Chronic (Addison's Disease)

For symptoms : weakness, fatigue, weight loss, hypotension,

hyperpigmentation.

Prevent hypoglycemia : inability to maintain the blood glucose level during fasting.

Prevent hypotensive crisis: In such individuals, minor noxious,

traumatic, or infectious stimuli may produce acute adrenal

insufficiency with circulatory shock and even death.

Acute adrenal Insufficiency

mainly Waterhouse-Friderichsen syndrome.

sudden withdrawal of long-term corticosteroid

therapy.

stress in patients with underlying chronic adrenal

insufficiency.The latter is termed critical illness–related corticosteroid insufficiency.

The dexamethasone stimulation test is used for the diagnosis of Addison's disease .

Treatment of Non adrenal disorders

Pharmacological Actions For most clinical purposes, synthetic glucocorticoids are used

because they have a higher affinity for the receptor, are less

activated and have little or no salt-retaining properties.

Hydrocortisone used for:

orally for replacement therapy,

i.v. for shock and asthma,

topically for eczema (ointment)

enemas (ulcerative colitis).

Prednisolone

the most widely used drug given orally in inflammation and

allergic diseases.

Pharmacological Actions Betamethasone and dexamethasone:

very potent,

w/o salt-retaining properties;

thus, very useful for high-dose therapies

(e.g., cerebral edemas).

Beclometasone, budesonide:

pass membranes poorly;

more active when applied topically (severe eczema for local

anti-inflammatory effects) than orally;

used in asthma, (aerosol).

Triamcinolone:

used for severe asthma

for local joint inflammation (intra-articular inj.).

The only glucocorticoid that has no effect on the fetus in pregnancy is prednisone [PRED-ni-sone]. It is a prodrug that is not converted to the active compound, prednisolone [pred-NIH-so-lene], in the fetal liver

Special Dosage Forms

• Beclomethasone, and several other glucocorticoids—

primarily budesonide and flunisolide, administered as

aerosols—have been found to be extremely useful in the treatment of asthma

• Beclomethasone, triamcinolone, budesonide, flunisolide,

are available as nasal sprays for the topical treatment of

allergic rhinitis. DOSES levels that are too low to influence adrenal function or have any other systemic effects.

• Corticosteroids incorporated in ointments, creams, lotions,

and sprays are used extensively in dermatology. These preparations are discussed in more detail in Chapter 61

1. Lipolysis + gluconeogenesis + protein catabolism = hyperglycemia agitation

, anxiety , mental status , tremor ] = diabetes = insulin resistance

2. Rapid Ca exertion + reduced Ca absorption = hypocalcemia = elevated PTH =

osteoporosis

3. Catabolic status = negative nitrogen balance =

muscle loss = reduced lean body mass = wasting myopathy = weakness

4. Minerocorticoid activity : hypernatremia (rare), hypokalemia, fluid retention,

increased blood urea nitrogen concentration.

Glucocorticoids have been reported to decrease the secretion of thyrotropin (TSH)

Cardiovascular effect and side effect

1. Synergistic effect with alpha receptors : vasoconstriction =

Dose effect

Patients treated with an average of 10 mg per day over

several months developed 50% fewer infections

compared to those treated with an average of 20 mg per

day.

Drugs that Reduce Corticosteroid Effects

Cholestyramine binds bile in the gastrointestinal tract to prevent its reabsorption

*these drugs decrease the absorption of corticosteroids

Phenytoin: inhibits cytochrome P450 enyzymes
