SUPPLIER CHANGE CONTROL
We recognize that continuous improvement efforts at our suppliers may require changes to manage cost, quality, delivery, and technology.
However, implementation of unapproved changes can have unintended effects on our products and therapies, our operations and, more importantly, our customers and patients.
Suppliers must notify Medtronic of changes made to materials, products, or processes.
ROLE OF THE SUPPLIER
Provide adequate advanced notification
Establish an effective process for triggering notification of changes to Medtronic
Define proactive process to identify and communicate changes from sub-tier suppliers
PROCESS IMPROVEMENT PLAN
We recognize that the current process is burdensome and time consuming
Medtronic is taking steps to standardize and simplify this process where possible Improve the inputs to the process by providing a
standard web based system for Supplier Change Request (SCR) submissions
Increase Supplier visibility to the process with automated notifications

Login to Portal
SubmitRequest
Monitor Status
1. Login into website: changerequest.medtronic.com
2. Complete Change Request & submit
3. Monitor Status - Medtronic will send you notifications for the following:
a) Receipt of completed request
b) When a Medtronic change owner is assigned
c) Approval or disapproval of change
CONTENT NEEDED FOR A CHANGE REQUESTChange Details
Information to describe the change and reason(s) why the change is being made
Change Evaluation What has been done to evaluate the change and effect on
product performance
Medtronic Impact What Medtronic sites and business units will be impacted by
the change The timing associated with the change
Supplier contact information Name, email address, phone number, etc.

If your quality agreement stipulates specific functions be notified of change requests (i.e. buyers, supplier owners), please list them in the Medtronic preferred contact section of form.
