1.0 Log in

1. You must be an approved clinic panel before login into the portal.

2. To login to the portal, please point your browser to http://sehat.perkeso.gov.my

3. Enter your username and password.

4. If your username and password is correct, you will see the following screen:

1.1 Cannot Log in

1.1.1 Wrong or Forgot Username

1. Please point your browser to http://sehat.perkeso.gov.my

2. Look for the “Forgot Login” link below the user login function.

3. Click on “Forgot Login”.

4. You will see the following screen:

5. Select “Lost Username”.

6. Enter the email address which you have used during registration.

7. Click “Send Username”.

8. Check your email. Your username will be sent to your email address. Please

check your Spam box if the email is not in your Inbox.

1.1.2 Wrong or Forgot Password

1. Please point your browser to http://sehat.perkeso.gov.my

2. Look for the “Forgot Login” link below the user login function.

3. Click on “Forgot Login”

4. You will see the following screen:

5. Select “Lost Password”.

6. Enter your username and email address which you have used during

registration.

7. Click “Send Password”.

8. Check your email. Your username will be sent to your email address. Please

check your Spam box if the email is not in your Inbox.

1.1.3 Forgot Both Username and Password

1. Repeat step 2.11, once you have received your username. Complete step

2.0 User Menu 1. Once you have login to portal. You will see the ‘User Menu’ as follow.

2.1 Validate Voucher

To confirm the owner of the Health Screening Program voucher by utilizing the voucher

number.

2.2 My Patients

List of patient which has been validated by the clinic.

2.3 Labs and Mammogram Centres

List of registered service providers.

2.4 My Profile

Information about the Clinic Name, Clinic Registration Number Address and contact

number.

2.5 Logout

To register out and disconnect from the SeHAT portal.

3.0 Validating a Voucher

Warning: Please validate the voucher/s before the examination to ensure the voucher

is valid and has not been used before.

1. Click on Validate voucher.

2. You will see the following screen.

3. Please read the instruction carefully.

4. Click on ‘Click Here to Continue with Validation’ to validate the voucher.

6. Enter the voucher number and click ‘Submit’. Your patient’s name will be appearing.

7. Check the patient’s details (e.g.; Full name, Identification No, Voucher No. and Employer

Code) are correct.

8. Click on the patient’s name and you will see the following screening;

9. Check and make sure your clinic and account details are correct before proceed.

10. Click ‘Edit’ and ‘Update Record’ and you will see the following.

11. Please click to confirm voucher. Failure to confirm may result to other HSP Panel Clinic to

claim the unconfirmed voucher. This is an important procedure as if you did not confirm the

voucher, labs and mammogram centre couldn’t validate the voucher.

4.0 Updating a Patient Record

1. To update patient record, move your cursor to ‘Edit’ button and ‘Update Record’ button will appear.

2. Click on ‘Update Record’ button and you will see the following screen;

3. Fill in patient’s address and contact number.

4. Click on ‘Clinical Examination’.

5. You will see the following screen. Fill all details required.

6. When all information is filled, click ‘Save’ and your screening are done. You can now refer your

patient to mammogram centre and the samples to the laboratory.

5.0 Referring a Patient’s Sample to a Lab

1. To refer your patient’s sample to a lab. Please print the following page;

Or photocopy the HSP voucher and enclose together with the blood and urine specimen. For

female who refuse to do Pap Smear test, please note the lab.

2. Once the lab has entered the lab results, you will receive a notification email for SeHAT and

you need to view the results and deliver consultation to the respective patient.

6.0 Referring a Patient for Mammogram

1. To refer your patient for mammogram screening, validate the patient’s mammogram voucher

and repeat step at 3.0 Validating a Voucher.

2. You will see the following screen and click on ‘Update Record’.

3. Click on ‘Clinical Examination’ and answer the following questions.

4. Click on ‘Billing Info’ to confirm your mammogram voucher and you will see as follow;

5. Tick on the first box to confirm your voucher.

5. Click ‘Save’ and you can refer your patient for mammogram screening by following the step at

6.0 Referring a Patient’s Sample to a Lab.

7.0 How do I know that the Report is Complete?

1. On ‘User Menu’, click on ‘My Patient’.

2. You will see the following screen which shows the list of your patients and the summarized

details.

3. Here, you can see the percentage of completion of your patient’s report.

4. If your patient’s report is 100% completed, view the report and refer to ‘File History’ to make

sure that there is no ‘Pending Completion’ in either clinical report or lab report.

5. If there is ‘Pending Completion’ in clinical report. Kindly click on ‘Update Record’ and ‘Save’.

6. If your patient’s report is not 100% complete, you can check what information you have been

missed out.

7. Click on ‘File History’ and you will see as follow.

8. Here, you can see the details that you left empty.

9. Once the details filled, your report will be 100% complete.

8.0 Submit a Record for Payment

8.1 Submit Standard Screening Record.

1. Before submitting the patient’s record, make sure that the patient’s record is 100%

completed and no ‘Pending Completion’ in ‘File History’.

2. In the patient profile, click on ‘Billing Info’ and you will see the following;

3. Click on ‘Update Record’ and you will see the following;

4. Make sure that the amount appear. The amount will appear once report is completed.

5. Click the bottom box to certify the report and and click ‘Save’ button.

6. Report will automatically receive by PERKESO for approval. You will receive the payment

within 14 working days after approval.

Warning: Payment will be processed once report is certified to be completed, true and

correct.

8.2 Submit Mammogram Record.

1. Once the mammogram centre completed the mammogram report, please go to ‘My Patient’.

2. Click on the respective patient profile for mammogram and ‘Update Record’.

3. Please certify that report is completed, true and correct.

Warning: Payment will be processed once report is certified to be completed, true and

correct.

4. Click ‘Save’.

5. Your mammogram record is ready for billing.

9.0 Maintaining my Clinic Information

1. Click ‘My Profile’ at your main page.

2. You will see the following screen. To edit your clinic information, click on ‘Update Details’.

4. Here you can change your username, password and other editable details. Then click ‘Save’.

10.0 Log out

1. To register out and disconnect from the SeHAT portal, back to main page.

2. Click on ‘Logout’.

11.1 Auto Log out

1. If you left this portal idle for more than 5 minutes, this system wills automatically

logout.

2. Re-login to access back to the portal.
