Dog description - colour - weight - sex - age (puppy, juvenile, adult, geriatric) - entire or already desexed - microchip number (not applicable at the moment) 4. Status (choose one): i) owned ii) semi-owned iii) street dog iv) feral/wild 5. Adult Females: Pregnant: i) yes, If yes number of foetuses ii) no

Body Condition Score Adults (choose one): i) underweight (ribs/spine/hips obviously visible, tucked abdomen, little or no body fat)) ii) good weight (ribs palpable but not visible, some body fat, but waist obvious) iii) overweight (ribs barely palpable, little or no waist, flat dorsum) Body Condition Score Puppies (< 6 months) and lactating females (choose one): i) skinny ii) not skinny
