual.

Patient satisfactionPatients and families complete surveys administered by an outside source. The information that we receive from these surveys guides our ongoing efforts to help patients experience faster healing and greater comfort.

Skills fairsWe hold skills fairs where employees increase their knowledge and understanding of all aspects of patient care. At a recent skills fair, employees learned the latest techniques for preventing infections and practiced methods for increased safety with patients who need assistance while walking.

LOCAL FOUNDATION helps local patients

Thanks to community support, our patients benefit from many types of treatment for healing and pain control. The Community Foundation for Southwest Washington awarded our agency two grants focused on an area of great need—medical equipment.

From the Cowlitz County Community Fund, we were awarded $5,000 for oxygen concentrators for hospice patients in Cowlitz County. From the Community Giving Fund, we were awarded $10,000 for an Ultrasound/transcutaneous electrical nerve stimulation unit, Vital Stim system, hospital beds and special mattresses.

This equipment makes it possible for us to improve the quality of life for patients in their homes. We are grateful to The Community Foundation for Southwest Washington and all of the individuals who contribute.

Hospital beds bring WELCOME RELIEF to patients

Patients who have been unable to sleep in the past have expressed great comfort in a hospital bed with the support of a mattress that allowed them to find a restful sleep position. They can also be positioned to eat, read and play games.

For some patients, a hospital bed allowed them to remain at home, where they wanted to live.

In addition to other important equipment, The Community Giving Fund grant allowed us to purchase six hospital beds and mattresses.

CARE for future generations

Dr. and Mrs. Larry McRae cherish the memories of their loved ones who received our care. Carol remembers when her mother settled into the Hospice Care Center in Longview and asked, “Is this heaven?” It was private, comfortable and the family received all the support that they needed.

“There needs to be a place for patients to get well. There also needs to be a place for people to learn how to die, where you can sit down and talk about what to do.” – Carol McRae

The McRaes feel it is very important that future generations have skilled, compassionate care. It was an easy decision for them to include Community Home Health & Hospice in their will. We were honored when they became founding members of the Community Legacy Society.

High steppin’ since 1997For 15 years, High Step Dance Academy’s award-winning dancers have been dancing from their hearts and raising money for Community Home Health & Hospice. They had a record-breaking gala event this year. Their gifts support our children’s bereavement program, which is available to all grieving children, not just hospice families.
