5
Live Attenuated Vaccines
Viral measles, mumps,
rubella, vacciniavaricella/zoster yellowfever,rotavirus
intranasal influenza, oral polio
Bacterial BCG, oral typhoid
http://forghani.blogfa.com
6
Inactivated VaccinesCannot replicate
Generally not as effective as live vaccines
Less interference from circulating antibody than live vaccines
Generally require 3-5 doses
Immune response mostly humoral
Antibody titer may diminish with tim

General recommendations
in immunization
http://forghani.blogfa.com
12
(A)Nonsimultaneous Administration of Two Live Parenteral Vaccines
Interference can occur between two live vaccines given less than 28 days apart
If two live injected vaccines, or live intranasal influenza vaccine, are given less than 28 days apart the vaccine given second should be repeated
http://forghani.blogfa.com
13
(B)Minimum Intervals and
AgesUse when person is behind
schedule or vaccination series needs to be accelerated
Vaccine doses should not be administered at intervals less than the minimum intervals or earlier than the minimum age
http://forghani.blogfa.com
14
(C)Violation of Minimum Interval or Minimum
AgeACIP recommends that vaccine doses
given up to four days before the minimum interval or age be counted as valid
Immunization programs and/or school entry requirements may not accept all doses given earlier than the minimum age or interval
http://forghani.blogfa.com
15
(D)Extended Interval Between Doses
Available studies of extended intervals have shown no significant difference in final titer
It is not necessary to restart the series or add doses because of an extended interval between doses

The following vaccination procedures are recommended
1-Check availability of the protocols, equipment and drugs necessary for the management of anaphylaxis, before each vaccination session.
2-Maintain and monitor vaccine refrigerator, and other ‘cold-chain’ components, according to current recommendations, and preferably check prior to each working day.
3-Provide, to the person to be vaccinated, or that person’s parent/caregiver, appropriate information about the risks and benefits of vaccination and the risks of vaccine preventable diseases.
4-Perform a pre-vaccination assessment to determine the vaccinee’s medical fitness for vaccination. Any concern about the person’s eligibility for vaccination must be discussed with a medical practitioner, paediatrician or public health physician with expertise in vaccination. )See Appendix 1 for phone numbers for State/Territory public health authorities.( If a person’s health status or suitability for vaccination cannot be determined, defer vaccination.
5-Following the provision of appropriate information, )see 3. above( and the pre-vaccination assessment )4. above(, obtain valid consent from the person to be vaccinated, or from that person’s parent/caregiver. This should be documented.
6-Advise the person to be vaccinated, or that person’s parent/caregiver that the vaccinee should remain under observation in a designated place for a minimum of 15 minutes after the vaccination.
7-The schedule, dose, route and technique of administration of the vaccines must be in accordance with NHMRC guidelines. Note: each individual dose must be checked to see that the expiry date has not lapsed, and that there is no particulate matter or colour change in the vaccine.
8-Administer the vaccine)s(. Also check the vaccination status of other family members and offer catch-up vaccination where appropriate.
9-Dispose of needles, syringes and vaccine vials in accordance with standard infection control guidelines. 10-Advise the person, or the parent/caregiver of a child who has just been vaccinated, on the management of the
common adverse events that may occur following immunisation. It is important that they be given a contact phone number in case a significant adverse event occurs within 24 to 48 hours of the vaccination.
11-Before departure, inform the person or the child’s parent/caregiver, preferably in writing, of the date of the next scheduled vaccination.
12-Document the details of vaccination: )i( on a record to be retained by the person, or the parent/caregiver of the person; )ii( on the relevant clinical record; and )iii( on an ACIR )or equivalent( encounter form, for children under the age of 7 years.
13-Promptly report any significant adverse event following immunisation to the Adverse Drug Reactions Advisory Committee )ADRAC(, or in some instances to the relevant State/Territory Health authorities )see Part 1.6, ‘Adverse events following

سرما زنجیرهCOLD CHAIN
Vaccines are sensitive to heat and freezing and must be kept at the correct temperature from the time they are manufactured until they are used
40
COLD CHAIN The system used for keeping and distributing
vaccines in good condition is called the cold chain.
The cold chain consists of a series of storage and transport links, all designed to keep vaccines within an acceptable range until it reaches the user.
41
Health facility refrigerators may be powered by electricity, gas, kerosene, or solar energy. Electric refrigerators are usually the least costly to run and the easiest to maintain, but they must have a reliable electricity supply.
Refrigerators
42
Cold boxes A cold box is an insulated container
that can be lined with ice-packs to keep vaccines and diluents cold during transportation and/or short period storage (from two to seven days).
43
Vaccine carriers Vaccine carriers are
insulated containers that, when lined with frozen ice-packs, keep vaccines and diluents cold during transportation and/or temporary storage. They are smaller than cold boxes and are easier to carry if walking. But they do not stay cold as long as a cold box — maximum for 48 hours with the lid closed.
44
VVM A vaccine vial monitor (VVM) is a label that
changes colour when the vaccine vial has been exposed to heat over a period of time. Before opening a vial, the status of the VVM must be checked to see whether the vaccine has been damaged by heat.
45
CCM Usually the cold chain
monitor is only used for large shipments of vaccine. The same card should remain at all times with the same batch of vaccine. The change in color is cumulative and relates to heat exposure over the whole life of the shipment and not to a specific point in the cold chain.
46
Thermometers Health facility staff use dial or stem
thermometers to monitor the temperature of refrigerators.
47
Freeze watch
If the freeze indicator (Freeze WatchTM) is exposed to temperatures below 0ºC for more than one hour, the vial bursts and releases the coloured liquid, staining the white backing card.
48
Freeze Tag another type of freeze indicator called the Freeze-tag. It
consists of an electronic temperature measuring circuit with associated LCD-display. If the indicator is exposed to a temperature below 0°C ± 0.3°C for more than 60 minutes ± 3 minutes the display will change from the “good” status into the “alarm” status as indicated on the picture below. The indicator is used to warn of freezing and is packed with DTP, TT and DT vaccines as well as with hepatitis B. Shelf life is 5 years.
49
Recommendations
Slight heat exposure is less damaging than freezing.
2ºC – 8ºC margins is difficult to maintain especially for a kerosene refrigerator.
Do not adjust thermostat to a higher (cooler) setting after a power cut. This could freeze the vaccines.
Do not adjust thermostat to a higher setting when vaccines arrive. This could freeze the vaccines.
50
Shake Test Step 1 — Prepare a frozen control sample: Take a vial of vaccine of the
same type and batch number as the vaccine you want to test, and from the same manufacturer. Freeze the vial until the contents are solid (at least 10 hours at -10°C) and then let it thaw. This vial is the control sample. Mark the vial clearly so that it is easily identifiable and will not be used by mistake.
Step 2 — Choose a test sample: Take a vial (s) of vaccine from the batch (es) that you suspect has been frozen. This is the test sample.
Step 3 — Shake the control and test samples: Hold the control sample and the test sample together in one hand and shake vigorously for 10–15 seconds.
Step 4 — Allow to rest: Leave both vials to rest by placing the vials on a table and not moving them further.
Step 5 — Compare the vials: View both vials against the light to compare the sedimentation rate. If the test sample shows a much slower sedimentation rate than the control sample, the test sample has most probably not been frozen and can be used. If the sedimentation rate is similar, the vial has probably been damaged by freezing and should not be used.
51
52
Light sensitivity
Some vaccines are very sensitive to strong light and their exposure to
ultraviolet light causes loss of potency. Consequently, they must
always be protected against sunlight or fluorescent (neon) light. BCG, Measles, MR, MMR and rubella
vaccines are equally sensitive to light (as well as to heat).
