This form is identical to the form that ITI sends out from the corporate office when you request a clinic to be set up for your employee(s). This new, two page form contains ALL of the vital information to help ensure that a clinic properly completes whatever testing is necessary. The form includes your employee’s name, your company’s name and account number(s), all pertinent billing information, as well as the specific type of testing that will be required for your employee. The Clinic Authorization Form also includes the laboratory information, MRO information, and how to handle ALL of the paperwork associated with the collection—as well as ITI’s contact information for any questions or concerns that might arise during the collection process. Please be certain to retain a copy of this letter and the new protocol sheet in Section D of your DOT Drug & Alcohol Compliance Binder. If you do not have binder or are in need of an updated one, please contact ITI so that one can be provided to you. Personalized copies of the Clinic Authorization Form and protocol sheet can be made available upon request from ITI’s Customer Service Department. Please send all requests to [email protected] or contact your Customer Service Analyst at 800-295-2587! ITI’s Customer Service Department sends out reminder letters to all DER’s 24-48 hours prior to a scheduled appointment, and we ask that you please respond with either your confirmation or cancella- tion. If our collectors show up and are turned away, you will be charged no less than a trip fee for our time and travel with the possibility of other fees as well! So be certain to confirm your appointments as soon as you receive the email or give us a call if you need to cancel and reschedule!

**Customer Service Department Members**

ITI is implementing a new Clinic Authorization Form to help alleviate the stress and confusion of

sending your employees to area clinics. This form is identical to the form that ITI sends out from the

corporate office when you request a clinic to be set up for your employee(s). This new, two page form

contains ALL of the vital information to help ensure that a clinic properly completes whatever testing

is necessary.

The form includes your employee’s name, your company’s name and account number(s), all pertinent

billing information, as well as the specific type of testing that will be required for your employee. The

Clinic Authorization Form also includes the laboratory information, MRO information, and how to

handle ALL of the paperwork associated with the collection—as well as ITI’s contact information for
