Application Process QUESTIONNAIRE AND COVER LETTER Students must submit the following three items to apply.

1. Contact information (name, email, phonenumber, and school)

2. PDF of the Completed Questionnaire3. One-page “practice” cover letter, applying for

their ideal energy and minerals career

SCHOLARSHIP DISBURSEMENT AND REPORTING Scholarship recipients will be formally announced at an official award ceremony during the summer/fall of 2020. Attendance is mandatory for scholarship recipients. To obtain the scholarship, recipients must provide OED with proof of enrollment from their selected university or technical college and OED will transfer the funds through the recipient sub-account to the selected institution.

• University scholarships will be provided intwo disbursements of funds.

• Technical college scholarships will beprovided as a one-time disbursement.

Scholarships will be deposited in the Utah Educational Savings Plan program, also known as a 529 plan, in an OED master account. Within the first 60 days of the scholarship offer, recipients must:

• Indicate which Utah university or technicalcollege they plan to attend and what subject they plan to study

• Complete the required Form 106 ScholarshipAccount Agreement (provided by OED) toestablish a sub-account

Scholarship recipients may defer spending of their scholarship for up to 2.5 years or 30 months. Failure to use scholarships within 2.5 years or 30 months will result in a cancellation of the scholarship and refund to OED.

SUBMISSION Applications shall be submitted via email to Kevin Brooks of the Governor’s Office of Energy Development (OED) ([email protected]) by June 15, 2020.

ELIGIBILITY Eligible applicants must currently be a 12th grade student enrolled at any public or private high school in Utah, or a first-year student pursuing a STEM undergraduate degree or energy-related technical certification at a Utah institute of higher learning.

Students must certify their intent to pursue a STEM course of study at a public or private Utah institution of higher learning or a license, certificate, or skill training that supports the energy industry at a school that is part of the Utah System of Technical Colleges. Previous winners of the scholarship are NOT eligible to apply.

NOTIFICATION Students will be notified within 30 days of the application deadline if they were selected to receive a scholarship. Scholarship recipients must agree to the requirements and conditions of the scholarship.
