The shoulder• A region that connects neck, trunk and upper limb to place hand in space
• Enjoys greater freedom of motion
• Relies on muscles and ligaments for stability
• To achieve its function, shoulder built up on 3 bones and 4 joints
Use of shoulder in sports
Power ,speed and drive for Throwing, pitching, tennis stroke, spiking, gymnastics , weight lifting, swimming etc.
Shoulder acceleration and deceleration are demanding actions in overhead throw
Overuse and excessive load—rotator cuff failure, proximal head migration and instability

Scapular muscles and movement
• 2 functions—provide stability of scapula when shoulder complex is loaded.
• -movement of scapula –pointing itself under the humeral head to maintain proper lever length and functional length of musls.
• Provides the fulcrum to elevate the arm in all planes
• Scapular motion by a force couple on a moving platform—the glenoid

Role of rotator cuff and capsular ligaments
• GH joint lacks bony stability as in hip
• Flat glenoid, no stability for a large head
• Only little contact, yet great stability
• Due to 2 factors-ability of scapula to rotate and support humeral head
• Efficiency of soft tissues enveloping head.
• Contraction of this group maintains the H head in constant contact with glenoid.
Stability of GH joint• Atmospheric pressure• Glenoid fossa and labrum—chock effect
• Glenohumeral ligaments and long head of biceps attached to labrum
• Inferior GH ligament complex like a hammock supporting head in abd and ext. rotation
• Middle GH ligament acts like a check rein to prevent posterior translation
Stability of GH joint ctd
• Superior GH ligament restricts inferior translation
• Collectively the capsular ligaments and labrum are static stabilizers
• Dynamic stability by rotator cuff and long head of biceps
• A thrower relies on dynamic effect of rotator cuff for joint compression to avoid capsule stretching
Glenoid labrum and tears
• Common in throwers• Large compression and shearing forces drive the head anteriorly and posteriorly
• Creating traumatic cartilage injury and labral tear
• Three types of surface motion at GH joint
• Rotation, rolling and translation (gliding)
Subacromial space and bursa
• Subacromial space is inferior to acromion, A-C joint, and coracoacromial ligament
• Subacromial bursa lies in the sub acrom. space, between the acromion process and the coracoacromial ligament above and the GH joint below
• The bursa cushions the rotator cuff
• from the overlying acromion
Ditinjau dari aspek gerak maka sendi bahu dapat dibagi menjadi dua, yaitu gerak secara
osteokinematika dan arthrokinmeatika.
Biomekanika shoulder joint
adalah gerak sendi yang dilihat dari gerak tulangnya saja.Gerakan fleksi• Yaitu gerakan lengan ke depan, ke arah atas mendekati kepala, bergerak pada bidang sagital dan axisnya melalui pusat caput humeri dan tegak lurus bidang sagital.
• Otot penggerak utamanya adalah otot deltoid anterior dan otot supraspinatus dari 0 – 90 derajat,
• sedangkan untuk 90 – 180 derajat di bantu oleh otot pectoralis mayor, otot coracobrachialis, dan otot bicep brachii.
Gerakan osteokinematika
Gerakan ekstensi• Yaitu gerakan lengan ke
belakang yang menjauhi dari posisi anatomis, bergerak pada bidang sagital.
• Otot penggerak utamanya adalah latissimus dorsi dan teras mayor.
• Sedankan pada gerakan hiper ekstensi teres mayor tidak berfungsi lagi, hanya sampai 90 derajat dan digantikan fungsinya oleh deltoid posterior.
Gerakan abduksi• Yaitu gerakan pada bidang frontal dengan axisnya horisontal.
• Otot penggerak utamanya adalah otot deltoid midle dan supraspinatur. Abduksi sendi bahu meliputi tiga fase, yaitu: abduksi 0’ – 90’ akan diikuti gerakan eksternal rotasi.
• Otot-otot yang berkerja pada fase ini adalah deltoid, seratus anterior, dan trapezius ascenden desenden. Gerakan ini dihambat oleh adanya tahanan peregangan dari latisimus dorsi dan pektoralis mayor.
• Abduksi 120’ – 180’ melibatkan otot deltoid, trapezius dan erector spine.
• Gerakan ini dikombinasikan abduksi, fleksi dan vertebra.
Gerakan adduksi• Yaitu suatu gerakan yang
merupakan kebalikan dari gerakan abduksi.
• Otot penggerak utamanya adalah pectoralis mayor dibantu oleh otot latisimus dorsi, teres mayor serta otot sub scapulari.
• Luas gerak sendinya pada bidang frontal.
Gerakan abduksi horizontalYaitu gerakan lengan yang mendekati tubuh dalam posisi abduksi lengan 90’ dan mencapai jarak gerak sendi 45’ yang dimulai posisi anatomis.
Gerakan adduksi horizontalYaitu gerakan lengan yang menjauhi tubuh dalam posisi abduksi lengan 90’ dan mencapai jarak gerak sendi 145’ yang dimulai posisi anatomis.
Gerakan eksorotasiYaitu gerakan sepanjang axis longitudinal yang melalui caput humeri. Gerakan ini dilakukan oleh otot infraspinatus, teres mayor dan deltoid posterior.
Gerakan endorotasiYaitu suatu gerakan yang merupakan kebalikan dari gerakan eksorotasi. Gerakan ini dilakukan oleh otot sub scapularis, pectoralis mayor, latisimus dorsi dan teres mayor
Gerakan sirkumduksiYaitu gerakan yang merupakan kombinasi dari semua gerakan di atas.
Pada gerakan arthrokinmeatika meliputi dua gerakan roll dan slide. Roll adalah suatu gerakan sendi dimana perubahan jarak titik kontak pada suatu permukaan sendi sama besarnya dengan perubahan jarak titik kontak permukaan sendi lawannya. Sedangkan slide adalah suatu gerakan sendi dimana hanya ada satu titik yang selalu kontak dengan titik-titik yang selalu berubah pada permukaan sendi lawannya.
Gerakan arthrokinematika
Pada sendi bahu meliputi :
1) Pada gerakan endorotasi caput humeris roll searah dengan gerakan endorotasi dan slidenya ke posterior.
2) Pada gerakan abduksi caput humeris roll searah dengan gerakan abduksi dan slidenya ke caudal.
3) Pada gerakan eksorotasi caput humeris roll searah gerak eksorotasi dan slide ventral agak medial
