Information & Background

The NHS England Early Diagnosis Cancer Team published the following documents in April 2019. They are available at https://www.england.nhs.uk/cancer/resources.

The Cancer Waiting Times Guidance v10.0 is also available from NHS Digital’s cancer waits page: https://digital.nhs.uk/data-and-information/data-collections-and-data-sets/data-collections/cancerwaitingtimescwt.

The oesophago-gastric cancer pathway handbook will support the system to deliver part of the NHS Planning Guidance 2019/20 (see s2.2.4 in Annex B which sets out a requirement for cancer alliances to lead on local implementation – https://www.england.nhs.uk/operational-planning-and-contracting/).

The oesophago-gastric cancer pathway is also referenced in the Cancer Waiting Times Guidance v10.0 (see section s2.6.2 on how to ‘start the clock’ for this pathway in regards to the new Faster Diagnosis Standard).

Therefore for pathways that are GP Direct Access OGD – triage to been seen in OPA – OPA. Start a 2ww referral at data of triage. OPA will be 2ww clock stop. The “triage” is likely to be decision on day of OGD itself.

The Cancer Alliances are required to report on a set of specific deliverables, these being:

Timed pathway for oesophago-gastric (OG) cancer

% % of Trusts that have performed a gap analysis on implementation of the 5 pathway steps (expected by Q1)

% % of Trusts that have a trajectory and plan agreed to implement the 5 pathway steps (expected by Q2)

% % of Trusts that have implemented the 5 pathway steps (Q3)% % of Trusts that have implemented the 5 pathway steps (Q4)

To support all trusts in the implementation of the best practice timed pathway, the SWAG Cancer Alliance have set aside £125,000 (deliverable 1.8) in funds with an aim of compliance with CWT diagnostic standards for Optimised OG Pathway and ensure quality standardisation reducing variability across region.

The project timescales we have for this are:

In Q1 & Q2 Map OG pathway at all trustsAgree IDA pathwayIdentify inefficiencies delaying diagnosis,Agree funding for trust improvements

In Q3 & Q4 Agree best practice in OG pathway managementDeliver trust improvements

OGD Gap Analysis:

We have asked each cancer manager in the Trusts in the SWAG alliance to undertake a GAP analysis audit of their existing OGD pathway.

As this is the first time that we have asked for this to be completed for OGD, we asked for as much information as they have and with their ‘real knowledge’ of their current service – rather than what their pathway defines.

The original deadline of 31st July, for completion and return, was extended to 21st August; to enable all trusts to complete and taking into account summer holiday/annual leave pressures.

These are to be to collated and reviewed to enable to SWAG cancer alliance to look at how we can best support all trusts in the implementation of the best practice timed pathway.
