1. Puja bonus ~500/- is sanction for sl ip worker f rom PHC unt ied fund .{action : AAA).

2. Wages of the Driver of M at ai PHC increased from~ 35001- to ~ 45001- per m onth. I.e.

~150 /- per day. And it is approved from 1st SEPTEMBER 2016.{Action : AAA) .

3. It has been decided that, t he slip worker wages wi ll be given in the following -a. Laxman Tripura ~ 1 50 /- per day. {~4500/- per month).

b. Bakul Deb Nath ~ 150 /- per day. (~4500/- per month).

c. Gouri Das ~ 116.66 per day (~3500/- per month) .

d. Ka lpana Ma lia ~ 116.66 per day (~3500/- per month) . And the wages st ructure w il l be effect from 1st september 2016. {Action MOIC)

4. Expenditure m ade on 19/05/2016 i s~ 7 3 1 /-is ap proved.(AAA)

each month and meeting of GOVERNING BODY wi ll be held every 3 month.

1. Sl ip worker payment to be simplified wit h discussion with all the MOICs of Hrishyamukh

RD block and SDMO Belonia. 2. Information to the CDPO regarding irregularity and misbehavior of AWW of Dronapara

AWC as report ed by loca l PRJ bodies. 3. Inauguration of newly built Doct ors and staff quarter is pending due to new electric

connect ion and source of wa t er supply. Informat ion to be given to the concerning

au thori ty by MOIC Mat ai PHC as early as possible. 4. M aintenance of internal electric connect ion various pa rt of the PHC wi ll be done by Mr

Di lfp Biswas as urgent basis within 1 month.

5. Maintenance of water pipe lines in wards and delivery room and cleaning of all the water tanks will be done by Mr Jiten Pa tari within 15 days.

At the outset of the meeting , the Medical Officer In-Charge, Matai PHC, Belonia, South Tripura, welcomed

the all members After discuss ions on various isst.:e, t he following decisions were taken:-

1. For regular payment of slip worker wages , communicate will be made by MOIC, Matai PHC.

2. Wa ter pipe line work done emergency basis. Bill is sa nctioned after deduction of Vat as per

voucher.

3. For smooth funct ioning of the PHC, MPS, AYUSH Doctor, Homeo Doctor, LDC is require, action will

be taken by MOIC, through submitting lett er to higher authority.

4. Meeting refreshment bi ll is approved as per voucher .

5. Gas woven repair is approved. And cost will be given from RKS Fundvertelepone.

6. Those members were not at tend the meeting MOIC, will contact over telephone for attending the

next schedule meeting.

After detailed discuss ion the following dec is ion ''ere made. whi ch are as fol io\\ s

1. To contact with AAA of Hrishyamukh CHC regarding wages of slip workers which is pending since JULY 2016 till date.

2. 5 nd' of labours will be engaged on 27.12 .16 and 28.10.16 for mass cleaning of all the rooms and corridors of 151 floor of MAT AI PHC by MAT AI gram panchayet.

3. Information and co-operation of all the PRI bodies during J.E vaccination campaign.

4. Application for human resources e.g. MPS,LAB TECH,AYUSH M.O. to the higher authority.

5. One way payment will be paid to the pregnant mother under JSSK and after discharge from

hospital AMBULANCE of Matai PHC will accompany her to home. 6. As solar plant has been installed the emergency line from matai bazaa r wi ll be discontinued

from 1.1.17. Generator bill will be paid from RKS up to the month of December 2016.

7. Approximate Rs 5000/- will be spent for the purpose of fire extinguisher in front of kitchen

room.

8. The work of gardening in front of Hospital building is started and maintenance cost may be used form RKS fund if available at the end.
