1) HIRARC is one of the systematic tools that can be used by the industry in managing

risk at work place. Describe the steps involved in HIRARC. Suggest ONE (1) step that you think the most important. Provide your argument.

2) You are working in a construction industry and required by your superior to formulate a

safety and health program at work place for the company.

3) Based on the work environment shown in Figure 1, write ONE (1) class of hazard that exists in the workplace and FIVE (5) possible routes of its entry to the workers. Are there other condition(s) at the work that you suspect may be hazardous but you are not sure about?

4) Figure 2 shows a mechanic at work. Assess the work environment of the mechanic and propose THREE (3) possible classes of hazards and the routes of entry that can affect the health and safety of the worker or environment for each hazard.

5) For EACH types of hazards identified above, propose ONE (1) standard control

measure that could be adopted by the manager to safe guard his worker from the hazard.

6) If the manager fails to provide sufficient safety and health control measures at the work place, what would be the ethical and legal implication to the manager and company if the workers were to meets an accident?

7) HIRARC is one of the systematic processes commonly used in risk management at

work place. Describe the meaning of HIRARC and illustrate the process flow of the steps involved in HIRARC.

8) Before hazards can be identified, it is important to classify and select work activities or

jobs of potential dangers to workers. Propose four (4) guidelines that you would adopt to select the work activities or jobs to be analysed – provide your arguement for each of them.

9) The success of occupational safety and health program at work place no longer relies

on technical aspects but have now shifted to organizational and human factors. The approach is focused on the values, attitudes, perception, commitment, pattern of behaviour of individuls, groups and management of the organization and development and promotion of safety culture. As a safety practioner, how would you evaluate the practice of safety culture in an organization.

10) In occupational safety and health program, risk can be assessed by qualitatively and quantitatively method. Compare and differentiate the two (2) approaches.

11) Selecting control measures in risk management is very critical as it has financial,

operational and adminstrative implications to the organization. Elimination or inactivation is one of the control measures at your disposal. Propose two (2) possible strategies that you could used to eliminate hazard at the work place and assess the possible drawbacks and ramification of such measures to the organization.

12) Hazards are classified in many different ways. But Physical Hazard and Chemical Hazard are two (2) most commmonly listed classifications in hazard indentification. Between the two hazards, which one would you rate or judge the most dangerous. Provide your arguement.

13) “A roof structure for newly handed over five stories office building partially collapsed and caused damages to the particular area and fatalities.”

a) Cost of damages can be categorized into two (2):

i) Direct ii) Indirect. Measures the indirect costs involved in the above accident and explain each of them.

b) Defend how a good safety and health procedures and practices can avoid this

accident and promoting a safe working culture especially in construction industry.

14) Safety is defined as a matter of relative protection from exposure to hazards or

freedom from hazard. Identify and predict the hazard potential based on the above

illustration. Give your opinion.

15) In Safety Management System, the risk assessment process will include:

i) Identification of hazard

ii) Controlling the risk

iii) Check and review

As an engineer, explain how we can control the risks to ensure safety in our living and

working area.

16) In managing risk at work place, the words hazard, danger and risk are used interchangeably in different contexts. Based on your understanding, differentiate between hazard and risk in a workplace.

17) One of the major benefits of OSH program at workplace is reducing accidents and health hazards. i) As an engineer, elaborate its importance from the ethical and moral values?

ii) Predict the financial impact from accidents and health hazards to your company

or organization.

18) As a Safety Engineer, assess the harmful acts and risk factors that can be found in the activity and the possible safety issues.

19) Propose the possible remedy that can be adopted during the activity to minimize the

hazards.

20) Explain: “A Registered Engineer shall at all times hold paramount the safety, health and welfare of the public”
