SCOPE OF WORK and TECHNICAL REQUIREMENTS Laboratory Safety Equipment and Calibration Purpose: According to the regulation issued by Ministry of Health Government of Vietnam, all lab equipment must be annually calibrated, especially, the laboratories that want to obtain biosafety certificate by authorities’ agencies. These equipment (e.g. Biosafety Cabinet -BSC, autoclave, incubator pipette) must be calibrated regularly. National Institute of Hygiene and Epidemiology, Pasteur Institute in Nha Trang, Pasteur Institute in Ho Chi Minh City have received training on BSC certification but require additional equipment for performing calibration services. Laboratory Safety Equipment and Calibration

Purpose: According to the regulation issued by Ministry of Health – Government of Vietnam, all lab

equipment must be annually calibrated, especially, the laboratories that want to obtain biosafety

certificate by authorities’ agencies. These equipment (e.g. Biosafety Cabinet -BSC, autoclave,

incubator pipette) must be calibrated regularly. National Institute of Hygiene and Epidemiology,

Pasteur Institute in Nha Trang, Pasteur Institute in Ho Chi Minh City have received training on BSC

- MadgeTech’s IFC200 interface cable communicates between stand alone

data loggers and the MadgeTech software. Through this interface, loggers

can be started, stopped or downloaded.

- The IFC200 has been redesigned, and is now “plug and play”. It can be

II. Terms and conditions:

1. Price:

The price is inclusive of all fees and labor related to shipping, handling, insurance, customs

clearance, and distribution to the sites outlined below

2. Capability of vendor/supplies

(a) Having at least 2 staff trained on biosafety and biosecurity

(b) Obtaining ISO 9001-2008

(c) Having an Authorized Letter of Manufacture for goods.

3. Delivery Terms

(a) Delivery Date is to be understood as the time the Goods as listed above have to be available at

the delivery locations listed in section 6.

(b) The Supplier will make 1 (one) distribution within 4-6 weeks of Purchase Order signing to

CDC’s recipient sites (“Recipients”) listed in section 6. Both parties will confirm in writing the

quantity of goods, time of delivery, and Recipient’s name and address before the delivery.

4. Quality Assurance

(a) Supplier commits that all Goods supplied under this Purchase Order shall be in compliance with

all appropriate national regulatory authorities, requirements of the Government of Vietnam and

with relevant certification standards for the production of Goods.

(b) Supplier guarantees that the Goods are 100% new and are in accordance with the

manufacturer’s highest standards.

(c) Supplier will make reasonable accommodations for Purchaser to inspect Goods before delivery.

Purchaser also has the right to inspect Goods after delivery. Upon inspection of the Goods,

USCDC shall promptly notify the Supplier of any damages to the Goods and Supplier shall

work with USCDC to remedy these damages.

(d) Purchaser may conduct quality surveys on randomly selected items. If surveys detect testing

result discrepancies not in line with the manufacturer’s specifications, Supplier agrees to

replace the damaged or defective Goods.

5. Services

(a) Supplier agrees to provide a one-year warranty for all equipment at each site.

(b) Supplier shall conduct installation and commissioning of the Goods and the Initial Training in

for each site. The costs for installation, commissioning and the initial training are included in

the price of goods.

(c) Supplier will undertake to supply spare part, chemical for running instrument in 5 years.
