History

• Basic info: collected during appointment ie weight, height, BMI, blood pressure & pulse

Examination

• Good practice - chlamydia test used for end of year audit & useful for sexual health LES

STD’s

• Contains sections where data is collected for Payment. There are 8 possible payments

• Audit: records reasons for fitting, complications, failed fittings for IUD and implant problems
