WHEREAS, the ASSIGNOR is willing assign in favor of the ASSIGNEE by way of pledge orcollateral security to secure the prompt and full payment said loan/s, cash advance/s and other lreditaccommodation/s granted by the ASSIGNEE to the ASSIGNOR, as well as those that the ASSIGNEEmay hereafter grant or extend to the ASSIGNOR, the following:

1. That the sald ASSIGNOR shall remain liable for any outstanding batance of theaforcmentioned oblig_ations, existing and those he/she may hereafter begranted, if the ASSIGI{EE is unable to actuafly receive the assigneJsalaries/sum/s for any other cause whatsoever;

2. That ln case ASSIGNEE fails to received his/her satary by reason of dismissat,admlnlstrative charge, suspension or any other iause resulting in th6withholdlng or fofeiture of salary/benefits, then the ASSTGNEE resirves therlght to collect from AssrGNoR real propefi/s, ail sum/s or money or cashdeposlts

3. THAT SAID ATM CARD SHALT NEITHER BE SOUGHT TO BE REPLACED NORAFFIDAVIT OF LOSS OF T}IE SAID ATM CARD SHALT BE EXECUTED BY THEASSIGI{OR TO THE DRAWEE/DEPOSITORY BANK WITHOUT THE EXPRESS

COI{SENT OF THE ASSIGNEE. THAT FAILURE ON THE PART OF THE ASSIGNORTO SECURE EXPRESS CONSEI{T FROM THE ASSIGNEE IN REPI.ACING SAID ATMCARD OR IN EXECUTING SAID AFFIDAVIT OF IOSS OF ATM CARD, WHILE THISCONTRACT IS STITL ENFORCEABL4 SHALL RENDER THE ASSIGNORCRIMINATLY UABTE OF PERJURY AND SUC}I OTHER CRIMES THAT MAY BEASCRIBED TO IT;
