With the Wellness Rewards Program, you can earn up to $300 when you take healthy actions that lead to healthy results. Beginning October 10, 2011, take just a few simple steps toward a healthier life and you can be on your way toward feeling better and reducing what you pay for health care. In addition to our 2011 focus on achieving a healthy body mass index (BMI), this year we are also rewarding healthy blood pressure. Both of these support heart health—and not only is a healthy heart one of the main components of good overall health, but heart disease and stroke can have a significant long-term effect on your family and lifestyle and, most importantly, is preventable. YOUR GUIDE TO EARNING Wellness Rewards Why Wellness? Disney is committed to the health and well-being of our employees and cast members. Supporting you in your goals to improve your health is one way we can help you reduce or manage health risks that often lead to disease or chronic conditions. Improving your health can save you and the Company money, because when you are healthier, you will probably need fewer trips to the doctor and use less expensive health care services. That means less out of your pocket—and Disney’s. The Wellness Rewards Program is designed to help you improve your health—and your bottom line. When we work together to be healthier, we can all share in the rewards. How the Program Works To be eligible for the Wellness Rewards Program, you must first take the WebMD Personal Health Assessment (PHA) and get a biometric screening at an approved location. Take the WebMD Personal Health Assessment. The PHA is a confidential assessment which helps you to understand your current health risks and what you can do to be healthier. (See page 3.) Get your biometric screening. Personal health statistics—such as blood pressure, cholesterol, blood glucose and body mass index (BMI)—are good numbers to know to understand your general level of health. To earn a reward in 2012, you can get these numbers tested at an approved biometric screening location. Your biometric results will be automatically uploaded into your PHA and you will earn your first $100 reward. This year, you must complete the PHA AND get a biometric screening to earn the reward. (See page 3.) Attain or make progress toward a health Body Mass Index (BMI). Body Mass Index (BMI) is a good indicator of body fat for most people and can help identify if your weight is a risk factor for future health problems, such as heart disease and diabetes. You can earn a $100 BMI reward in 2012 if you attain a healthy BMI or make progress toward a healthier BMI. If you don’t hit the BMI mark at first, take advantage of the many programs and resources available through Disney Healthy Pursuits to help you make progress toward a healthier BMI. What You Can EarnYou must take the PHA and get a biometric screening to be eligible for any rewards under the Wellness Rewards Program.

Remember—you must be enrolled in a 2012 Disney medical option to earn rewards.

How Rewards Are PaidYou can get each $100 reward, paid in full, in January 2012 if you meet the reward requirement by November 23, 2011. If you earn your rewards after November 23 anytime up to September 15, 2012, they will be paid as soon as possible, generally 6–8 weeks after you meet the requirement.

You can elect to receive your rewards (and the rewards your spouse/ partner earns) in one of two ways:

• Included in your paycheck (subject to applicable taxes), or

• As a tax-free deposit to your Health Care Flexible Spending Account (FSA).

Either way, you can use the rewards you earn to help reduce what you pay in health care during the year. Don’t forget—When your spouse/partner participates, you can double the amount you receive!

Earn It How When You Get

PHA plus Biometric Screening Reward

Complete the PHA AND get your blood pressure, cholesterol, blood glucose and BMI tested at an approved screening location and have your results electronically uploaded into your PHA.

October 10, 2011– September 15, 2012 $100

BMI Reward If your BMI is 18.5–24.9, you will immediately qualify for the BMI reward, ORYou can be retested at any time during the year. If you are able to show progress (either achieve a healthy BMI of 18.5–24.9 or reduce your initial BMI by 10% or more), you will qualify for the reward at that time.

October 10, 2011– September 15, 2012 $100

Blood Pressure Reward

If your blood pressure reading is in the healthy range below 140/90, you will immediately qualify for the blood pressure reward, ORYou can be retested at any time during the year. If you achieve a blood pressure in the healthy range (below 140/90), you will qualify for the reward at that time.

Total $300*Do you have an enrolled spouse/same-sex domestic partner? You can earn up to $600 when you participate together.

What’ a Healthy BMI?

Your height and weight are used to calculate your body mass index (BMI). A BMI of 18.5–24.9 is considered normal for the average adult, according to guidelines established by the Centers for Disease Control and Prevention (CDC).

How to Access the PHALog in to www.webmdhealth.com/disney—or go to Disney Add It Up! Online from the Quick Links on The Hub or directly at www.DisneyAddItUp.com and click “Take the PHA.”

If you have already registered for the site, you will need your user ID and password. If you are registering for the site the first time:

• Employees: Enter your 8-digit personnel number (PERNR), including any leading zeroes.

• Spouse/partner (not employed by Disney): Enter the employee’s 8-digit PERNR (including leading zeroes), followed by your birthdate in MMDDYYYY format.

• Disney couples: If you and your spouse/partner both work for Disney, you should register for the site separately, each using your own PERNR.

Please note: Paper versions of the PHA (in multiple languages) may be available in some locations.

Where to Get Your Biometric ScreeningsTo be eligible for a reward, you must get your biometric screenings at one of these approved screening locations.

Where To Go What To Do

Onsite Biometric Screenings

• Get your biometric screening at a Disney Healthy Pursuits wellness fair or other onsite event that may be offered at your location (includes Center for Living Well in FL and Parks & Resorts Health Services locations).

• To find an onsite screening location near you, go to Disney Add It Up! Online from the Quick Link on The Hub or directly at www.DisneyAddItUp.com.

• Your results will be uploaded into your PHA.

Doctor’s Office • Go to Disney Add It Up! Online to download the “Physician Biometric Screening Results” form to take to your doctor’s office.

• Your doctor must complete the form with your test results and fax to the confidential number included on the form.

• Your results will be uploaded into your PHA.

Convenience Care Clinic

• If you’re enrolled in a CIGNA medical option, you can get your initial biometric screening at a convenience care clinic in the CIGNA network. To find a clinic near you, go to www.cigna.com. Click on “Provider Directory” under the “Customer Care” tab and select the “Convenience Care Clinic” option.

• For your initial screening: Go to Disney Add It Up! Online to print the “Physician Biometric Screening Results” form to take to the clinic. (Convenience care clinics are not available for retests.)

• The clinic provider must complete the form and fax it to the confidential number on the form.

• Your results will be uploaded into your PHA.

Quest Diagnostics Patient Service Center

• Go to https://my.blueprintforwellness.com (enter registration key “Disney”for your initial screening or “Disneyretest” for a retest) and schedule an appointment at a location near you.

• Your results will be uploaded into your PHA.

IMPORTANT: You will need to know your PERNR for your screening. You can find your PERNR on your Company ID card, your paystub or the “Personal Tab” of The Hub.

For More Information

Exceptions: Due to an individual’s particular medical or health situation, an alternative may be considered for certain requirements to earn the results-based rewards. Under these circumstances, your doctor will need to complete and submit a “Physician Request for Program Alternative” form (available at www.disneyadditup.com) certifying that a program requirement would be inaccurate, inappropriate or unattainable for the individual. Consideration will be given on an individual basis as to whether a reasonable alternative should be provided. The Company will also modify or waive program requirements when necessary to comply with applicable federal, state and local law.

This document highlights provisions of and changes to the Wellness Rewards Program as of October 10, 2011. You may or may not be eligible to participate in this program, depending on job status, work location or applicable collective bargaining agreement, as well as whether you are enrolled in a Disney medical option. For a complete description of the program, refer to the detailed program guide at www.disneyadditup.com. The Company reserves the right to amend or terminate this program at any time.

To confirm your rewards, go to www.webmdhealth.com/disney and check the handy status box on the Home page to see which rewards you have earned and which are still to be met.

Eligibility: All full-time, U.S.-based (and U.S. expat) employees, cast members and their spouses/same-sex domestic partners who are enrolled in a 2012 Disney medical option are eligible to participate in the Wellness Rewards Program. Completion of the PHA and a biometric screening is required in order to receive rewards under the Wellness Rewards Program. Employees and cast members must first complete the PHA in order for their enrolled spouses/partners to be eligible.

We Take Your Privacy Seriously

Any personal health information you provide will remain confidential and will be handled by WebMD and other designated health care partners according to their strict privacy guidelines. Our health care plan vendors will use this information solely to identify programs and resources that may help you achieve your health goals.

Disney will only receive aggregate data (no individual results) to help us guide our health care plan vendors to better understand the overall health needs of our plan participants. No personally identifiable information will be provided to Disney at any time.
