Annual Report to the Community

66 Years of Service to Our Community

After almost nine years here on the Central Coast, I am proud that we have succeeded in achieving the VNA & Hospice’s goals of maintaining our status as the premier provider of home health and hospice care for our communities. Our integrated programs deliver real services to our citizens. Continuum of care remains at the forefront of our minds. Our highest priority is to allow our patients to be independent. We are committed to helping our clients to make their own decisions and achieve their own personal goals throughout the length of their lives.

With these thoughts in mind, we remain committed to our six decade old mission statement:

“The Visiting Nurse Association & Hospice is dedicated to providing the highest quality health care to residents of the Central Coast by meeting their individual needs in an ethical, effective, caring and fiscally responsible manner.”

This past year, we have achieved truly remarkable goals. We have continued and expanded our home palliative care program throughout our 5000 square mile service area on the Central Coast. In conjunction with this program commitment, the VNA solidified Central Coast Alliance for Health and Blue Shield palliative program specific contracts to guarantee these services to our targeted clients. We also successfully launched and integrated our Palliative Care Team, and we began the process of duplicating our successful palliative model for our Cardiac program.

We have more good news as well. Of significant importance is the fact that we have achieved and maintained a 4.5 rating for two consecutive years for our Home Health Quality of Patient Care evaluation. We also welcomed three new members to our Board of Directors. And, we added nine nurses who are trained in Certified Wound and Ostomy treatment skills.

Our success cut across all programs and services provided by the VNA & Hospice. Whether it has been in the area of personnel and administrative enhancement, or increased needed services in our Home Health & Hospice programs, or significant accomplishments by our successful Business Development and our important Fund Development divisions, our sustained and committed advancement in health care and hospice services is unchallenged.

Beyond these achievements, we provide our life-enhancing services wherever our clients need us. And we are striving to expand our services to every area of Monterey and San Benito Counties, as well as the southern ends of Santa Cruz and Santa Clara Counties.

Together, with clinical staff, operational staff, volunteers, donors and the patients/families that we serve, VNA & Hospice makes a difference in our community.

All board members and committees serve without compensation and all graciously assisted with fundraising events and other VNA activities.

Dearest Donors:

I am always struck by the generosity and compassion of our area residents. Whether it is to support families with children entering the 7th grade who need their state-mandated “Tdap” immunization, or providing respite care for VNA patients with Alzheimer's disease, or supporting our Hospice Team which cares for end of life patients…our community has always been there for the VNA & Hospice. I offer our heartfelt thanks to our generous donors and passionate benefactors for their significant contributions and outstanding efforts so that the Visiting Nurse Association & Hospice may continue to provide high-quality home health, hospice and community services to those who need it most. Clearly, this is a testimony to our donors’ belief in our critical role in sustaining the health of our communities.

Our challenges continue to be financial uncertainties in the area of healthcare reimbursement payments. We also face challenges with our rapidly growing senior population. Without you, our donors, we could not provide compassionate care to our patients, your neighbors and friends. Our staff and volunteers work diligently to care for patients, while our loyal and steadfast donors, with their financial support, help to maintain our excellence.

An unrestricted gift or bequest intended for the general purposes of the VNA & Hospice, one which the VNA & Hospice is free to use either to establish an endowment fund or to expend in whole or in part as it deems best, might read simply:

"I hereby give (give, devise and bequeath) to The Central Coast VNA & Hospice, a California non-profit 501(c)3 public benefit corporation, Tax id 94-1205572 (. . .percent of my estate or percentage of my residuary estate) for the general purposes of the VNA & Hospice as the VNA & Hospice may determine."

André Edwards, RN, BSN, MSN, NP is the Clinical Coordinator for Specialty Programs for the VNA Home Health. André is the lead for the VNA Cardiac Care program where the care is delivered by a specialized clinical team for people living with cardiac diagnosis. André became interested in cardiology after his mother had a heart attack, therefore has a drive for the program to focus on relief of symptoms and stress related to cardiac diagnoses. The goal is to improve the quality of life for the patient and family by educating, demonstrating, and addressing physical and emotional needs. This is why it is natural to see André walking all over Monterey County; he also enjoys riding his bike and traveling. André loves spending time with his wife, Toni creating meals at home that are tasty and nutritious. André’s true desire is to provide a positive healing experience by building trusting relationships between patients, the community and health care providers.


For the past year and a half, your team provided compassionate care in the last phases of my mother’s life so she could live at home

as fully and comfortably as possible. Your care was family-centered and your team kept in contact with our family when decisions had to be made. We were impressed by the quality care that VNA Hospice provided and how your nurses arrived in a timely manner when they were called. They and your other providers were always professional, kind and exceptional. –Vernon Fernandez

Nadereh Nasseri, RN, BSN is the Manager of Care Coordination for the VNA & Hospice. She works with discharge planners of hospitals to provide appropriate home health care for patients leaving for home from the hospital. She oversees strategical development by leading the VNA Clinical Marketing team to drive program awareness and community wide relationships for VNA Hospice, Home Health including specialty programs, and Community Services.

Nadereh also has a BS in Business Administration and has worked for over 27 years in nursing, nursing administration, staff development and education in the field of home health, palliative and hospice care. Nadereh left her birth country of Iran to attend University of Kansas just five hours before the Islamic Revolution began in Iran. She loves photography and all the photos of the behind the scenes were taken by her. She is very artistic and loves the outdoors where she spends time with her adult children and grandson. She attributes herself with a great sense of humor and is often heard to say, “Life is too short, hurry up”!


I can’t even begin to express my gratitude for ALL you did for my mom to keep her pain free, comfortable, and low anxiety. You were able to give her the gift

of comfort which is all we can ask with her disease. Thank you to all the staff and volunteers who cared and understood who mom was going through and what the family needed too! THANK YOU! –Judy Wills

Anybody, anything born in this world, sooner or later has to be at the end of their life. Our son passed away peacefully on VNA Hospice service. He

had no pain, just like he went to sleep. We thank you so much on how VNA took care of him so nicely and with dignity. –The Kyutoku Family

Claudia Renteria is the Assistant Care Coordinator for VNA & Hospice. Claudia’s overall role is to support the structure of the palliative and business development teams. She is a native of Salinas, so naturally she is an asset to our Latino outreach program development and when needed face to face translation support. She is an expert in understanding our home health and hospice programs. You can hear Claudia’s infectious laugh though out the building. Claudia loves to read and is always creating new adventures for her children on a budget. Claudia believes in the Dr. Seuss quote, “The more that you read, the more things you will know; the more you learn, the more places you go!” She once purchased local Disney products including backpacks and sent them ahead, to the hotel, so when her family arrived they were on the beds for each child with everything you would want for a grand adventure in Disneyland.

Commendation to the VNA Medical Social Worker who’s excellent assessment skills and intervention saved a patient from severe elder abuse and the threat of

Rebecca Young, RN is the Patient Care Planning Supervisor for VNA Home Health and Hospice. Working with a team of schedulers and interfacing with physicians, hospitals, and other external referral sources, it is Becky’s expertise in case management and utilization management that makes her an asset in this role. She has extensive experience in insurance and Medicare Advantage Plans from her previous positions in the Midwest, where she grew up. Becky started her career as a visiting nurse in hospice, as a licensed vocational nurse. She received her RN degree and began leadership roles in the industry. When not supervising the scheduling of admissions and patient home visits; Becky is intimately involved in her young children’s lives and her love of animals. It is not unusual to see her with her three rescue pugs at her children’s events.


I think the home care was really good. My mom’s progress is very limited but that’s because of her condition. Overall, all the physical therapists

were really good. Specifically, Dan was awesome. –Anonymous

Everybody was great. Well everybody has come in here and just treated me like I was part of their family. Very friendly, loved to visit while they took care of my needs.

All the nurses and all the health aides that came over were very patient, very knowledgeable, and always willing to help to go above and beyond what they
