FOREWORDTheseYaleNewHavenHealthSystemapprenticeshipstandardshaveastheirobjectivetraininginvariousoccupationsinhealthcaretobeskilledinallphasesoftheindustry.Thesponsorrecognizesthatinordertoaccomplishthis,theremustbewell-developedon-the-joblearningcombinedwithrelatedinstruction.Thisrecognitionhasresultedinthedevelopmentofthesestandardsofapprenticeship.TheyweredevelopedinaccordancewiththebasicstandardsrecommendedbytheConnecticutDepartmentofLabor,OfficeofApprenticeshipTraining,asabasisfromwhichthesponsorcanworktoestablishanapprenticeshiptrainingprogramthatmeetstheparticularneedsofthearea.

SECTIONI–PROGRAMADMINISTRATIONResponsibilitiesoftheYaleNewHaven

A. Cooperateintheselectionofapprenticesasoutlinedinthisprogram.

B. Ensurethatallapprenticeshaveunderwrittenapprenticeshipagreements.

C. Reviewandrecommendapprenticeshipactivitiesinaccordancewiththisprogram.

D. Establishtheminimumstandardsofeducationandexperiencerequiredofapprentices.

E. RegisterthelocalapprenticeshipstandardswiththeRegistrationAgency.

F. Hearandresolveallcomplaintsofviolationsofapprenticeshipagreements.

G. Arrangeevaluationsofapprentices'progressinmanipulativeskillsandtechnicalknowledge.

H. Maintain records of all apprentices, showing their education, experience, and progress inlearningtheoccupation.

I. Certify to the Registration Agency that apprentices have successfully completed theirapprenticeshipprogram.

J. Notify,within45days,theRegistrationAgencyofallnewapprenticestoberegistered,creditgranted, suspensions for any reason, reinstatements, extensions, completions, andcancellationswithexplanationofcauses.

K. Supervise all the provisions of the local standards and be responsible, in general, for thesuccessfuloperationof the standardsbyperforming thedutieshere listed. Cooperatewithpublic and private agencies, which can be of assistance in obtaining publicity to developpublic support of apprenticeship. Keep in contact with all parties concerned, includingapprentices,employers,andjourneyworkers.

L. Provide each apprenticewith a copy of these standards, alongwith any applicablewrittenrules and policies. Require the apprentice to sign an acknowledgment receipt of same.Followthisprocedurewheneverrevisionsormodificationsaremadetotherulesandpolicies.

M. Whennotified thatanapprentice’s related instructionoron-the-jobprogress is found tobeunsatisfactory, the sponsor and employer will determine whether the apprentice shouldcontinueinaprobationarystatusandmayrequiretheapprenticetorepeataprocessorseriesofprocessesbeforeadvancingtothenextwageclassification.Shoulditbefoundinthecourseof thisdeterminationthat theapprenticedoesnothavetheabilityordesiretocontinuethetraining to become a journeyworker, the sponsorwill, after the apprentice has been givenadequate assistance and opportunity for corrective action, terminate the apprenticeshipagreement,asprovidedin29CFR§29.7(h)(1)(2)(i)and(ii).

N. Theemployerwillprovideeachregisteredapprenticewithcontinuousemploymentsufficientto provide the opportunity for completion of his or her apprenticeship program. If thesponsor isunable to fulfill its trainingand/or theemployerunable to fulfill itsemploymentobligation inconformancewith thesestandards, thesponsorwill,perSectionXXIIIof thesestandardsandwiththeapprentice’sconsent,makeagood-faithefforttofacilitateatransferoftheapprenticetoanotherregisteredemployerforcompletionoftheapprenticeship.

If conditions of business make it necessary to temporarily suspend the period ofapprenticeship, apprentices suspended for this reason will be given the opportunity to

resume their active apprenticeships before any additional apprentices are employed. Thesuspensionandreinstatementofapprenticesshallbedoneinrelationtoretentionofthemostadvancedapprenticeandinaccordancewiththecompanypolicyforbreaksinseniority.

ResponsibilitiesofNurseResidencyProgram(NRP)Coordinator:

• Provide oversight for theNRP, supervising scheduling andprogram communicationforthenurseresidents,unitandservicelineleadership,andotherkeystakeholders

• Notifyunitleadershipofanynurseresidentabsenceintheprogram

• Communicateconcernsandperformanceofnurseresidentstounitleadership

• Facilitateprogramdatacollectionandoutcomestrackingasappropriate

ResponsibilitiesofResidentFacilitator

• Experienced, professional, clinically competent nurse withminimum of Bachelor ofScience Nurse (BSN) who commits to investing in the professional development ofnewlylicensednursesparticipatingintheNRP

• Facilitate monthly NRP clinical reflections session and communicate themes andconcerns to the NRP Coordinator verbally or through the "resident FacilitatorFeedback"form

• Establishinterpersonalrelationshipwiththenurseresidentthatconveysinvestmentinthenurseresident’ssuccessanddevelopment

Responsibilities of Patient Service Manager (PSM)/Assistant Patient Service Manager(APSM)/Educator:

• EnsurenurseresidentsareabletoattendmonthlyNRPsessionswithassignedcohort

• Provide guidance and information to the nurse resident as appropriate to assist inprogramcompletion

• ConferwithnurseresidentincompletionofProfessionalDevelopmentPlan

• Conferwith nurse resident and approve Evidence Based Practice (EBP) LeadershipProjectTopic,provideassistancewithanyunit-specificinformation

• AttendEBPLeadershipProjectPresentationforanynurseresidentonyourunit

ResponsibilitiesofAdvisoryBoard:

• Provide strategic direction and advocacy for policies and resources necessary toensure participation in the Vizient/American Association of Colleges of Nursing(AACN)NurseResidencyProgram;promotetheacquisitionofknowledge,skills,andprofessionalbehaviorsnecessarytodeliversafe,high-qualitycare

SECTIONII-EQUALOPPORTUNITYPLEDGE–Title29CFR§§29.5(b)(21)and30.3(b):Yale New Haven Health System will not discriminate against apprenticeship applicants orapprentices based on race, color, religion, national origin, sex (including pregnancy and genderidentity),sexualorientation,geneticinformation,orbecausetheyareanindividualwithadisabilityoraperson40yearsoldorolder. YaleNewHavenHealthSystemwill takeaffirmativeaction toprovide equal opportunity in apprenticeship and will operate the apprenticeship program asrequired under Title 29 of the Code of Federal Regulations, part 30. and Chapter 814c of theConnecticutGeneralStatutes.SECTIONIII-AFFIRMATIVEACTIONPLAN–Title29CFR§§29.5(b)(21)and30.5Sponsorswithfive(5)ormoreregisteredapprenticesarerequiredtoadoptanaffirmativeactionplan (Appendix C) and selection procedures (Appendix D), which will become part of thesestandards of apprenticeship. However, the Office of Apprenticeship Training encourages thedevelopmentofthesetwoplansforallprogramsregardlessofapprenticenumbers.Forprogramswithfewerthanfive(5)apprentices,theseplansarenotrequired,andthesponsormaycontinuetoselect apprentices in conformance with its current human resources and equal employmentopportunityhiringpolicies.SECTIONIV-QUALIFICATIONSFORAPPRENTICESHIP–Title29CFR§29.5(b)(10)ApplicantswillmeetthefollowingminimumqualificationsfortheprogramandthosespecifictotheoccupationselectedlistedinAppendixA.Thesequalificationstandards,andthescorerequiredonanystandard foradmission to theapplicantpool,mustbedirectly related to jobperformance,asshown by a statistical relationship between the score required for admission to the pool andperformanceintheapprenticeshipprogram:

A. Age

Minimumqualificationsrequiredbythesponsorforpersonsenteringtheapprenticeshipprogram,withaneligiblestartingagenotlessthan18years.

B. Education

HoldaBachelor,Associate,orDiplomadegreeinNursing.

C. Physical

Applicants will be physically capable of performing the essential functions of theapprenticeship program, with or without a reasonable accommodation, and withoutposingadirectthreattothehealthandsafetyoftheindividualorothers.

SECTIONV-APPRENTICESHIPAGREEMENT–Title29CFR§§29.3(d)and(e)and29.5(b)(11)After selection of an applicant for apprenticeship, but before employment as an apprentice orenrollment in related instruction, awritten Apprenticeship Agreementwill cover the apprentice(AppendixB)signedbytheEmployerandtheapprenticeandapprovedbyandregisteredwiththeRegistration Agency (Connecticut Department of Labor, Office of Apprenticeship Training), andsubmitted to the Sponsor. Such agreement will contain a statement making the terms andconditions of these standards a part of the agreement as though expresslywritten therein. TheSponsorwill furnishacopyofeachApprenticeshipAgreementtotheapprentice, theRegistrationAgency (ConnecticutDepartment of Labor,Office ofApprenticeshipTraining), and theEmployer.Additional copies are provided to the Veteran’s State Approving Agency for those veteranapprenticesdesiringaccesstoanybenefitstowhichtheyareentitled.PriortosigningtheApprenticeshipAgreement,eachselectedapplicantwillbegivenanopportunityto readand review theseStandardsofApprenticeship, theEmployer’swritten rulesandpolicies,andtheApprenticeshipAgreement.TheRegistrationAgency(ConnecticutDepartmentofLabor,OfficeofApprenticeshipTraining)willbeadvisedwithinforty-five(45)daysoftheexecutionofeachApprenticeshipAgreementandwillprovidealltheinformationrequiredforregisteringtheapprentice.SECTIONVI–SUPERVISIONOFAPPRENTICESANDRATIOS–Title29CFR§29.5(b)(7)Noapprenticeshallworkwithoutproperoradequatesupervisionofthejourneyworker.For the purpose of these apprenticeship standards, adequate or proper supervision of theapprentice means the apprentice is under the supervision of a fully qualified journeyworker orsupervisor at all times who is responsible for making work assignments, providing OJL, andensuringsafetyattheworksite.Toadequatelyorproperlysuperviseanapprenticedoesnotmeantheapprenticemustbewithineyesightorreachofthesupervisor,butthatthesupervisorknowswhattheapprenticeisworkingon; is readily available to the apprentice; and is making sure the apprentice has the necessaryinstructionandguidancetoperformtaskssafely,correctly,andefficiently.The sponsor shall establish a numeric ratio of apprentices to fully proficient workers (journeyworkers/mentor/ trainer) consistentwithproper supervision, training, safety, and continuity ofemployment throughout the apprenticeship. The ratio language must be specific and clearlydescribed as to its application to the jobsite, workforce, department, or plant. The ratio ofapprentices to fully proficient workers (journey workers/ mentor/ trainer) will be as noted inAppendixA.

SECTIONVII-TERMOFAPPRENTICESHIP–Title29CFR§29.5(b)(2)The term of occupation is described for the occupation supplemented by the required hours ofrelated instruction as stated on the Work Process Schedule and Related Instruction Outline(AppendixA).The employer may utilize a career lattice as a pathway for apprentices to move upward in theorganization, which may or may not include an interim credential leading to the Certificate ofCompletionofApprenticeship.SECTIONVIII-PROBATIONARYPERIOD–Title29CFR§§29.5(b)(8),(b)(20)All applicants selected for apprenticeship will serve a probationary period. The probationaryperiodissixmonthslong,basedonYNHHhumanresourcespolicy.During the probationary period, either the apprentice or the Sponsor may terminate theApprenticeship Agreement, without stated cause, by notifying the other party in writing. Therecords for each probationary apprenticewill be reviewed prior to the end of the probationaryperiod. Recordsmayconsistofperiodicreportsregardingprogressionmade inboththeOJLandrelatedinstruction,andanydisciplinaryactiontakenduringtheprobationaryperiod.Any probationary apprentice evaluated as satisfactory after a review of the probationary periodwillbegivenfullcreditfortheprobationaryperiodandcontinueintheprogram.Aftertheprobationaryperiod,theApprenticeshipAgreementmaybecanceledattherequestoftheapprenticeormaybesuspendedorcanceledbytheSponsorortheEmployerforreasonablecauseafterdocumentedduenoticetotheapprenticeandareasonableopportunityforcorrectiveaction.Insuchcases, theSponsorwillprovidewrittennoticetotheapprentice, theEmployer,andtotheRegistrationAgencyofthefinalactiontaken.SECTIONIX-HOURSOFWORKApprenticeswillgenerallyworkthesamehoursasjourneyworkers,exceptthatnoapprenticewillbeallowedtoworkovertimeifitinterfereswithattendanceinrelatedinstructionclasses.ApprenticeswhodonotcompletetherequiredhoursofOJLduringagivensegmentwillhavethetermofthatsegmentextendeduntiltherequirednumberofhoursoftrainingareaccrued.

SECTIONX-APPRENTICEWAGEPROGRESSION–Title29CFR§29.5(b)(5)Apprenticeswill bepaid aprogressively increasing scheduleofwages and fringebenefits duringtheirapprenticeshipbasedontheacquisitionof increasedskillandcompetenceonthe jobandinrelatedinstruction.Before an apprentice is advanced to the next segment of training or to fully proficient orjourneyworker status, the sponsorwill evaluate all progress to determinewhether advancementhas been earned by satisfactory performance in OJL and in related instruction courses. Indeterminingwhethersatisfactoryprogresshasbeenmade,thesponsorwillbeguidedbytheworkexperienceandrelatedinstructionrecordsandreports.The progressive wage schedule and fringe benefits will be an increasing percentage of the fullyproficientorjourneyworkerwagerate.Thepercentagesthatwillbeappliedtotheapplicablefullyproficient or journeyworker rate are shownon the attachedWorkProcess Schedule andRelatedInstructionOutline(AppendixA).Innocasewillthestartingwagesofapprenticesbelessthanthatrequiredbyanyminimumwagelawthatmaybeapplicable.SECTION XI - CREDIT FOR PREVIOUS EXPERIENCE – Title 29 §§ CFR 29.5(b)(12) and30.4(c)(8)Yale New Haven Health System may grant credit towards the term of apprenticeship to newapprenticeswhodemonstratepreviousacquisitionofskillsorknowledgeequivalenttothatwhichwouldbereceivedundertheseStandards.ApprenticeapplicantsseekingcreditforpreviousexperiencegainedoutsidethesupervisionoftheEmployermustsubmit therequestat the timeofapplicationand furnishsuchrecords,affidavits,and any additional documentation as requested by the Employer to substantiate the claim.Applicantsrequestingsuchcreditwhoareselectedintotheapprenticeshipprogramwillstartatthebeginningwage rate. The request for creditwill be evaluated and a determinationmade by theEmployer during the probationary period when actual on-the-job and related instructionperformancecanbeexamined.Priortocompletionoftheprobationaryperiod,theamountofcreditto be awarded will be determined after review of the apprentice’s previous work andtraining/educationrecordandevaluationof theapprentice’sperformanceanddemonstratedskillandknowledgeduringtheprobationaryperiod.Anapprenticegrantedcreditwillbeadvancedtothewageratedesignatedfortheperiodtowhichsuchcreditaccrues.TheSponsorandRegistrationAgencywillbeadvisedofanycreditgrantedandthewageratetowhichtheapprenticeisadvanced.Thegrantingofadvancedstandingwillbeuniformlyappliedtoallapplicableapprentices.

SECTIONXII-WORKEXPERIENCE–Title29CFR§29.5(b)(3)and30.8DuringtheapprenticeshiptheapprenticewillreceivesuchOJLandrelatedinstructioninallphasesoftheoccupationnecessarytodeveloptheskillandproficiencyofaskilledjourneyworker.TheOJLwillbeunderthedirectionandguidanceoftheEmployer’ssupervisoroftheapprentice(s).SECTIONXIII-RELATEDINSTRUCTION–Title29CFR§29.5(b)(4)EveryapprenticeisrequiredtoparticipateincourseworkrelatedtothejobasoutlinedinAppendixA. A minimum of 144 hours of related instruction is recommended for each year of theapprenticeship. Apprentices agree to take such courses, as the sponsor deems advisable. Thesponsor will secure the instructional aids and equipment it deems necessary to provide qualityinstruction.Incities,towns,orareashavingnovocationalschoolsorotherschoolsthatcanfurnishrelated instruction, the sponsor may require apprentices to complete the related instructionrequirementthroughelectronicmediaorotherinstructionapprovedbytheRegistrationAgency.Thesponsorwillinformeachapprenticeoftheavailabilityofcollegecredit(ifapplicable).Anyapprenticewho isabsent fromrelated instructionwillsatisfactorilycompleteallcourseworkmissedbeforebeingadvancedtothenextperiodoftraining.Incasesoffailureofanapprenticetofulfill the obligations regarding related instruction without due cause, the sponsor will takeappropriatedisciplinaryactionandmayterminatetheapprenticeshipagreementafterduenoticetotheapprenticeandopportunityforcorrectiveaction.To theextentpossible, related instructionwillbecloselycorrelatedwith thepracticalexperienceandtrainingreceivedonthejob.Thesponsorwillmonitoranddocumenttheapprentice’sprogressinrelatedinstructionclasses.Thesponsorwillsecurecompetent instructorswhoseknowledge,experience,andabilitytoteachwill be carefully examined and monitored. SECTION XVIII - CANCELLATIONANDDEREGISTRATION – Title 29 CFR §§ 29.5(b)(18) and29.8(a)(2),and2938(b)(8)These Standards will, upon adoption by Yale New Haven Health System, be submitted to theRegistration Agency for approval. Such approvalwill be acquired before implementation of theprogram.YaleNewHavenHealth System reserves the right to discontinue at any time the apprenticeshipprogramsetforthherein. TheRegistrationAgencywillbenotifiedwith45daysinwritingofanydecisiontocanceltheprogram.TheRegistrationAgencymayinitiatederegistrationofthesestandardsforfailureofthesponsortoabide by the provisions herein. Such deregistrationwill be in accordancewith the RegistrationAgency’sregulationsandprocedures.Yale New Haven Health System will notify each apprentice of the cancellation of the program(whether voluntary or involuntary), and the effect of same. If the apprenticeship program iscancelledatthesponsor’srequest, thesponsorwillnotifytheapprentice(s)within15daysofthedate of the Registration Agency’s acknowledgment of the sponsor’s request. If the RegistrationAgency orders deregistration of the apprenticeship program, the sponsor will notify theapprentice(s)within15daysoftheeffectivedateoftheorder.Thisnotificationwillconformtotherequirementsof29CFR§29.8.SECTIONXIX-AMENDMENTSORMODIFICATIONS–Title29CFR§29.5(b)(18)These Standardsmay be amended or modified at any time by the Sponsor provided that noamendmentormodificationadoptedwillalteranyApprenticeshipAgreementinforceatthetimewithout the consent of all parties. SECTIONXXI-TRANSFEROFANAPPRENTICEANDTRAININGOBLIGATION–Title29CFR§29.5(13)The transfer of an apprentice between apprenticeship programs and within an apprenticeshipprogram must be based on agreement between the apprentice, the affected apprenticeshipcommitteesorEmployers,andtheSponsor,andmustcomplywiththefollowingrequirements:

i. Thetransferringapprenticemustbeprovidedatranscriptofrelatedinstructionandon-the-joblearningbythecommittee,Employer,orSponsor;

ii. Transfermustbetothesameoccupation;andiii. A new Apprenticeship Agreement must be executed when the transfer occurs

betweentheEmployers.Theapprenticemust receive credit from thenewEmployer for the trainingalreadysatisfactorilycompleted.If the Employer is unable to fulfill his/her training obligation due to lack of work or failure toconform to these Standards, the Employer will make every effort to refer the apprentice withhis/herconsenttoanotherEmployer,theSponsor,andtheRegistrationAgencyforplacementintoanother registered apprenticeship program. Thiswill provide the apprentice an opportunity forcontinuousemploymentandcompletionoftheirapprenticeshipprogram.SECTIONXXII-RESPONSIBILITIESOFTHEAPPRENTICEApprentices,havingreadtheseStandardsformulatedbyYaleNewHavenHealthSystemandsignedan Apprenticeship Agreement with Yale New Haven Health System, agree to all the terms andconditions contained therein and agree to abide by Yale New Haven Health System’s rules andpolicies,includinganyamendments,servesuchtime,performsuchmanualtraining,andstudysuchsubjects,astheSponsormaydeemnecessarytobecomeskilledintheoccupation.In signing the Apprenticeship Agreement, apprentices assume the following responsibilities andobligations under the apprenticeship program. HYBRID OCCUPATION: An occupation using an apprenticeship approach that measures theindividualapprentice’sskillacquisitionthroughacombinationofaspecifiedminimumnumberofhours of on-the-job learning and the successful demonstration of competency as described in aworkprocessschedule.INTERIM CREDENTIAL: A credential issued by the Registration Agency, upon request of theappropriatesponsor,ascertificationofcompetencyattainmentbyanapprentice.JOBCORPSCENTER:AnyofthefederallyfundedJobCorpscentersthroughouttheU.S.andPuertoRico.JobCorpsservesyouthsandyoungadults16-24yearsofage.EmployersthatwishtohireJobCorpsgraduateswhoaretrainedinanyoccupationcoveredunderthesestandardsandwhomeettheminimumqualificationsforapprenticeshipmaydosoviathedirectentryprovisiondescribedinAppendixD:QualificationsandSelectionProcedures.JOURNEYWORKER: A worker who has attained a level of skill, abilities, and competenciesrecognized within an industry as having mastered the skills and competencies required for theoccupation. REGISTRATION AGENCY: Is identified as the Connecticut Department of Labor, Office ofApprenticeshipTraining,200FollyBrookBlvd,Wethersfield,CT,06109. OfficeofApprenticeshipTraining is responsible for registering apprenticeship programs and apprentices, providingtechnicalassistance,conductingreviewsforcompliancewith29CFR§§29and30,andconductingqualityassuranceassessments.RELATED INSTRUCTION: Anorganized and systematic formof instruction designed to providethe apprentice with the knowledge of the theoretical and technical subjects related to theapprentice’s occupation. Such instructionmay be given in a classroom, through occupational orindustrial courses, or by correspondence courses of equivalent value, electronicmedia, or otherformsofself-studyapprovedbytheRegistrationAgency.SPONSOR: Any person, association, committee, or organization that operates an apprenticeshipprogram and inwhose name the program is registered. That assumes the full responsibility foradministrationandoperationoftheapprenticeshipprogram.

STANDARDS OF APPRENTICESHIP: This entire document including all appendices andattachments hereto, and any future modifications or additions approved by the RegistrationAgency.SUPERVISOROFAPPRENTICE(S):AnindividualdesignatedbytheEmployertosuperviseorhavechargeanddirectionofanapprentice.TIME-BASEDOCCUPATION:Anoccupationusinganapprenticeshipapproachthatmeasuresskillacquisition through the individual apprentice’s completion of at least 2,000 hours of on-the-joblearningasdescribedinaworkprocessschedule.TRANSFER: A shift of Apprenticeship Agreement from one program to another or from oneEmployer within a program to another Employer within that same program, where there isagreement between the apprentice, the affected apprenticeship committee or Employer, and theSponsor.YOUTHBUILD: A youthand communitydevelopmentprogram that addresses core issues facinglow-income communities: housing, education, employment, crime prevention, and leadershipdevelopment.
