Who reports? All PBIS school buildings that began training

during the 2011-12 school year and earlier.

Each school has its own building code and must enter data for their school.

The DE Requires

Every school listed on the Iowa Public and Non-public schools lists must submit specific data annually.

PBIS data must meet this requirement as well. However, for THIS YEAR there may be some exceptions.

Please note: This is the last year that one data report can be entered

for two schools located at the same, or different, physical addresses. The Department of Education requires that every school with a unique building code provide data for their school, even if there is one principal and one leadership team for both schools.

All schools must use the name of their school as listed on the DE public and non-public school lists.

PBIS Data Collection System Reporting dates extended: January 2 – 25

All AEAs will have their own link that will be sent to each school by their AEA PBIS Coordinator.

It is the responsibility of the school based PBIS Leadership Team to identify the person who will enter the data. Internal coach – team data manager

Please use the worksheet to prepare your data for entry. It is recommended that you keep this copy as you can not print out your completed report unless you print each page separately.

FAQ’s What if I have a problem entering data on the

survey monkey site? Contact Susan Bruce at [email protected]

or call 515-725-2244 Can I enter decimals or zeroes?

Yes and if there is an instance were the data is “0” enter the 0 and do not leave it blank.

Can I print the whole report? No. Each page can be printed separately by right

clicking and selecting “print.”

What if I do not see my school listed in the drop down menu? Contact your PBIS Coordinator or Susan Bruce. If

your school can not be added through the DE, send the completed worksheet to Robin Galloway at [email protected].

What happens if my school does not enter data? Submitting data is required for all implementing

PBIS schools. Schools that do not enter data will be coded as inactive in the PBIS database.

Do schools that use SWIS need to access and report using this system? Yes – At this time, there is no way to merge data

entered into SWIS with the DE PBIS Data Collection System.

By running a Year End Report you can get most of the data needed. You can obtain total number of major referrals from the Avg. Referrals per Month Report. Another report to generate is the Ethnicity Report (data from Graph #2)

Who sees this data and what is done with it? This data helps to evaluate the level of

implementation statewide. Important decisions regarding implementation, sustainability and technical assistance needs are made using this data. The data is aggregated and analyzed on an AEA and state level. Annual reports and data briefs are developed and shared with key stakeholders.

*Data is not analyzed on an individual school basis by the DE.
