During online registration, fill out a W-9 (U.S.A.) or TD-1 (Canada) form and submit it to Waterstone. 4. Start entering claims as soon as you send in the order. Must provide all required information when filing a claim. CLAIM REDEMPTION CRITERIA 1. All claims must be filed using the Waterstone Appreciation Rewards online program. 2.
