Admission and Re-Entry Policy The candidate must be able to provide government issued photo identification for proof of age and a high school gradu-ate or equivalency. Students under the age of 18 must have parental consent and parents will be required to sign on all enrollment documents. The Chrysm Institute of Esthetics does not currently admit students under post-secondary eligi-bility provision Ability to Benefit (ATB) test. A fully com-pleted application form must be submitted along with a non-refundable $50 application fee, a copy of High School diplo-ma or equivalency, a copy of government-issued identifica-tion, and immigration information (if applicable). The appli-

cation fee is refundable only if the candidate is not accepted. All high school diplomas must be verifiable. Acceptable documentation for checking the validity of a stu-dent’s high school completion can include; the diploma and a final transcript that shows all courses taken, or the judgment of the state department of education in which the high school is located. A student’s self-certification is not sufficient to validate the high school diploma in question. If the institution has reason to believe that the high school diploma is not valid or was not obtained from an entity that provides secondary school education (for instance, if the diploma or transcript was bought or the student was required to per-form little or no work), the institution will evaluate the validity of the student’s high school completion. There is no appeals process held by the institution or the Department of Education if the high school diplo-ma is deemed invalid. A student is notified of acceptance when the enrollment contract is signed. The school shall be responsible and liable for the act of its admissions personnel. No school, agent, or admission personnel shall knowingly make any statement or representation that is false, inaccurate, or misleading re-garding the school. Students with Foreign High School Diploma Students with foreign high school diplomas are responsible for translating their documents into English in addition to having their diploma evaluated for U.S. High School Diploma Equivalency by a qualified agen-cy. The translated and evaluated diploma is a part of the application process, therefore, students cannot ap-ply prior to the translation and evaluation of their diploma. Re-Entry Students wishing to re-enter are considered on a case by case basis and may re-enter a program after they have withdrawn at the discretion of Director of Education. Re-entry requests are reviewed to determine whether the circumstances that prevented the student from committing to their schedule have been resolved and additionally are subject to class availability at the requested time of re-entry. Additional tuition charges will not be assessed if the student re-enrolls within 180 days from their Last Date of Attendance. The tui-tion rates current at the time of re-entry will apply to the balance of training hours needed for students who re-enroll more than 180 days from their Last Date of Attendance unless other arrangements have been approved

by the Director of Education. Students who withdraw prior to completion of the course and wish to re-enroll will return in the same satisfactory academic progress status as at the time of withdrawal. Credit for Experience/Transfer It is the policy of The Chrysm Institute of Esthetics to credit hours previously earned at State licensed learn-ing institutions. A certified and verifiable transcript is required from an accredited institution. In addition, you must successfully pass theory exams. An evaluation of practical application may be necessary for de-termining the number of hours you will be credited and the number needed to complete the course. A maxi-mum of 300 hours may be transferred in accordance with Virginia State Regulations. The Chrysm Institute of Esthetics does not guarantee transferability of credits earned to any other institution. With regard to Sat-isfactory Academic Progress, a student’s transfer hours will be counted as both attempted and earned hours.

Prerequisite: You must have scheduled Professional Esthetics Boards or be a cur rently Licensed Professional Esthetician. The school’s program offerings are consistent with its stated purpose, to provide career focused education that inspires and empowers our diverse student body to succeed in employment with confident, profes-sional skills in the exciting, growing field of Esthetics. Name and Level of Employment Professional Esthetics program graduates, who attain licensure, are able to practice as Licensed Estheti-cians in the State of Virginia. Occupation opportunities and levels of employment for Licensed Estheti-cians range from entry level Esthetician positions available in salons and spas through skin care facility operator/ownership. Master Esthetics program graduates, who attain licensure, are able to practice as Licensed Master Estheti-cians in the State of Virginia. Occupation opportunities and levels of employment for Licensed Master Estheticians range from Master Esthetician positions available in salons, spas and Medical facilities with Physicians. Additional occupation opportunities include Skin care facility operation and ownership. Professional Esthetics Course and Activities Description 600 Hour- Professional Esthetics Training Program The program combines theory and 220 practical applications to prepare students to confidently practice in a professional role in the field of esthetics, SOC 39-5094. A student must complete the required studies as well as pass written and practical examinations before graduating. Educational Objectives: By the end of the course, students should be able to perform consultation, cleansing and analysis proce-dures, manual facials and treatments, machine and electrical treatments, body treatments and back treat-ments, make-up application and hair removal. Professional Esthetics Practical Application This course covers the full spectrum of basic techniques required for use by the professional esthetician. The student will be introduced to a variety of product lines and learn the proper use of equipment. The following minimum number of practicums must be completed: Consultation, Cleansing and Analysis 35 Manual Facials and Treatments 65 Machine or Electrical Facials 50 Body Treatments and Back Treatments 20 Make-up 25 Hair Removal 25

Theory & Methodology An understanding of the history of the field, an overview of human anatomy and physiology, and a solid grasp of theory and methodology of esthetics are necessary requirements for this class. An outline of the curriculum is as follows:

Instructional Methods: Class begins with theory instruction where students will take notes from lectures given by the instruc-tor. Workbook exercises are assigned to be completed as homework and students are tested on infor-mation chapter by chapter. The second part of class is practical instruction, where the practical instruc-tor guides the students through the application of procedures learned in class. Grading Procedures Each grade is calculated by averaging test scores; the number of questions answered correctly is divid-ed by the total number of points possible/questions on the test. The grading scale for both theory examinations and practical evaluations shall be as follows with a passing grade of a score of 75% (C) or above: Excellent-A: 100-93, Very good-B: 92-85 Satisfactory-C: 84-75, Unsatisfactory-F: 74 or below

600 Hour– Master Esthetics Training Program The program combines theory and practicum to prepare students to confidently practice in the field of esthetics, SOC 39-5094. A student must complete the required studies as well as pass written and practi-cal examinations before graduating. Facility The Chrysm Institute of Esthetics teaches from its only location, centrally located in the Town Center area of Virginia Beach, Virginia. Virginia Beach is a popular family oriented vacation destination locat-ed on the East coast. The institution is located at 281 Independence Blvd, Suite 101, Virginia Beach, VA 23462, and has an expanded campus facility in Suites 202 and 208, of the same building. The facility includes: two fully equipped large practicum classrooms, makeup classroom, locker room, Theory classroom, student library, and reception and waiting area with fully stocked retail display. In 2015, The Chrysm Institute of Esthetics expanded to accommodate a growing Master Esthetics program. The expansion to the facility includes an open floor plan practical room, reception area, student testing center, instructor office, and a private room for laser and IPL treatments. We are proud to have available to the students, state of the art equipment and tools that exceed program requirements. The school and its facilities are adequately equipped to meet the needs of handicapped students and individuals in need of the assistance of a wheelchair or crutches. Additionally, students with intellectual disabilities must pro-vide medical documentation and may discuss their specific needs with the Director of Education.

Satisfactory Academic Progress Policy All students who attend The Chrysm Institute of Esthetics will be held to our mandated Satisfactory Ac-ademic Progress policy regardless of payment method, program and schedule. A complete list of these standards is included in our school catalog ensuring notification of these expectations prior to enrollment in any program. Minimum Satisfactory Academic Progress Requirements Minimum satisfactory academic progress shall be determined by a cumulative grade average of theory and practicum grades of no less than 75% and 80% attendance rate. The attendance percentage is deter-mined by dividing the total hours accrued by the total number of hours scheduled. Grading Scale The grading scale for both theory examinations and practical evaluations shall be as follows with a pass-ing grade of a score of 75% (C) or above:

Excellent-A: 100-93, Very good-B: 92-85, Satisfactory-C: 84-75, Unsatisfactory-F: 74 or below

Academic Progress Evaluations The qualitative element used to determine academic progress is a reasonable system of grades as deter-mined by assigned academic learning. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated at the end of each unit of study. Practical as-signments are evaluated as completed and counted toward course completion only when rated as satis-factory or better. If the performance does not meet satisfactory requirements, it is not counted and the performance must be repeated. Student’s academic progress is evaluated at the end of each actual 300 clock hour interval.. The first evaluation will occur no later than the midpoint of any program. The final evaluation may include a comprehensive theory examination, final practicum evaluation and up to four mock state board exams. At the end of each evaluation period, the school will determine if the student has maintained at least 80% cumulative attendance since the beginning of the course which indicates, given the same attendance rate, the student will graduate within the maximum time frame allowed. Evaluations will include a review of theory test grades, missing exams or assignments as well as practi-cal skill development. Adequate practical skill development shall be determined by the following:

1) Passing grades in practicum evaluations 2) Maintaining progression in skills determined by practicum evaluations 3) Practicum hours yet to be completed in relation to hours remaining on the contract.

Students must maintain practicum progress that allows for completion of all requirements within the contract time frame. Attendance Policy Students are required to attend a minimum of 80% of the hours possible based on the applicable attend-ance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum require-ments. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 80% cumulative attendance since the begin-ning of the course which indicates that, given the same attendance rate, the student will graduate within the maximum time frame allowed. When a student must miss school, absences should be both valid and verifiable, i.e., illness, bereave-ment, etc. All students should notify the Institution if they are going to be late or absent as soon as is possible. It is the responsibility of the student to complete assignments and any material covered during the absence. A leave of absence may be granted at the discretion of the school director, provided the re-quest is made in advance and in writing. Leave of absence will be evaluated on a case by case basis.

Determination of Progress Status Students who meet the minimum requirements for attendance and academic performance are considered to be making satisfactory academic progress until the next scheduled evaluation. A hard copy of their Satisfactory Progress Determination will be made available to the student at the time of each evaluation and students will be notified of any evaluation that impacts eligibility for financial aid, if applicable. Students deemed not maintaining satisfactory progress may have their Title IV funding interrupted, un-less the student is on warning or has prevailed upon appeal resulting in a status of Probation. Warning Students who fail to meet minimum requirements for attendance or academic progress are placed on warning and considered to be making satisfactory progress while during the warning period. The stu-dent will be advised in writing on the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the warning period the student has still not met both the attendance and academic requirements, he/she may be placed on probation and if applicable, students may be deemed ineligible to receive Title IV funds. An academic progress warning period extends to the next scheduled evaluation. A student can re-establish satisfactory academic progress within the warning period if he/she can achieve a cumulative grade average of theory and practicum grades of no less than 75% and attendance percentage increases to a minimum of 80% of the total clock hours scheduled to be complet-ed. Probation Students who fail to meet minimum requirements for attendance or academic progress after the warning period may be placed on probation and considered to be making satisfactory progress while during the probationary period, if the student appeals the decision and prevails upon appeal prior to being placed on probation Additionally, only students who have the ability to meet the satisfactory progress policy standards by the end of the evaluation period may be placed on probation. The student will be advised in writing on the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the probationary period, the student still has not met both the attendance and academic re-quirements, he/she will be determined as not making satisfactory progress and, if applicable, students will not be deemed eligible to receive Title IV funds. Students may be placed on academic progress probation for the following:

1. The institute evaluates the student’s progress and determines the student fails to achieve satisfactory academic progress within the warning period AND

2. The student appeals the negative progress determination prior to the probationary period AND

3. The institute determines that satisfactory progress standards can be met by the end of the subsequent evaluation period.

4. The institution develops an academic plan for the student that, if followed, will ensure that the student is able to meet the institution’s satisfactory academic progress requirements by a specific point within the maximum timeframe established for the individual student.

Re-Establishment of Satisfactory Academic Progress Students may re-establish satisfactory progress and Title IV aid by meeting minimum attendance and academic requirements by the end of the probationary period. Appeal Procedure If a student is determined to not be making satisfactory progress, the student may appeal the determina-tion. The student must submit a written appeal to the school on the designated form with supporting documentation of the reasons why the determination should be reversed. This information should in-clude what has changed about the student’s situation that will allow them to achieve Satisfactory Aca-demic Progress by the next evaluation point. Appeal documents will be reviewed and a decision will be made and reported to the student within 10 business days. The appeal and decision documents will be retained in the student file. If the student prevails upon appeal, the satisfactory academic progress deter-mination will be reversed.

A student may appeal a negative progress determination for reasons he/she feels were mitigating circum-stances that prevented the student from maintaining Satisfactory Academic Progress, such as death of a family member, illness, injury or other mitigating circumstances. In order for appeals to be considered, previously mitigating circumstances that prevented the student from maintaining Satisfactory Academic Progress must be resolved and possible for the student to achieve satisfactory academic progress by the end of the evaluation period. The appeal will be reviewed by appropriate personnel and a determination will be made. All decisions on appeal are final. If a student is placed on probation, Title IV financial aid eligibility will be re-established. Maximum Time Frame Students must complete their program within the enrollment contract terms. 125% of the contracted pro-gram length is considered the maximum time frame for completion. Maximum time frame terms are cal-culated as follows for students enrolled in a 600 hour Professional Esthetics and Master Esthetics.

Professional Esthetics Full time students –37.5 weeks (750 Hours) Professional Esthetics Part time students –62.5 weeks (750 Hours) Master Esthetics Full time students –37.5 weeks (750 Hours) Master Esthetics Part time students – 62.5 weeks (750 Hours)

Contract end dates and maximum time frame will be extended by an approved leave of absence. The contract period and maximum time frame will be extended by the same number of days taken in the leave of absence.

Extra Instructional Charges If cumulative attendance falls below 80% and the student will exceed the maximum time frame allowed to complete the program, additional tuition rates of $250 a week will be assessed.

Interruptions, Course Incompletes, Withdrawals If enrollment is temporarily interrupted for a Leave of Absence, the student will return to school in the same progress status as prior to the leave of absence. Hours elapsed during a leave of absence will ex-tend the student’s contract period and maximum time frame by the same number of days taken in the leave of absence and will not be included in the student's cumulative attendance percentage calculation. Students who withdraw prior to completion of the course and wish to re-enroll will return in the same satisfactory academic progress status as at the time of withdrawal. Noncredit and Remedial Courses Noncredit and remedial courses do not apply to this institution. Therefore, these items have no effect upon the school's satisfactory progress standards Transfer Hours With regard to Satisfactory Academic Progress, a student’s transfer hours will be counted as both at-tempted and earned hours for the purpose of determining when the allowable maximum time frame has been exhausted. SAP evaluation periods are based on actual contracted hours at the institution. Vaccination Policy The Chrysm Institute of Esthetics does not require vaccinations for admission into our programs. Any-one interested in getting more information about vaccinations should contact their local public health department or consult with their health care provider.

Graduation and Diploma Award Diplomas are awarded and students may graduate upon completion of the following:

Mandatory exit counseling with Financial Aid Office, if applicable Maintain satisfactory academic progress Complete all required hours of their designated program Tuition paid in full prior to graduation unless other arrangements have been approved by the

Director of Education Maintain an average grade on theory exams of no less than 75% Maintain an average grade on practical evaluations of no less than 75% Complete all practical requirements and submit practical requirement sign-off sheets Pass final evaluations which may include a comprehensive theory examination and final

practicum evaluation (final mock board) Perform up to 4 mock state board exams Turn in all required paperwork and supporting documentation

Students are responsible for verifying that all program requirements have been completed prior to sched-uling exit counseling and signing exit paperwork.

Official Withdrawal Procedures For students who withdraw from enrollment will receive a fair and equitable assessment of fees accord-ing to the Refund Policy dictated in the Enrollment Contract. The formal cancellation date will be deter-mined on the earlier of the dates that

Step 1: The student must provide notification. The date of withdrawal is the date of the initial notifi-cation to the school.

Employment/Placement Assistance Students are notified on an ongoing basis of opportunities that are available in the field for which they may apply. Students are emailed job opportunities via their school email accounts. There is no formal career advising or placement services offered by the school. The school will assist graduates in finding suitable employment by posting area employment opportunities and teaching job readiness classes, but placement is not guaranteed.

Refund Policy For applicants who cancel enrollment or students who withdraw from enrollment a fair and equitable settlement will apply. The following policy will apply to all terminations for any reason, by either party, including student decision, course or program cancellation, or school closure. Any monies due the appli-cant or students shall be refunded within 45 days of official cancellation or withdrawal. 1. All fees and payments, except the non-refundable application fee, remitted to the school by a pro-

spective student shall be refunded if the student is not admitted, does not enroll in the school, does not begin the program or course, withdraws prior to the start of the program, or is dismissed prior to the start of the program.

2. The school shall provide a period of at least three business days, excluding weekends and holidays, during which a student applicant may cancel his enrollment without financial obligation other than the nonrefundable application fee.

3. Following the period described in section 2 above, a student applicant (who has applied for admis-sion) may cancel, by written notice, his enrollment at any time prior to the first class day of the ses-sion for which application was made. When cancellation is requested under these circumstances, the school is required to refund all tuition paid by the student less the nonrefundable application fee. A student applicant will be considered a student as of the first day of classes.

4. The date of the institution's determination that the student withdrew should be no later than 14 cal-endar days after the student's last date of attendance as determined by the institution from its attend-ance records. After 14 calendar days, the institution is expected to have determined whether the stu-dent intends to return to classes or to withdraw. In addition, if the student is eventually determined to have withdrawn, the end of the 14-day period begins the timeframe for calculating the refunds. In the event that a written notice is submitted, the effective date of termination shall be the date of the written notice. The school may require that written notice be transmitted via registered or certified mail, or by electronic transmission, or in writing per the Refund Policy in the enrollment contract. The school is required to submit refunds to individuals who have terminated their status as students within 45 days after receipt of a written request or the date the student last attended classes whichev-er is sooner.

5. In the case of a prolonged illness or accident, death in the family, or other special circumstances that make attendance impossible or impractical, a leave of absence may be granted to the student if re-quested in writing by the student or designee. No monetary charges or accumulated absences may be assessed to the student during a leave of absence. A school need not treat a leave of absence as a withdrawal if it is an approved leave of absence.

6. If a student does not resume attendance at the institution on or before the end of an approved leave of absence, the institution must treat the student as a withdrawal and the date that the leave of ab-sence was approved should be considered the last date of attendance for refund purposes.

7. For students who enroll and begin classes but withdraw prior to course completion (after three busi-ness days of signing the contract), the following schedule of tuition earned by the school applies. All refunds are based on scheduled hours:

Fractions of credit for courses completed shall be determined by dividing the total amount of time required to complete the period or the program by the amount of time the student actually spent in the program or the period, or by the number of correspondence course lessons completed, as de-scribed in the contract.

Expenses incurred by students for instructional supplies, tools, activities, library, rentals, service charges, deposits, and all other charges are not required to be considered in tuition refund computa-tions when these expenses have been represented separately to the student in the enrollment contract and catalogue, or other documents, prior to enrollment in the course or program. The school shall adopt and adhere to reasonable policies regarding the handling of these expenses when calculating the refund.

For students receiving Title IV financial aid, unapproved Leaves of Absence immediately trigger a Return to Title IV calculation and students are withdrawn. When a student is withdrawn under these circumstances, the Last Date of Attendance is considered the withdrawal date and depending on the length of the Leave of Absence, the student could find themselves in repayment of their loans imme-diately after.

I understand that in the event of any dispute arising out of this agreement on services hereunder, the courts of the Circuit Court of the City of Virginia Beach have sole and exclusive venue. I further agree that should the school prevail on any dispute regarding this agreement, the school shall be en-titled to recover its attorney’s fees, and if the matter involves collection, 25% of the amount of the outstanding shall be agreed to be reasonable.

We received our original certificate to operate by the State Council of Higher Education for Virginia in March 2005. The institution was ac-credited by NACCAS in September 2012 and approved by the federal Department of Education in March 2013. Academic Support Services and Other Available Services The Chrysm Institute of Esthetics does not currently offer any formal academic support services, hous-ing or career counseling services. Students are apprised of their academic status on an on-going basis and meet with the Director of Education or an instructor approximately every 150 hours.

Access to Records/Release of Information The institution guarantees each student (or parent or guardian if the student is a dependent minor) access to that student's records. Note: Parents of students under the age of 18 who request information regard-ing the student will be permitted to receive this information without explicit consent from the student. For students 18 years of age or older, personal information is not released unless requested by the stu-dent. In the event that a student wishes to release information to a third party, the student may request a Release of Information Authorization form, granting the institution the right to release specified infor-mation for each time the student wants to release information. The student also reserves the right to re-scind the authorization in writing at any time. Records are maintained while the student is enrolled in the student database software as well as a hard copy file. Files are maintained permanently after the student is no longer enrolled.

FERPA Policy and Procedures The Family Educational Rights and Privacy Act (FERPA) afford eligible students certain rights with re-spect to their education records. (An “eligible student” under FERPA is a student who is 18 years of age or older or who attends a postsecondary institution.) These rights include: The right to inspect and review the student's education records within 45 days after the day The

Chrysm Institute of Esthetics receives a request for access. A student should submit to the Financial Aid Office a written request that identifies the record(s) the student wishes to inspect. The school official will make arrangements for access and notify the student of the time and place where the records may be inspected. If the records are not maintained by the school official to whom the re-quest was submitted, that official shall advise the student of the correct official to whom the request should be addressed.

The right to request the amendment of the student’s education records that the student believes is inaccurate, misleading, or otherwise in violation of the student’s privacy rights under FERPA. A student who wishes to ask the school to amend a record should write the school official responsible

for the record, clearly identify the part of the record the student wants changed, and specify why it should be changed. If the school decides not to amend the record as requested, the school will notify the student in writing of the decision and the student’s right to a hearing re-garding the request for amendment. Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing.

The right to provide written consent before the university discloses personally identifiable information (PII) from the student's education records, except to the extent that FERPA authorizes disclosure without con-sent. The school discloses education records without a student’s prior written consent under the FERPA ex-ception for disclosure to school officials with legitimate educational interests. A school official is a person employed by The Chrysm Institute of Esthetics in an administrative, supervisory, academic, research, or sup-port staff position (including law en-forcement unit personnel and health staff); a person serving on the board of trustees; or a student serving on an official committee, such as a disciplinary or grievance com-mittee. A school official also may include a volunteer or contractor outside of The Chrysm Institute of Es-thetics who performs an institutional service of function for which the school would otherwise use its own employees and who is under the direct control of the school with respect to the use and maintenance of PII from education records, such as an attorney, auditor, or collection agent or a student volunteering to assist another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an educa-tion record in order to fulfill his or her professional responsibilities for The Chrysm Institute of Esthetics.

Upon request, the school also discloses education records without consent to offi-cials of another school in which a student seeks or intends to enroll. The school will make a reasonable attempt to notify each student of these disclosures.

See the list below of the disclosures that postsecondary institutions may make without consent. FERPA permits the disclosure of PII from students’ education records, without consent of the student, if the disclosure meets certain conditions found in §99.31 of the FERPA regulations. Except for disclo-sures to school officials, disclosures related to some judicial orders or lawfully issued subpoenas, disclo-sures of directory information, and disclosures to the student, §99.32 of FERPA regulations requires the institution to record the disclosure. Eligible students have a right to inspect and review the record of disclosures. A postsecondary institution may disclose PII from the education records without obtaining prior written consent of the student – To other school officials, including teachers, within The Chrysm Institute of Esthetics whom the

school has determined to have legitimate educational interests. This includes contractors, consult-ants, volunteers, or other parties to whom the school has outsourced institutional services or func-tions, provided that the conditions listed in §99.31(a)(1)(i)(B)(1) - (a)(1)(i)(B)(2) are met. To officials of another school where the student seeks or intends to enroll, or where the student is already enrolled if the disclosure is for purposes related to the student’s enrollment or transfer, sub-ject to the requirements of §99.34. (§99.31(a)(2))

To authorized representatives of the U. S. Comptroller General, the U. S. Attorney General, the U.S. Secretary of Education, or State and local educational authorities, such as a State postsecondary au-thority that is responsible for supervising the university’s State-supported education programs. Grievance Policy and Complaint Resolution Policy If a student has a complaint, please allow your Instructor an opportunity to resolve the problem first. If a student has a disagreement with the Instructor or the Instructor has not resolved a complaint, the student should discuss the matter with the Director, Chris Werne. A student may obtain and submit a grievance form to any of the Chrysm Institute’s staff members. On the form, students have the opportunity to spec-ify the problem in summary, in detail, and then to suggest a solution. Upon submission of the form, the Director of Education, Chris Werne, will review the grievance and if necessary, meet with the student. If the Director determines that there is any action necessary, she will implement changes and/or remedy the situation as best possible remaining within compliance to regulations. Students are not subject to un-fair action/treatment as a result of the initiation of a complaint proceeding. Leave of Absence Policy All requests for leaves of absence must be submitted in advance to the Financial Aid Department in writ-ing and must include the reason for the request, start and end dates of absence, and the student’s signa-ture. At this time, there is no limit to the number of Leaves of Absence a student may take, so long as it does not exceed a total of 180 days in any 12 month period. For students receiving Title IV financial aid, unapproved Leaves of Absence can immediately trigger a Return to Title IV calculation and students are withdrawn. When a student is withdrawn under these circumstances, the Last Date of Attendance is con-sidered the withdrawal date and depending on the length of the Leave of Absence, the student could find themselves in repayment of their loans immediately after . The Chrysm Institute of Esthetics may grant a leave of absence to a student who did not provide the re-quest prior to the leave of absence due to unforeseen circumstances. (ex. Car accident, injuries, death in the family, etc.) The Chrysm Institute of Esthetics will grant an extension in the contract end date and maximum time frame based upon the length of the leave of absence, with no additional charges incurred to the student. This extension must be agreed upon by all parties and signed and dated in an Enrollment Contract Addendum which will be maintained in the student’s file.

Upon completion of the course, you must submit application to the State of Virginia to apply to take both the Theory and Practical exams. You must pass these exams in order to receive your license and perform these services in the State of Virginia. The school will assist each student and guide them through the instructions and applications for the State Board Examinations. Physical Requirements of the Industry and Nature of Employment: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is required to stand; walk; sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms.; have the ability to provide legible communication to clients. Industry Standards: Commonly expected responsibilities of an esthetician include but are not lim-ited to; performing skin care treatments within the scope of practice.; possessing communication skills, maintaining a clean and professional personal image, as well as a clean and professional salon image; promoting and selling skin care products; working designated shifts as scheduled and informing the em-ployer ahead of time when unable to work scheduled shifts; receiving clients; building a clientele base; and following sanitation guidelines per OSHA and State requirements. Oversight Restrictions: The board may deny licensure to any applicant with a record of conviction and may require submission of criminal record.

Attendance Policy Student’s required hours of attendance are delineated in the student’s enrollment contract. It is the student’s responsibility to adhere to and ensure that contracted hours are met weekly. In the event of an absence, hours can be made up, however students may not complete additional hours in advance. With regard to absence, if a student is unable to attend a class, notification must be made to the school by phone. Absences in excess of 14 calendar days will result in withdrawal from enrollment. In the event of withdrawal, tuition balance owed and/or any refund due will be calculated per the Refund Policy signed by the student in the enrollment contract. Extended absences may qualify for Leave of Absence approval. All requests for a Leave of Absence must be submitted in writing to the Financial Aid Office in advance and must include the reason for request, the start and approximate end dates of absence, and the student signature. A request for a Leave of Absence may be rejected if required information is missing or the excuse is deemed invalid. Please refer to the Leave of Absence Policy in the student catalog and at the http://www.chrysm.org website. Tuition must be current. If a student’s tuition is in arrears, attendance is prohibited until tuition is made cur-rent. The CFO will give out monthly statements showing amounts and date due to cash-paying students. If tuition is not paid by the due date, students will not be permitted to attend classes until tuition is made cur-rent.

Hours Policy Hour requirements and completion must fall within your contract terms. For example, if you are in a part time, 12 hour per week program, you may not complete more than 12 hours per week. You may make up missed hours, but you may not go ahead in hours. Per the Department of Education, we are required to teach your entire contract length. You are not allowed to graduate early. This policy applies to all students, what we do for one, we do for all. There is no benefit then, in trying to bank hours and move ahead. Excessive violation of this policy could result in revocation of your student badge and clocking privileges until your actual hours are equivalent to your scheduled hours. You cannot graduate earlier than your contracted end date.

Codes of Practice Dress Code Students should always present a professional image. Professional students are required to wear white scrubs that can be found at any uniform shop and Master students may wear scrubs of any color. Student uniforms must always be clean. Long hair must be worn pulled back and fastened and not able to fall forward when you bend. Fingernails must be moderately short and well groomed. White tennis shoes or flats are best. White laboratory coats are recommended but not required. Tardiness If students are more than 15 minutes late to class, they will not be admitted entry into the theory class-room and will not be permitted to clock in for the remainder of the theory hour. Students may not per-form practical during their theory hour. Students are expected to be on time to class and in the event they miss theory due to tardiness, students are still expected to maintain their contracted weekly hours and make up the time they missed. Instructors may grant entry to students more than 15 minutes late on a case by case basis. Models for Practice All students are required to be models for make-up, facial treatments and waxing unless there is a medi-cal reason. You may invite your friends or family to receive student services at the current student pric-ing. All students may also perform practicals on the public . Professionalism All smoking must be done in designated areas outside the building. Breath sprays or mints must always be used instead of chewing gum. Gossiping or negative opinions about any of the students or staff will not be tolerated.

Termination The refund policy applies to all terminations for any reason. If a student maintains unsatisfactory grades or academic progress, they may be dismissed from the program. The school may immediately terminate a student’s enrollment for noncompliance with General Policies; the signed enrollment contract; State Laws and regulations; Improper conduct; Any negative/disrespectful comments about the school, staff, or other students; Behavior that is considered disruptive or any actions which could cause bodily harm to a client, a student, or employee of the school, injury to the school’s reputation ; willful destruction of school property; theft/any illegal act.

Student Rights, Responsibilities, and Privileges Per the terms of the enrollment contract, the student:

Agrees to pay applicable school and state fees and provide all required registration paper-work in a timely manner.

Agrees to comply with all Standards of Conduct, General Policies, State Laws and Regula-tions, and educational requirements including clinic assignments.

Agrees not to refuse to perform client services or other program requirements. Agrees to provide all financial aid documents, if applicable, in the designated time frame. Agrees to comply with the school’s dress code at all times and project a professional image. Agrees to comply with the assigned schedule for the applicable program of study which may

change from time to time at the discretion of the school. Agrees to attend theory class as scheduled for the duration of the course of study regardless

of whether all required tests have been taken and passed. Understands that minimum attendance and grade requirements must be maintained for satis-

factory progress; failure to comply will result in possible probation or termination from the program loss of eligibility according to the policy found in the catalog.

Understands that he/she is responsible for the state licensing exam fee and other examina-tion or licensing related expenses.

Understands and agrees that he/she is responsible for reporting to the school, state board examination grades, progress, licensure date as well as employment information.

Evaluating Program Success The Chrysm Institute of Esthetics requests feedback from current students and graduates to better im-prove programs offered. The Chrysm Institute of Esthetics meets with its Advisory Committee and cre-ates an Institutional Plan annually based on feedback from current students, graduates, and the Advisory Committee.

Appeals Process If a student is determined to not be making satisfactory academic progress, the student may appeal the determination. The student must submit a written appeal to the school with supporting documentation of the reasons why the determination should be reversed, on the designated form available from the Finan-cial Aid Office. This information should include what has changed about the student’s situation that will allow them to achieve satisfactory academic progress by the next evaluation point. Appeal documents will be reviewed by the Director of Education and a decision will be made and re-ported to the student within 10 business days. The appeal and decision documents will be retained in the student file. If the student prevails upon appeal, the satisfactory academic progress determination will be reversed and federal financial aid will be reinstated, if applicable. Campus Security Information is individually emailed to each student’s email annually and is disseminat-ed to potential and current employees upon hire. 2015 Cost of Attendance

Financial Aid Description The Chrysm Institute of Esthetics offers Pell Grant, Direct Subsidized Loans, and Direct Unsubsidized Loans to enrolled students who qualify. Pell Grant is need-based gift aid that does not require repay-ment. Direct Subsidized Loans are also calculated on a need-based basis, but require repayment. Direct Unsubsidized Loans are non-need based aid and are loans for which all students may qualify. The De-partment of Education is the source of these funds. In order to continue participation in the financial aid program, students must maintain a 75% academic grade average and 80% attendance average.

Methods and Frequency of Disbursement of Financial Aid Upon deposit of funds from the Department of Education, disbursements are applied to student accounts. Generally for Professional Esthetics students, disbursements are made in two payments. The first pay-ment is at the beginning of the program. The second at mid-term. Note: the second disbursement can only be released to the student is the student has successfully completed half the number of weeks in the program, 300 hours, and be maintaining satisfactory academic progress. The disbursement dates listed on the award letter are approximate and subject to change. Any monies due the student will be paid by check directly to the student within 14 days of disbursement from the Department of Education once the student has a zero balance with the school. For Master Esthetics students and students who took out financial aid at another institution in the same award year: amount of aid available, frequency of disbursements, and disbursement hour/weeks require-ments for financial aid varies from student to student due to overlap periods and 150% Direct Subsidized Loan Limits provisions.

Note: Some students may be counted twice as there are students who were already enrolled, began classes, and/or graduated Professional Esthetics and continued their education into Master Esthetics within the same year.
