Scheduling Appointments1. Log-on to www.CareHere.com and click on “members only.”2. Enter your username and password.(click forgot username/password if you don’t remember)3. Click on “Appointments SS” in the green navigation bar on the left-hand side of the screen.4. Click “Get Appointments” to view all available appointments today without making any preference selections.

Or You can narrow your appointment options based on your preferencesWhen: Choose a date range and times for when you would like to be seen.What: Choose what type of appointment you need.Where: Choose where you would like to schedule your appointment.Who: Choose who you would like to see for your appointment based on your other appointment preferences.

9. Click “Get Appointments” based on your selected appointment preferences. (You can also change the drop down beside“Get Appointments” to view the first available appointment based on your preference selections.)

First-time User?Register by going to www.CareHere.com and click “member login,” “Click I need to register for the first time with my Access Code.” Beside “first time registration,” enter access code: PCFSD7. Click “go.” Review the consent form and if you agree, check “I agree.” For identifications, you will need to enter your social security number and birth date, create a username and password, and enter your email address. On the contact page, review all of your information for accuracy. Complete the health page, skipping any field for which you don’t know the answer. A confirmation email will be sent to the email address you provided. You may now log in as a member by clicking the “go to login page” button. If you do not have an email address, please call CareHere at 877-423-1330 for scheduling assistance.

Tobacco CessationAs of July 1st, 2016 all District work sites became tobacco free. We know this can create a challenge for tobacco users. Therefore, weare offering resources to help you ease into this process. Please visit for up-to-date information on the tobacco cessation programsavailable through CareHere and Tobacco Free Florida:

Resources available to help you prepare for a Tobacco Free Workplace:

1. Eight (8) week Tobacco Cessation class conducted by CareHere Tobacco Cessation Health Coach. These classes consist of onehour meetings once a week, which are designed to support participants into becoming tobacco free.

a. Employees covered under the District’s group medical plan, who have been approved by a CareHere provider for medication,will have access to Chantix for $35 copay while working through the program.

b. Employees who are not covered may work with their primary care physician (PCP) to receive Chantix. The cost of Chantixmay vary depending on the individual’s outside coverage. A $75 off coupon will be provided for these employees.

c. Participants are monitored closely by CareHere. Classes are offered onsite or online if you cannot commit to a once-a-weekmeeting. Online classes are only available to employees insured under Pasco County Schools group health plan.

2. Tobacco Free Florida offers resources to all employees of Pasco County Schools. In addition to online and toll-free counseling,Tobacco Free Florida offers free face-to-face classes through the Florida Area Health Education Centers (AHEC). AHEC hoststwo (2) onsite classes; one 2-hour session or a six-week group session. AHEC also provides participants with one month freeNicotine Replacement Therapy (gum, lozenges, and patches) for those who enroll in one of the two programs.
